About this file
This is a Scope of Work amendment for a cost reimbursement contract between the Utah Department of Health and Human Services, Refugee Health and TB division, and Wasatch Homeless Health Care to provide housing for individuals experiencing homelessness with active tuberculosis. The contractor is responsible for maintaining a minimum of two housing units accommodating individuals with active TB for the duration of their treatment, utilizing various housing options such as apartments, motels, or trailers. The performance measure tracks the number of TB patients referred who were housed in designated units, with the contractor reporting all patient admissions, discharges, and available unit counts to the Department.
