About this file
This is a Minimum Qualifications Form for Category 4 of a Request for Qualifications (RFQ) issued by the San Francisco Department of Public Health (SFGOV-0000011976) for the establishment of a prequalified list of vendors to provide as-needed technical assistance and consulting services. The RFQ seeks qualified responders to deliver public health and health care program and operations implementation and project management services. The document outlines the specific expertise required for Category 4 services, which encompass complex public health initiatives such as disease outbreak control, injury prevention, vaccination campaigns, chronic disease prevention, and sexual health initiatives, as well as health care operational services including service line development, facility operations, program design, resource allocation, and organizational change management.
Responders must meet minimum qualification requirements to be deemed responsive and eligible for evaluation. Qualifying staff must possess a bachelor's or master's degree in public health or a related field and have completed a general project management or project management in public health certification course, or alternatively, demonstrate three years of project management experience in public health combined with completed coursework in project management. Failure to satisfy these minimum qualifications will result in non-responsive status and elimination from further evaluation stages. The form requires responders to print their name and provide a signature with date to certify that their staff meet these specified qualifications.
