About this file
The document is a Proposer Information and Certification Sheet for an RFP (Request for Proposals) issued by the State of Oregon for the procurement of adult male and newborn high-fidelity simulators to be used in a College Simulation Lab for training nursing students. The RFP, numbered 21.22.01, is seeking proposals from vendors to provide this equipment to meet the growing safety training needs resulting from increasingly complex and acute health conditions in acute and community-based care settings. The RFP has a closing date for proposals, and the contract awarded will likely include renewal options.
The certification sheet requires the proposer to acknowledge receipt of any addenda, confirm the proposal is a firm offer for 180 days, agree to the contract terms, and certify compliance with various state laws and regulations related to non-discrimination, wage parity, and conflict of interest. The proposer must also certify that the contents of the proposal are truthful and accurate. No specific pricing terms, set-asides, incumbent information, or funding details are provided in this document.
