About this file
This is a compliance document for a Request for Qualifications (RFQ) issued by the City and County of San Francisco, Department of Public Health, for the establishment of a prequalified list of vendors to provide as-needed community outreach and engagement services under Sourcing Event ID SFGOV-0000011976. The document outlines mandatory compliance requirements applicable exclusively to non-profit organizations that receive a cumulative total of at least $250,000 annually in City funds or City-administered funds and qualify as non-profit organizations under Chapter 12L of the San Francisco Administrative Code. Non-profit entities meeting these criteria must submit responses addressing two specific items: a statement describing efforts to comply with Chapter 12L provisions regarding public access to meetings and records, and a summary and disposition of all substantiated complaints filed with the City within the previous two years concerning Chapter 12L compliance, with a statement confirming no complaints if none exist.
Failure to comply with Chapter 12L reporting requirements or material misrepresentation in submissions constitutes grounds for rejection of the response and potential termination of any subsequent agreement. Non-profit organizations that do not receive the minimum $250,000 in annual City funds or that do not meet the definition of a non-profit organization under Chapter 12L are not subject to these compliance requirements and may indicate this exemption on the form. Responders requiring additional space for explanations may attach separate documentation to support their submissions.
