About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form required by the Incorporated County of Los Alamos Procurement Division in New Mexico for the Senior Services: Senior Center Operations, Home-Based Services, and Care Coordination solicitation. The form serves as a mandatory certification that offerors and bidders must complete and submit as part of their proposal response. The document requires authorized signers to attest that neither they nor any principals of their company are debarred, suspended, proposed for debarment, or voluntarily excluded from covered transactions by any federal, state, or local agency, and that they have not been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records within the preceding three-year period.
The certification additionally requires attestation that offerors have not violated contract provisions, had unsatisfactory performance records, violated ethical standards under the County Procurement Code, are not current or former County employees, are not immediate family members of County employees or public officials, and have not had public transactions or contracts terminated for cause or default within three years. Offerors unable to certify any statement must provide written explanation. The form explicitly states that falsification of any certification statement constitutes grounds for proposal rejection or contract award rescission, though acknowledgment of responsibility matters does not necessarily preclude consideration for award.
