About this file

This is an Insurance Coverage Declaration for Letcher County, Kentucky, related to a Hazardous Materials Inspection Services contract. The document establishes mandatory insurance requirements that contractors must satisfy and submit with their proposals for consideration. Failure to provide complete insurance documentation may result in proposal rejection.

Contractors are required to maintain three categories of insurance coverage: Commercial General Liability with a minimum aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000; Comprehensive Automobile and Water Vehicle Liability with a minimum combined single limit of $1,000,000 per occurrence for any automotive equipment used in service performance, along with Physical Damage Insurance covering owned or rented machinery, tools, equipment, office trailers, and vehicles; and Worker's Compensation insurance with employer's liability coverage of not less than $1,000,000 per occurrence for each accident or illness. The Worker's Compensation policy must include a provision prohibiting cancellation or material changes without thirty days' prior written notice to the County. Owner/operators who have filed a Notice of Election to be Exempt from Worker's Compensation must provide a signed copy of that notice meeting applicable state exemption requirements. Contractors must submit copies of all insurance policies, including endorsements, with their proposals and identify the insurance carrier, type of coverage, coverage limits, and expiration dates for each policy.
