MEMORANDUM FOR: Directorate Public Works (DPW)

ATTN: Joshua Mowery, Industrial Engineering Technician SUBJECT: Safety Office Review for Charpy Impact Tester

1. Purpose: Letterkenny Army Depot (LEAD) Safety Office review of one (1) Charpy impact tester. This machine is for impact testing of weld procedure qualification specimens required by Mil-Std 3040. This specification establishes the minimum requirements for the purchase of a Charpy Impact Tester. Specifications are based upon commercially available products and shall meet or exceed the minimum requirements specified in purchase description section 4.0. The system is comprised of both the machine and a mounting base.

The contractor will accept trade in of the old Charpy impact tester machine without the mounting base for a discount on the new tester. The new tester will be mounted on the old system’s base.

The contractor shall be responsible for informing all of his workers, direct or contracted, and ensure they follow the information within this purchase description specification.

The contractor shall also have a representative onsite to perform the setup, startup, and training of the equipment and be the point of contact (POC) to communicate and coordinate with Letterkenny’s POC.

This purchase order is for material, material delivery, testing, training and warranty.

A warranty is included in the purchase: The contractor shall furnish with the equipment a standard commercial warranty or full one year warranty (parts, labor, and shipping, at minimum), whichever is greater. The contractor shall provide commercial warranties for any subcontracted components that will be underwritten. The contractor shall supply all corresponding warranty contact names and phone numbers before the end of the project. The warranty shall be effective beginning immediately after final acceptance of the equipment and not before that time.

The Contractor shall supply all parts and materials, except Government furnished, necessary to perform the tasks specified in the contract. The Government will not be held responsible for damage to the Contractor's supplies, materials, equipment or personal belongings brought onto the installation by employees of the Contractor.

Foundation: The machine shall be level to within 3:1000 and securely bolted to a concrete floor not less than 150 mm thick, or when this is not practical, the machine shall be bolted to a foundation having a mass not less than 40 times that of the

SUBJECT: Safety Office Review Charpy Upgrade for Letterkenny Army Depot pendulum. The bolts shall be tightened as specified by the machine manufacturer. The Contractor will use the foundation provided by the Government.

Dust Control: The contractor shall control the amount of dust generated during the installation of the system foundation and components and shall prevent the spread of dust to occupied portions of the building. Use of water will not be permitted when it will result in, or create hazardous or objectionable conditions such as ice, flooding or pollution.

Installation of New Equipment: The contractor shall be responsible for supplying all parts, material, equipment, tools, and labor required to start up and set up the equipment provided under this contract. This shall include final leveling, checking utility connections, startup, and setup. The contractor shall remove and dispose of all packaging and preservative material. Removed components shall not be taken from the Depot property. Removed components shall be placed in a location designated by the

TPOC.

Fluids: The contractor shall provide all necessary fluids and chemical additives required for the start-up and proper operation of the equipment. The contractor shall supply LEAD with SDSs (per LEAD-R 385-1 and LEAD Safety Bulletins) for all the necessary fluids within 15 days calendar after the award of contract.

The Pendulum type tester of rigid construction for Charpy V-Notch impact testing with the machine mechanism shall be enclosed in a safety barrier that protects user from debris created by normal machines.

The machine safety barrier shall include machine guarding that meets or exceeds LEAD-R 385-1, Chapter 17-3, identifying and mitigating hazards associated with machinery at LEAD.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, nip/pinch points, rotating parts, flying chips, and sparks. Examples of guarding methods are physical barrier guards, electronics safety devices, guarding by distance, and physical restraints. The point of operation of machines whose operations expose an employe to injury shall be guarded. The guarding device shall be in conformity with any appropriate standard (OSHA (1910.211-1910.219), ANSI, Equipment Owner’s Manual) and meet or exceed all LEAD-R 385-1 requirements.

In the absence of machine guarding that meets or exceeds LEAD-R 385-1, guarding shall be designed and constructed (by LEAD Engineering) as to prevent the operator, or any employee who may come in contact, from having any part of his/her body in the danger zone during operations.

LEAD Engineering will complete a formal risk assessment (in accordance with LEAD- R385-1 is required) using a DD Form 2977 that has been reviewed by the chain of command, signed by the risk acceptance authority, and posted within Tab 22 of the cost center safety communication binder

All machines designed for fixed locations shall be securely anchored to prevent walking or moving during operation.

The equipment will generate a minimum energy capacity of 400J (300 ft-lbs.)

The equipment will have a motorized brake to stop the pendulum and a motorized return to the pendulum start position.

Calibrate IAW ASTM E23 - 2018, to include direct and indirect verifications IAW Annex A2 of ASTM E23. Calibration/verifications required after final installation.

Equipment Hazards: All hazards inside the machinery shall be guarded against unauthorized access. Clearly visible signs and symbols shall warn personnel in the area, and interlocks, safety mats and redundant deactivation systems shall guard against accidental access to the tooling and moving parts. Guards shall be easily removable to facilitate inspection, maintenance, or repair.

Mercury Restriction: The equipment shall not contain mercury compounds nor be exposed to free mercury during manufacture.

Asbestos Restriction: Asbestos and materials containing asbestos shall not be used in or on the equipment.

PCB Restriction: Equipment and materials containing Polychlorinated Biphenyl (PCB) shall not be shipped to Letterkenny Army Depot.

The New Equipment Requirements are listed in Section 4.0 of the purchase description.

2. The following information is recommended to be added to the Purchase Description (PD) and contract.

a. The contractor is reminded that LEAD maintains a Safety Management System (SMS) and is ISO 45001 certified, AS9100 and AS9110 certified.

b. The Contractor will meet or exceed all of the LEAD safety and Occupational Health Program LEAD-R 385-1, LEAD Safety Bulletins, Army Safety Program and Occupational Health Program AR-385-10, OSHA, NFPA, ANSI, 29 CFR Title 1910 and 29 CFR Title 1926. In the case of a conflict among these laws, regulations, and standards contact the COR for clarification.

Installation Resources\LEAD Safety and Occupational Health Program. The Army Safety Program and Occupational Health Program is AR-385-10.

d. As part of the applicability of engineering controls, all equipment with rotating and moving parts at the point of operation shall meet or exceed the LEAD 385-1 Chapter 17-3 machine guarding requirements.

e. The contractor shall meet or exceed LEAD lockout/tagout (LOTO) action and procedures for addressing and controlling hazardous energy during service and maintenance of machines and equipment in accordance with LEAD-R 385-1, Chapter 12. See LEAD Technical Point of Contact (TPOC) for the safe servicing and maintenance of machines and equipment, in which unexpected, energized equipment, damage to equipment or start-up of machine or equipment, or release of stored energy, could cause injury to employees.

f. Equipment installation/set-up, service, maintenance, and testing shall be conducted accordance with the equipment manufacturer specifications, and according to the frequency recommended by the manufacturer.

g. The Contractor shall ensure all employees and all associated sub-contractor’s employees possess all required licenses for operating any equipment used in the performance of this contract. The Contractor must ensure that any personnel performing under the contract are fully trained, licensed, certified, competent, and qualified for the position in which they shall be serving.

h. The contractor shall assess each worksite for hazards prior to the start of work.

i. All chemicals on LEAD worksites shall be vetted and approved by the LEAD

Hazardous Material Approval Group (HMAG) prior to use per LEAD-R 385-1 Safety Bulletin 23-02. Contractor (Labor workforce) employees shall comply with LEAD HMAG/HMID procedures.

j. A list of all approved hazardous materials and Safety Data Sheets (SDS) will be maintained at all worksites.

k. All contractor personnel shall wear the appropriate LEAD approved PPE when in the following work areas: industrial buildings, construction sites, test sites and test facilities as directed by LEAD-R 385-1.

l. Performance Requirement: All contractor personnel shall wear the appropriate LEAD approved PPE 100% as required by LEAD-R385-1.

m. Performance Requirement. Meet or exceed LEAD-R 385-1, the LEAD safety and Occupational Health Program.

n. If contractor personnel determine a procedure is unsafe, they shall discontinue work immediately and contact their Program Manager who shall notify safety personnel and the COR.

3. The LEAD TPOC shall ensure the Contractor Labor force provides a completed Army Material Letterkenny Depot (AMLD) Form 4733, LEAD Contractor Chemical Checklist, and include Safety Data Sheets (SDS) for all chemicals to be brought onto the Depot prior to performing work on-site in accordance with LEAD-R 385 Safety Bulletin 23-01. The

Directorate HMAG coordinator is the point of contact responsible for facilitation the HazMat validation. Approved LEAD SDSs shall be maintained at all worksites.

4. The LEAD Safety Office requests to be included in technical reviews, installation, and training.

5. If the requirements for this contract change, the proponent of the contract must submit all changes to the LEAD Safety Office for review.
