About this file

This is an appendix of fillable forms for a Request for Proposal issued by the Office of Procurement Services for Fairfax County Public Schools and the County of Fairfax, Virginia, to establish a contract for interscholastic volleyball officials services for boys and girls high school volleyball games. The document serves as a comprehensive bidder data sheet and certification package that all respondents must complete and submit with their proposals. Bidders must designate primary service and fiscal representatives with corresponding contact details and specify payment addresses if different from their primary business location. The document also includes fields for vendors to identify subcontractors, anticipated dollar amounts, and vendor classifications.

Bidders must also disclose any proprietary information requiring protection under Virginia Code Section 2.2-4342F and complete business classification designations (small business, minority-owned, or woman-owned). The document requires disclosure of any business licenses in Fairfax County and information regarding the percentage of work to be performed within the county. Additionally, bidders may indicate which local public bodies they authorize to use the established contract through the sample listing provided. All forms must be completed and returned with the bid submission to the Office of Procurement Services at 8115 Gatehouse Road, Suite 4400, Falls Church, VA 22042.
