About this file
The document is a Fork Maintenance Inspection Form from NAVSUP Publication 538, Sixth Revision, dated 11/15/02. The form is designed for comprehensive inspection of forklift forks, requiring a detailed assessment of various physical characteristics and conditions. The inspection must be conducted at intervals not exceeding 12 months or whenever a defect or permanent deformation is detected.
The form includes a comprehensive checklist for evaluating both left and right forklift forks, with sections covering manufacturer's safe working load (SWL) markings, surface cracks, blade and shank straightness, fork angle, fork tip height differences, blade thickness, positioning locks/latches, fork hooks, and plated fork conditions. Any detected defect is grounds for rejecting the equipment, and the material handling equipment (MHE) must be removed from service until the fork is repaired or replaced. The form provides spaces to document the manufacturer, model, year, serial number, and contract number, as well as inspection results, inspector details, and the next due date for inspection.
