About this file
This file contains Form 1295 instructions and definitions for Los Fresnos Consolidated Independent School District (C.I.S.D.) in Texas, establishing compliance requirements for vendors bidding on the CTE Center IP Intercom System Replacement project. All vendors submitting proposals must complete and submit the Certificate of Interested Parties Form 1295 electronically with the Texas Ethics Commission prior to contract execution. The form must be completed online at the Texas Ethics Commission website, printed with the certification number, and included with the proposal response. The submission must include the proposal number, contract or RFP name, and a description of services or goods provided.
The document defines key terms including "business entity," "controlling interest," and "interested party" to establish disclosure requirements. Vendors must report any individuals with controlling interests exceeding 10 percent ownership, board membership in entities with ten or fewer members, or officer positions. Intermediaries who receive compensation and communicate directly with Los Fresnos C.I.S.D. regarding the contract must also be disclosed. The regulation applies to all business entities regardless of profit status, though exemptions exist for publicly traded companies, utilities, sponsored research contracts, interagency contracts, and certain health and human services contracts where value cannot be determined at execution. The Texas Ethics Commission provides instructional videos and frequently asked questions resources to assist vendors in completing Form 1295 accurately.
