About this file
This is a certification forms document for Louisville Metro Government's Invitational for Bid (IFB) 260244, related to the Via-Smart Project in Jefferson County, Kentucky. The project addresses safety and reliability concerns across multiple viaducts by installing detection devices and equipment to alert the traveling public and government agencies of roadway flooding, approaching over-height vehicles, and completed over-height strikes. The certification forms establish the procedural and compliance requirements that bidders must fulfill as part of their competitive sealed bid submission, including acknowledgment of all addenda, bidder information documentation, authorized agent signatures, and Metro Louisville Revenue Commission and Federal ID Numbers. Bidders must also confirm registration in the federal Central Consultant Registration (CCR) system. The contract award will be made by Louisville Metro Government's Purchasing Director Kevin Moore according to the terms and conditions of the IFB, with a contract effective date and expiration date to be determined upon award.
Bidders are required to submit a proposal or bid bond in the amount of five percent of the total bid as a guarantee of performance. Within fifteen days of receiving notice of award, the successful bidder must execute the contract, provide performance and payment bonds, furnish evidence of required liability insurance, comply with process agent requirements, and submit a proposed work schedule. All bidders and subcontractors must certify compliance with applicable Kentucky statutes including those covering corporation and utility taxes, sales and use taxes, income taxes, wages and hours, occupational safety and health, unemployment compensation, and workers compensation. Additionally, bidders must certify non-collusion in bidding practices, that they have examined the project site and specifications, acknowledge receipt of all addenda, and perform no less than thirty percent of the total contract value themselves per Kentucky Standard Specifications for Road and Bridge Construction. All contractors must certify compliance with Build America, Buy America Act requirements and relevant federal regulations including 23 U.S.C. § 313 and 23 C.F.R. § 635.410.
