About this file

This is an Insurance Requirements sample document issued by the City and County of San Francisco for state and local contract opportunities. The document outlines mandatory insurance coverages that contractors must maintain throughout the full contract term. Required coverages include Commercial General Liability Insurance with limits of $1,000,000 per occurrence (including Abuse and Molestation coverage for services involving vulnerable populations); Commercial Automobile Liability Insurance at $1,000,000 per occurrence with Combined Single Limit; Workers' Compensation at statutory amounts with Employers' Liability Limits of $1,000,000; Professional Liability Insurance at $1,000,000 per claim; Technology Errors and Omissions Liability at $5,000,000 per claim; Cyber and Privacy Insurance at $20,000,000 per claim; and Pollution Liability Insurance with limits subject to risk manager review. All policies must name the City and County of San Francisco, its officers, agents, and employees as Additional Insureds and must be primary to any other available insurance.

The document specifies that contractors must provide thirty days' advance written notice of cancellation, non-renewal, or reduction in coverages, with ten days' notice for non-payment. Claims-made policies must remain in force for three years beyond the contract expiration date. All insurance carriers must have ratings comparable to A-, VIII or higher and be authorized to do business in California. Contractors must furnish certificates of insurance and additional insured endorsements before commencing services, and any lapse in required coverage will result in suspension of payment requests until reinstated coverage is provided. If subcontractors are used, they must provide equivalent insurance coverage and name both the City and the primary contractor as additional insureds. The City's Risk Manager retains authority to review and revise insurance amounts and requirements based on the risk profile of the services.
