Summary: DLA-20 National Council for Mental Wellbeing Consulting Agreement

This is a consulting agreement executed between the National Council for Behavioral Health (DBA National Council for Mental Wellbeing) and the Utah Department of Health and Human Services, effective May 1, 2026. The agreement provides for DLA-20 training and consulting services utilizing copyrighted assessment tools developed by Willa S. Presmanes and Jensen. The scope of work includes a DLA-20 Trainer Module covering one DLA-20 assessment tool, consisting of a train-the-trainer series for up to 12 staff members. Deliverables include one hour of pre-planning, four two-hour train-the-trainer sessions (eight hours total), review and individualized feedback on required assignments, and a comprehensive training packet including PowerPoint presentations, vignettes, handouts, and supplementary materials. The project will be completed within a three-month timeframe. The initial recertification fee shall be waived for the September 2027 recertification cycle, with standard biennial recertification fees beginning in September 2029.

The total fixed cost for consultation services is $10,000.00, payable at the project's beginning. The agreement permits the Utah Department of Health and Human Services to train one individual from each of Intermountain Health and University of Utah Health Care agencies to support their internal teams in DLA-20 tool usage. The contract restricts the client from training, certifying, or providing access to the DLA-20 tool or related materials to third parties without prior written consent from the National Council. Client's obligations include full disclosure of necessary information, compliance with applicable federal and state healthcare laws, and maintaining confidentiality of proprietary information for three years following termination. The agreement is governed by the laws of the District of Columbia and may be terminated by mutual written consent, with termination within thirty days requiring reimbursement of all incurred fees and expenses plus full payment for any unresched consultation dates.
