About this file
This is a compliance document for a Request for Qualifications (RFQ) issued by the City and County of San Francisco, Department of Public Health, for the establishment of a prequalified list of vendors to provide as-needed community outreach and engagement services (Sourcing Event ID: SFGOV-0000011976). The document specifically addresses Chapter 12L compliance requirements that are mandatory for non-profit organizations only. Non-profit entities that receive a cumulative total of at least $250,000 annually in City funds or City-administered funds and meet the definition of a non-profit organization under San Francisco Administrative Code Chapter 12L must comply with the outlined provisions. Failure to comply with Chapter 12L reporting requirements or material misrepresentation in submissions constitutes grounds for rejection of the response and potential termination of any resulting agreement.
The compliance requirements mandate that eligible non-profits provide two items: a statement describing efforts to comply with Chapter 12L provisions regarding public access to the organization's meetings and records, and a summary and disposition of all complaints concerning the organization's Chapter 12L compliance filed with the City within the last two years that were deemed substantiated by the City. If no such complaints were filed, respondents must include a written statement confirming this fact. The document provides space for responses or allows respondents to attach separate documentation if additional explanation is needed. Organizations that do not receive at least $250,000 in annual City funds are exempt from these requirements and may indicate this exemption on the form.
