About this file

This document is a Change Order for a contract between the Utah Department of Technology Services (DTS) and Aavenir, originally established on March 1, 2021, addressing the inclusion of a Payment Request Form (PRF) within the Invoiceflow system. The change order specifically targets a designated group of users and departments within SOU (State of Utah), introducing a new pop-up interface for payment-related information submission that will follow a defined approval workflow. The implementation will be isolated to the targeted users, ensuring existing Invoiceflow users and processes remain unaffected.

The change order involves 100 hours of effort, with a total pricing of $12,500, to be paid in two installments: 50% upon design and development completion of the Payment Request Form, and the remaining 50% upon completion of the Change Order. The document outlines that the deployment and testing will be conducted on SOU instances with SOU personnel during guided sessions, due to limited access to GSS (likely a system or platform). The change order was signed on July 30, 2025, by Jesal Mehta, CEO and Founder of Aavenir, and Tyrel King, Financial Manager II for the State of Utah, indicating mutual agreement and approval of the proposed modifications.
