1.0 BACKGROUND

Citadel Environmental Services, Inc. (“Citadel”), was contracted by the Veterans Administration San Diego Healthcare System (VASDHS) (the “Client”), to provide asbestos and moisture consulting services in conjunction with a large scale moisture intrusion event impacting several areas on the west wing, Basement through Fourth Floor, of the Veterans Administration San Diego Medical Center (VASDMC), Building #1 (the “Building”).

The VASDMC consists of two buildings, (Building #1 and Building #2) reportedly constructed between 1968 and 1972. The buildings are concrete and steel frame structures. Building #1 is a six-story structure including a Penthouse and Basement Level, with interstitial floor space at each floor above the drop ceilings that house the mechanical systems for each floor. The total area of the buildings is estimated to be 1,045,000 square feet. Asbestos-containing spray applied fireproofing materials have been identified throughout the interstitial floor spaces as identified in the facilities Asbestos Operations and Maintenance Program (O & M).

Citadel understands that a fire sprinkler located inside the 4-West (4W) DLO Project Area was damaged just after midnight on August 7, 2015. Asbestos abatement of flooring materials was underway in the 4W DLO Project Area, when the Contractor (Unlimited Environmental) moved a scaffold system, snapping a sprinkler head and causing water to flood for approximately one (1) hour before being shut off. The fire suppression loop has not been recently (within the past 3-months) flushed therefore this was classified as a black water loss due to the potential for pathogens to be present in the water. The water ran down via interstitial floor spaces (INT) to 3- West (3W), 2-West (2W), 1-West (1W), Stairwell 1 and the Basement Kitchen.

2.0 SCOPE OF SERVICES

Citadel Environmental Services, Inc. (Citadel) was contacted by Veterans Administration San Diego Healthcare System (VASDHS) (the “Client”) to provide asbestos and moisture consulting services.

The scope of the investigation included:

1) Performing an initial visual inspection and outlining a scope of work for the mitigation efforts, limited to impacted areas of 4W, 3W, 3W-INT, 2W, 2W-INT, 1W, 1W-INT and the Basement Kitchen, to identify moisture-impacted and/or microbial-impacted materials;

2) Conducting area (air) monitoring to determine ambient airborne fiber concentrations (using phase contrast microscopy) in occupied moisture-impacted areas;

3) Collecting water samples from pooled/standing water in moisture-impacted areas to determine fiber concentrations and submitting the samples to an independent accredited laboratory for analysis; and

4) Providing a written report to include all field documentation, moisture maps, photographic documentation, observations, methodologies and recommendations for the remediation.

Citadel’s Senior Industrial Hygienist, Mr. Chris Roberts, and Field Representative, Mr. Josh Hoover, conducted the initial investigations on August 7 and 8, 2015. Mr. Roberts is a California-Certified Asbestos Consultant (CAC#08-4450) and Mr. Hoover is a California-Certified Site Surveillance Technician (CSST #14-5288). All work was performed under the general supervision of Ms. Michelle Campbell, an ABIH Certified Industrial Hygienist (CIH #10996 CP) and Certified Asbestos Consultant (CAC # 09-4519). The Contractor contracted the services of Patriot Environmental (Patriot), to provide an assessment of moisture-impacted materials. Citadel’s assessment was independent of Patriot’s investigation.

3.0 FIELD AND ANALYTICAL METHODOLOGY

Moisture Investigation

Citadel conducted visual inspections for signs of visible moisture damage and/or suspect microbial growth (SMG). The visual inspections were performed on August 7 and 8, 2015, and were limited to the following moisture-impacted areas (occupied areas):

4W – Corridor 4039, Room 4513, Room 4506, Corridor 4053, Room 4040, Room 4501, Room 4500, Room 4069, Room 4066, Room 4070, Room 4070C, Room 4070A, Room 4058, Room 4059, Room 4520, Room 4078, Room 4078A, Room 4079, Room 4035, Room 4022, Room 4039A and Nuclear Medicine.

3W – Room 3056, Room 3058, Room 3060, Room 3053, Room 3065, Room 3068, Room 3064, Room 3063A, Room 3051, Room 3078, Room 3076, Room 3074, Room 3074A, Room 3073, Room 3083 and Room 3046A.

2W – Room 2088, Room 2086, Room 2085, Room 2040, Room 2092, Room 2094, Room 2096, Room 2084, Room 2082, Room 2081, Room 2098, Room 2080, Room 2068, Room 2077, Corridor 2108, Room 2113, Room 2111, Room 2108, Room 2058, Room 2056, Room 2054, Room 2052, Room 2050, Room 2049, Room 2047, Room 2046, Room 2069, Room 2068, Room 2048, Room 2063, Room 2067, Room 2064, Room 2061, Room 2062

1W – Room 1230, Room 1229, Room 1231A, Room 1227, Corridor 1233, Room 1225, and Room 1224, and Room 1220.

Basement – Corridor B-125, Room B-134, and Corridor B-140.

Stairwell 1 – Floors Basement through Fourth.

Citadel also conducted visual inspections of the 1W-INT, 2W-INT and 3W-INT floors.

In moisture-impacted areas, Citadel employed the use of a pin-type moisture meter (Delmhorst BD-2100) and a FLUKE® TiR infrared camera to check potentially impacted surfaces for relative moisture content. Materials were considered wet or impacted if their moisture content was greater than that of like materials from non-impacted areas of the building.

The Delmhorst uses two pin electrodes to detect differences in the capacitance of the signal to determine water content when the pins are inserted into the material so that they penetrate the surface of building materials. The FLUKE® TiR infrared camera is a non-contact device that detects infrared energy and converts the measurements to temperature readings. The camera then converts those temperature measurements into a thermal image on a video monitor which may also be saved as a jpg image.

The Delmhorst is designed to measure moisture levels in wood, concrete, EIFS, sheetrock, and other materials. It has a 6-40% moisture range for wood, a 0.2-50% range for gypsum and a 0 to 100

3350 LA JOLLA VILLAGE DRIVE

5137 1182 0_VASD_4W_DLO_Sprinkler_Break_Limited_Moisture_Investigation_Report_CR|VASDHS 3 relative scale for all other non-wood materials. Regarding Sheetrock, readings below 0.5% indicate a dry condition, those between 0.5% and 1% correspond to borderline conditions, and readings over 1% indicate wet conditions. Elevated readings may represent ambient humidity, minor moisture intrusion, or the current status of a material previously wetted that has since partially dried out.

Air Monitoring – Fiber Concentrations

Citadel collected area (air) samples at Client designated locations throughout occupied moisture-impacted areas. As the samples were collected, the location of each air sample was marked on floor plans provided by the Client. Drawings indicating each air sample location can be found in Appendix B.

Samples of airborne fiber levels were collected onto 25-millimeter, 0.8 micron pore, mixed cellulose ester filter membranes contained in conductive cassettes. The extended cowl of the cassette remained open-faced and positioned downward at a 45 angle throughout the sampling period.

Air sampling pump flow rates were documented at the beginning and end of the sampling period using a rotometer calibrated against an intermediate flow standard.

Air samples were analyzed on site by Phase Contrast Microscopy (PCM) at 400X magnification according to the National Institute of Occupational Safety and Health (NIOSH) Method 7400, "A" counting rules. PCM analysis "counts" all structures with a length greater than 5 microns, and a length-to-width ratio of greater than 3:1. It is possible, therefore, that non-asbestos, fibrous dust such as carpeting, fiberglass, and cellulose fibers were "counted."

Alternative Method of Remediation:

Citadel has utilized ThermaPureHeat® Technology to “super heat” and pasteurize stairwell’s to avoid compromising the integrity of fire rated walls. This would be considered an acceptable alternative method of remediation for this area.

3350 LA JOLLA VILLAGE DRIVE

5137 1182 0_VASD_4W_DLO_Sprinkler_Break_Limited_Moisture_Investigation_Report_CR|VASDHS 25

Interstitial Floor Spaces

In addition to assessing occupied areas of the VASDMC, Citadel conducted a walkthrough inspection of the interstitial floor spaces as follows: 1W-INT; 2W-INT, and 3W-INT. The following observations were noted:

3W-INT: saturated spray applied fireproofing and plaster in multiple locations totaling approximately 500 square feet;

2W-INT: saturated spray applied fireproofing and plaster in multiple locations totaling approximately 400 square feet; and

1W-INT: saturated spray applied fireproofing and plaster in multiple locations totaling approximately 175 square feet.

The INT’s are not designed for occupancy and are classified as a regulated area due to the presence of asbestos-containing spray applied fireproofing. As such, any employees, contractors and/or vendors entering are required to don appropriate personal protective clothing and respiratory protection. Additionally, the INT’s house mechanical equipment and often have a significantly higher ambient temperature than the occupied areas of the medical center. The medical center operates on a single pass (100% exhaust) HVAC system with little to no potential for INT air to enter occupied areas without a physical disturbance being created (e.g. removing ceiling tiles). Based on this information, Citadel is recommending that the moisture-impacted fireproofing and plaster materials be “fogged” with a suitable anti-bacterial cleaning agent and then left to dry (without the aid of drying equipment), with progress tracked on a weekly schedule.

General Recommendations for the Remediation of Moisture-Impacted Materials

The following recommendations pertain to the removal of moisture-damaged and/or microbial-impacted building materials:

Removal of moisture-impacted and/or microbial-impacted materials should be performed in accordance with standard industry practice (IICRC S500 Standard & Reference Guide for Professional Water Damage Restoration & IICRC S520 Standard & Reference Guide for Professional Mold Remediation), by an experienced remediation contractor, utilizing negative pressure containment(s).

Consult the VASDMC O & M Program to identify asbestos-containing materials and their location.

All asbestos removal operations shall be performed by a Cal/OSHA-DOSH-registered and California-licensed asbestos contractor. All disturbances of asbestos-containing materials, and/or abatement operations, should be performed under the surveillance of a third-party Cal/OSHA Certified Asbestos Consultant retained by the Client.

All disturbances of asbestos-containing materials, and/or abatement operations, must be performed in accordance with the Cal/OSHA requirements set forth in 8 CCR 1529. Given the location of the subject facility, all asbestos abatement must also be performed in accordance with San Diego Air Pollution Control District (SDAPCD) requirements.

Finally, notification of the presence and location of asbestos-containing materials shall be made to all employees and vendors who work within the VASDMC, in accordance with California Health and Safety Code, Section 25915, et seq. (also known as Connelly Notification Bills).

Please note that the actual scope of work is subject to change based on actual field conditions once intrusive work (removing drywall, etc.) is completed.

Contact Citadel for final visual inspection, air monitoring (asbestos) and swab sampling (sewage indicator organisms - as deemed necessary) prior to releasing areas for occupancy.

6.0 RESULTS

Air Monitoring

Air samples were collected from designated occupied moisture-impacted areas to determine airborne fiber levels. Non-aggressive sampling techniques were utilized.

Results of the air samples indicated that airborne fiber levels on August 7, 2015, following the moisture intrusion event remained below the Environmental Protection Agency's recommended re-occupancy level of 0.010 fibers per cubic centimeter of air (<0.010 f/cc). Air sample data can be found in Appendix D.

Water Sampling

Per the Client’s request, water samples were collected from standing/pooled water in occupied areas for the determination of asbestos structures. Composite samples were collected to ensure the minimum sample volume of one (1) liter required for analysis was achieved. All results were reported as non-detected for asbestos fibers. Results are summarized in Table 3.0 below.

Laboratory reports are presented in Appendix E.
