Item Reference Question Government Response

099659‐Saniglaze

Is this a replacement to the Sherwin Williams B73 Latex Eg‐shell Epoxy that is approved at the facility? If not we need a lot more information.

Per the paperwork for Saniglaze, only a certified installer can apply this material. Also the Iowa City VAMC does not allow for spray finish in the main hospital. Please Advise

Use product as originally specified.

099100‐Harmony Line

We do not recommend the Harmony Line. We suggest basis of design to be SW Promar 200 Zero Latex Line. The Promar line has a light and dark base for accent colors where Harmony is limited to a light base. Harmony also has limitations to touch‐up where Promar line is excellent for touchup.

Change paint line to SW Promar 200 per owner.

Bid Docs please check that all Blueprint Documents Uploaded are full size. Lately they have been uploading the wrong size. The Bid docs were issued at 30 x 42. Check your properties on the pdf drawings

1‐8W‐MP101

There two VAV’s in the Pharmacy room tagged TU8W19B & TU8W19C that are not called out on the mechanical schedules. Are these new and if so can they be added in forthcoming amendment?

TU‐8W19B and TU‐8W19C are existing air terminal units that are to remain in place.

TU‐8W19D is an existing unit that is relocated.

M101 There are multiple grilles tagged R‐3 that are not scheduled. Will these grills be scheduled in forthcoming amendment?

RG‐3 shall be a 12x12 grille with blades @ 35" deflection and 3/4" on center. See included revised Air Device Schedule.

Ref Wall types Can the 8th floor to underside of deck height be provided? Generally, the bottom of slab is about 11’‐1”.

09 22 16 2.2

The non‐structural metal framing spec says the metal stud framing can be 30mil &/or 19mil as far as thickness. The wall types drawing 1‐8W‐A‐ 002 has most of the framing shown as 18ga which would be 43mils thick.

Which is correct?

Per the VA spec guidelines, metal stud framing for walls should be 33 mils (20 ga).

Several rooms appear to have gypsum ceilings but are not identified by keynote for removal. The new reflected ceiling plans shows ACT ceilings in these areas. Are these hard ceilings to remain ? If so will there be adequate space for ductwork, piping etc.?

Existing gypsum ceiling to be removed where new ACT is being placed. The plaster and drywall/joint compound were sampled and the laboratory results were No Asbestos Detected (NAD). The results can be found in the Limited Asbestos, Lead and Mold Inspection Report.

1‐8W‐AD‐108 Do all walls scheduled for removal contain lead paint?

The lead‐containing ceramic tile and lead‐containing ceramic cove base included in the abatement scope were the only items that had test results above regulatory limits. The results can be found in the Limited Asbestos, Lead and Mold Inspection Report. Trace amounts of lead below the EPA HUD definition of lead‐based paint were identified in the project areas. It should be noted that OSHA does not recognize this as a threshold value for lead exposure. Although the potential lead hazard for normal demolition or construction activities is low, actual lead exposure may vary significantly depending upon the specific task, procedure and other work area conditions. Proper OSHA compliance is considered performance based and includes evaluating potential work exposures with air monitoring or biological exposure monitoring (blood lead testing), if required.

1‐8w‐E‐601 states that panel 803H is to be removed and turned over to owner. Sheet 1‐8W‐E‐401 Keynote 3 shows panel 803H as being relocated.

Reviewed with VA electrical staff. Panel 803H is not required and can be deleted per E‐ 601.Not 3 on E‐401 should be clarified to denote location for relocated panel 8PW only.

1‐8W‐El101

Sheet note C. Does this note apply to lighting in room’s 8w19a/b/c/d‐ inpatient pharmacy and nurse station 8w13 as these rooms appear to be fed from a critical panel yet are not labeled as an “EM lights” and do not show a normal power lighting circuit.

Sheet Note C would not apply to the fixtures in 8w19 or 8w13, these are to be switched lights on the CE circuit as shown. Note C would apply to fixtures labelled as EM or EM/NL which could be switched in some applications. This would not apply to the 8W renovation at this time.

1‐8W‐FA101

Detail 1 fire alarm riser shows existing fire alarm Nac panel feeding the new Transponder in room 8w03. Sheet 1‐8W‐FA101 keynote 1 in key plan at lower left hand side of drawing also shows a keynote 1 to be placed on the east end of building.

The notation at the Key plan is referencing the approximate location of the existing FA panel (8e32).

1‐8w‐FA101 Is keynote 5 part of the alternate deduct or shall it be part of base bid work and not part of the alternate deduct

Keynote 5 details extension of central FA system to the renovation. It should be part of the base bid work.

1‐8W‐TK102

Key notes 2 and 12. Shall the work to relocate the Lenel panel be considered part of base bid and not part of alternate deduct as stated in note 12.

Relocation of the Lentel panel is only required if alternate is taken to include the work scope noted. Lentel panel would be relocated to allow installation of electrical equipment in the designated electrical closet under the alternate scope of work.

Are we to match existing interior signs on site for this project. Would you have some pictures of the signs we are try to match or colorized drawings?

Yes

1‐8W‐ ID401

Is CRR‐1 required in the pharmacy storage 8W19A B behind the racks?

See 16/ID401

Yes see drawings

1‐8W‐ ID401

Is there any Chair Rail (CR‐1) required on the project that was excluded from the drawings?

No Chair Rail

Can you clarify the wall demolition at room 8W20 & 8W21? It looks as if we demo the walls AD108, but then on the A108 they seem to be existing to remain?

Walls and door in the existing Toilet 8W20 are to be removed and constructed new.

8W20 will be renamed to Pharmacy HAC. Walls and door in HAC 8W21 are existing to remain.

1‐8W‐PD102

Is there a sink in room 8W18d that needs to be demo’d? We are removing the existing plumbing in the 8W18 wing and starting new, correct? Various missing notes on AD108 and PD102 for LAV’S & WC.

Remove sink and lavatories in 8W18 and 8W18D. Intent is that all plumbing fixtures be removed within the project area except where indicatred to be relocated.

General Typical clarifications/Information

What are the quality assurance requirements?

During the entire performance of this contract, until completed and accepted, Contractor shall provide a fulltime Superintendent who is exclusively dedicated to the supervison of this project. Superintendent is to meet all construction compentcy requirements, including OSHA credentialing and have satisfactory performance as determined by the Certifying Office. (OSHA 10 required for any workman on a contructions site and OSHA 30 for Superintendent)

Are there any unique requirements for the As‐Built Drawings/Safety Data Sheets and Security Plan?

The Superintendent shall maintain two full size sets of as‐built drawings which will be kept current during construction of the project, to include all contract changes, modifications and clarifications. The Superintendent is required to have MSDS ’s of all hazardous materials on site. The Superintendent is required to maintain a Safety and Security plan. Plans need to be unique to the current project – No Generic Plans!

What are the quality assurance requirements?

Superintendent’s responsibilities are to assure your subs are installing per the construction documents. Throughout the specifications it talks about Quality Assurance – provide a Quality Assurance plan to include coordinating the site with your subs, providing coordination drawings and scheduling them to do work. i.e. If your sprinkler sub puts his pipe where the VAV is supposed to go and makes it inaccessible for maintenance. To the VA, this is a coordination and quality assurance issue on the prime . Also, provide coordination drawings, to include elevations, sizes of duct work, pipes, conduits, VAV’s, FCU’s etc. Don’t do the work then come back and tell us it doesn’t work! Lack of Coordination and Quality Assurance only creates delays and needless change orders

Are there are any unique Infection Control requirements at Iowa City?

Record as part of Superintendents daily Quality Assurance Plan.

‐ Negative Air Hepa Filters need to be cleanedonce a week, if not more.

‐ Paths of egress to remain clear and free from obstacles.

‐ Sweep inside your construction site daily with sweeping compound.

‐ Insure sticky matts are available and changed regularly throughout the day ‐ Contain construction waste in tightly covered container.Secured to transport vehicle.

‐ Insure outside of the construction area is free of dust debris. This includes to and from the site, inside elevatorss, corridors, loading docks and dumpsters.

What are the requirements for daily logs and payrolls?

superintendent are to provide detailed Daily Logs to include Name of cubsontractor, Subcontrators employee name, Where and what they are working on, Superintendents name, date and time. Payrolls should be verrifiable through the Daily logs

Will more than one set of construction keys be provided for this project

One set of keys will be issued to the Full time Superintendent. Plan on using combination locks that accept a Best 7 pn core. Combinations cannot be written anywhere on the outside of the construction area.

Are trash chutes allowed during demolition?

There is no feasible place on the westside of the facility to place a trash chute. All debris must be taken down through the freight elevator on covered carts.

What is required for GEMS documentation?

You are required to separate rubble and recyclables and provide documentation on how much. Don’t wait until the end of the project! See the attached GEMS Training for Contractors. If you use our dumpsters there is a requirement to separate wood, cardboard, metal and general trash. If there is cardboard in any of the dumpsters the city landfill with not take them per City of Iowa City Ordinance. Also, if you get caught placing cardboard or not separating materials ‐ dumpster privileges may be taken away. Note: We have security cameras looking at our dumpsters and your superintendent will be responsible to dumpster dive and separate correctly.

In the pre‐construction meeting it was said that the basement work needs to be completed before any work is done on 8W. Is this correct?

Correct, the basement mechanical work is required to be complete before 8W work can happen. The basement compounding pharmacy needs to be fully operational.
