About this file
This is an Insurance Coverage Declaration form for Letcher County, Kentucky, in connection with an Invitation for Bid for Asbestos Abatement Services. The document establishes the mandatory insurance requirements that contractors must maintain and provide documentation for when submitting proposals for pre-demolition asbestos abatement and removal services at residential and commercial properties acquired by Letcher County through a voluntary property buyout grant program. Contractors must submit proof of all required insurance policies, including endorsements, with their proposals; failure to do so may result in proposal rejection. The declaration requires contractors to complete a coverage table identifying their insurance carriers, types of coverage, limits, and expiration dates.
The insurance requirements include three categories of coverage: Commercial General Liability with a minimum aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000; Comprehensive Automobile and Water Vehicle Liability covering any automotive equipment used in service performance with a minimum combined single limit of $1,000,000 per occurrence and Physical Damage Insurance for owned or rented machinery, tools, equipment, office trailers, and vehicles; and Worker's Compensation with employer's liability coverage of not less than $1,000,000 per occurrence for each accident or illness. The Worker's Compensation policy must include a clause prohibiting cancellation or material changes without thirty days' prior written notice to the County. Firms with owner/operators who have filed a "Notice of Election to be Exempt" must provide a signed copy of said notice demonstrating compliance with applicable Worker's Compensation law requirements.
