The purpose of this amendment is to make the following changes:

1. Attach Q&A Responses;

2. Attach “Price Schedule” Attachment E

3. To revise or remove language from solicitation.

4. Add due date for General RFP Questions/Clarifications of 8/29/2019

5. Extend the date for Receipt of Proposals to 9/24/2019

1. The following language in Section A.3 (d) is hereby deleted:

This acquisition will be conducted using tiered Cascading Set-aside Procedures (IAW VHA Memorandum #17-09, June 21,2017 and in compliance with 38 U.S.C 8127) to include only SDVOSB, VOSB, and Small Business firms (See Section E. 8).

Is hereby replaced with the following language:

4. The following language in Section B.5 is hereby deleted:

B.5 MINIMUM WARRANTY REQUIREMENTS

The warranty requirements listed in the minimum technical requirement (See attachment D) are considered minimum, unless otherwise stated. If the manufacturer’s/vendor’s standard warranty exceeds the specified warranty, the manufacturer’s/vendor’s standard warranty will apply. An Offeror’s warranty terms that are unfavorable or places greater restrictions on the Government than stated in FAR 52.212-4(p) will not be accepted.

And is hereby replaced with the following language:

B.5 MINIMUM WARRANTY REQUIREMENTS

The warranty requirements listed in the minimum technical requirement (See attachment D) are considered minimum, unless otherwise stated. If the manufacturer’s/vendor’s standard warranty exceeds the specified warranty, the manufacturer’s/vendor’s standard warranty will apply. An Offeror’s warranty terms that are unfavorable or places greater restrictions on the Government than stated in FAR 52.212-4(o) will not be accepted.

5. The following language in Section B.22 is hereby deleted:

TECHNOLOGY REFRESH AND EXCHANGE

(a) All items/devices offered shall be state-of-the-art technology. State-of-the-art is defined as the most recently designed components and products in current production that are announced for marketing purposes, available, maintained and supported in accordance with requirement specified in the solicitation. Absolute components and products are not acceptable.

(b) In the event a manufacturer announces that contract products, devices and parts are to be upgraded, discontinued, or are discontinued, within the first year of contract award, or the first six months of any exercised option, the VA CSAS shall have the following right under the contract with regard to items it has ordered from the contractor.

The VA CSAS, at its option, may exchange, at no cost to the VA CSAS, the previously ordered products, devices and parts for the manufacturer’s upgraded items. VA DLC/SDC will pay for return shipping costs on exchanged products, devices, and parts for the manufacturer’s upgraded items.

(c) If upgrades become available after award of this contract, the contractor may request that the upgraded items be substituted in the contract for the awarded items per paragraph (d) below. The contractor will add or substitute them for the awarded items following evaluation and approval by the technology refresh process.

(d) The contractor shall provide the following information to the Contracting Officer:

(i) A list of specific awarded items that the contractor proposes to update.

(ii) Product literature for the new item(s), a detailed description of the differences between the awarded item(s) and the new items(s) and a specific analysis of the comparative advantages and disadvantages of the new item(s).

(iii) A pricing proposal for the proposed substituted items.

(e) If the Contracting Officer agrees to any substitution, a bilateral contract modification adding the upgraded items to the contract shall be required prior to any orders placed for the new item.

(f) New technology products shall be available for a period of no less than 1 year.

And is hereby replaced with the following language:

TECHNOLOGY REFRESH AND EXCHANGE

(a) All items/devices offered shall be state-of-the-art technology. State-of-the-art is defined as Products in current production that are currently available from the manufacturer. Obsolete components and products are not acceptable.

(b) In the event a manufacturer announces that contract products, devices, and parts are to be upgraded or discontinued, or are discontinued within the first year of contract award or the first six months of any exercised option period, the contractor shall propose a replacement item that meets or exceeds the contract requirements. The VA CSAS and the contractor shall agree on a fair and reasonable price for the replacement item.

(c) If upgrades become available after award of this contract, the contractor or the Contracting Officer may request that the upgraded items be substituted for the obsoleted item in the contract. The contractor will add or substitute them for the awarded items following evaluation and approval by the technology refresh process.

(d) During the technology refresh process, the contractor shall provide the following information to the Contracting Officer:

(i) A list of specific awarded items that the contractor proposes to update.

(ii) Product literature for the new item(s), a detailed description of the differences between the awarded item(s) and the new items(s) and a specific analysis of the comparative advantages and disadvantages of the new item(s).

(iii) A pricing proposal for the proposed substituted items.

6. The following clauses at section C.15 of the solicitation under clause 52.212-5 is hereby checked and incorporated by reference:

7. The following language in Attachment D Minimum Technical Requirements for FACIAL INTERFACE LINERS that reads as follows is hereby deleted:

Offerors will submit documentation that products meet United States FDA 510K Standard (“FDA Standard”).

8. The following language in Attachment A “ DETAILED INFORMATION FOR CONTRACT LINE ITEMS” is deleted:

9. The following language in Section E.1 (f) of the solicitation that reads as follows is hereby deleted:

Please Note: Offerors are encouraged to propose the widest practicable variety of makes, models, sizes, and quantity-per-package for mask liners, and for various makes, models, sizes, and styles of facial interfaces.

10. The following language in Section E.1 (h) of the solicitation is deleted:

(h) Associated Accessories: Associated accessories are limited to the following: Chin Straps and Tubing. Additionally, the replacement parts are limited to the following: Power Cords, Filters, Cushions, Power Supply, Humidifier Water Chamber, and Secured Digital (SD) Cards.

Associated accessories and replacement parts will only be considered for a corresponding CLIN for a PAP device(s) and/or Facial Interface(s), if an award is made for a PAP device(s) and or Facial Interface(s). For example, award for associated accessories and replacement parts will only be considered for CLIN 0001, if a PAP device is awarded for CLIN 0001. If applicable, replacement parts shall be compatible with offered and awarded PAP device model(s).

And hereby replaced with the following language:

E.4 SPECIFIC INSTRUCTIONS TO OFFERORS REGARDING PROPOSAL PREPARATION

(a) Offerors shall provide information as described below. Offerors that fail to submit complete information in the manner described below, for either the Business or Technical Proposal, may be excluded from consideration for failing to meet a requirement of this solicitation.

The offeror shall submit its proposal in two SEPARATE volumes in the format and quantities described below. The volumes shall be legible, organized, and marked Volume I - Business and Volume II – Technical, respectively. The offeror must submit both hard paper copy and Compact Disk Read Only Memory (CD/DVD-ROM) of its proposal in the numbers specified below. The CD/DVD-ROMs shall not be password protected or encrypted. Please Note: Due to VA information technology policies offers may not be submitted on “thumb drives.”

(b) The offeror shall submit the hard copy and CD-ROMs containing the volume section copies in a sealed envelope or box. The envelope or box shall be addressed to the following address and shall show the time specified for receipt, the solicitation number, and the name and address of the offeror:

(c) Section I – Volume I-Business shall contain the following information:

(1) Standard Form 1449: Properly completed and signed Standard Form 1449 and all relevant solicitation attachments.

(2) Amendments: Acknowledgement of amendments, if applicable.

(3) Identifier: Unique entity identifier/tax identification/DUNS number (see section A.4).

(4) Price Schedule: Worksheets for all offered CLIN(s) (See Attachment E “Price Schedule”) and associated accessories/replacement parts, must be completed using Attachment E “Price Schedule” and submitted with proposal. Unit prices for each offered product must be provided in the “Price Schedule”. The worksheets are in Microsoft Excel format and shall be submitted in Excel as well. Offerors may also submit a .pdf version if desired. The price schedules shall not be altered except for entering product prices and entering product information such as make/model and descriptions.

(5) Facial Interface Sizes: Offerors shall provide pricing for all sizes within each CLIN item/model/kits offered for Facial Interfaces and Mask Liners as customarily available in the commercial marketplace. For example, if the item/model/kit sizes customarily available in the commercial marketplace are Extra Small, Small, Medium and Large, then the offeror SHALL provide pricing for Extra Small, Small, Medium and Large.

(6) Completing the Schedule of Items and Associated Attachments: The offeror is required to offer prices for each item offered for the base year and four option year periods identified in the Schedule of Items. Prices awarded for the base and option year periods will remain in effect for the respective contract base year or option periods, if exercised, without an increase. Please see the solicitation’s Attachment E for Excel “Price Schedule”. Offerors that propose for partial years or for less than the base and all four (4) option years shall be ineligible for contract award. Evaluation of option periods and award of option year pricing shall not obligate the Government to actual exercise of any option(s) periods.

(7) Representations and Certifications: Completed offeror’s Representations and Certifications (E.17, if not completed in SAM) and registration in the System for Award Management (SAM) Database at www.sam.gov. Offer shall provide a response to paragraph (b) of 52.212-3. In addition, offerors must complete and submit the certifications found in this section E, at paragraphs E.6, E.7. E.10, E.11 and E.16.

(8) Manufacturer/Distributor: If other than the manufacturer, the offeror shall submit, with the offer, a letter indicating that the offeror is an authorized distributor for the products offered. The offeror, if other than the manufacturer, shall submit a letter of commitment from the manufacturer, which assures that the offeror’s source of supply is sufficient to satisfy the Government requirements for the duration of the period, to include base and option periods. The letter must be from the manufacturer on company letterhead and noting the commitment by the product name, brand, and origin of the product. It should also provide an overview of how this business relationship will work between the manufacturer/distributor. Offers from manufacturer/distributor that do not submit the letter of commitment with all the information noted above may be excluded from further consideration.

(9) Catalog/Price List: The offeror must provide a current commercial catalog or price list that demonstrates the items are available commercially.

(10) FSS Price List: If item(s) offered are on a Federal Supply Schedule (FSS), submit a copy of your FSS price list.

(11) Vetbiz: Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) Offer: Offerors asserting they are SDVOSB or VOSB owned business concerns must be registered in the Vendor Information Page (VIP) at the time the offer is submitted. The VA web site for VIP registration is www.vip.vetbiz.gov. Registration in VIP will be required for SDVOSB or VOSB wanting to be considered.

(d) Section II – Volume II – Technical/Minimum Requirements:

(1) All offerors shall provide a Technical Proposal to be considered for award. The Technical Proposal must provide sufficient information to demonstrate the capability of satisfactorily meeting the Government’s requirements detailed in this solicitation (See Attachment D “Minimum Technical Requirements”). At a minimum, the Minimum Technical Requirements Proposal must include the following:

(i) For all products that are not listed in Attachment F “Approved Product List”, the offeror shall submit an Attachment A “Detailed Information for Contract Line Items” and provide standard product documentation and product descriptive literature such as published specifications, product brochures, advertising materials or other materials that fully describe the offered products and their respective features, capabilities, functions, compatibility features, sizes, lengths weights, colors, and fabric/materials. Failure to do so may result in a rejection by the Government of all or part of the offer.

(ii) Descriptive Literatures: For the purpose of determining technical acceptability, descriptive literature must identify the appropriate model/brand, and type to clearly demonstrate that the items offered meet the product descriptions (both minimum technical requirements and minimum functions) listed in the Minimum Technical Requirements, “Attachment D”, Schedule of Items. Notes within a manufacturer/dealer catalog do not meet this requirement. Information that is not marked properly could cause offered CLINs to be considered technically unacceptable. If the descriptive literature does not adequately cover the information the offeror should provide a written proposal documenting their offered products capabilities. The technical proposal, product documentation and descriptive literature will be used when evaluating the items.

(iii) Size Description: If the size descriptions listed for any of the offered items utilize different language to describe a size other than what is used by Industry or the offeror, the offeror can submit a comparable sized item. If a comparable size is offered, the offeror shall provide a written explanation in the solicitation’s “Attachment A” describing the equivalent size for the corresponding CLIN, as listed in Attachment D “Price Schedule.

(iv) Warranty Terms: The offeror must provide the terms of any warranty covering the offered products for review by the Government. Please Note: The warranty requirements listed in the minimum technical requirement (See attachment D) are considered minimum, unless otherwise stated. If the manufacturer’s/vendor’s standard warranty exceeds the specified warranty, the manufacturer’s/vendor’s standard warranty will apply.

(v) Approved Product List: Offerors shall identify which items are being offered that are listed in the Approved Product List (Attachment F) by highlighting all devices and associated accessories/replacement parts offered.

(vi) Kits: Offered kits (See Section E.1(m)) shall include a complete explanation of the kit’s contents in attachment A.

(vii) Digital Photograph: A digital photograph of each item offered must be provided on a CD-ROM. If possible, color photographs are preferable. Photographs should be multiple angles of the items to highlight details on various sizes. The resolution must show detail at 4”X4” + 1” (640X480 is ideal).

And is hereby replaced with the following language:

E.4 SPECIFIC INSTRUCTIONS TO OFFERORS REGARDING PROPOSAL PREPARATION

(a) Offerors shall provide information as described below. Offerors that fail to submit complete information in the manner described below, for either the Business or Technical Proposal, may be excluded from consideration for failing to meet a requirement of this solicitation.

Any offerors that do not provide pricing for all sizes or equivalent sizes of Facial Interfaces and Facial Interface Kits size configurations that are available from the manufacturer for the particular Make and Model Offered may be excluded from further consideration.

(6) Completing the Schedule of Items and Associated Attachments: The offeror is required to offer prices for each item offered for the base year and four option year periods identified in the Schedule of Items. Prices awarded for the base and option year periods will remain in effect for the respective contract base year or option periods, if exercised, without an increase. Offers from manufacturer/distributor that do not submit the letter of commitment with all the information noted above may be excluded from further consideration.

(9) Catalog/Price List: The offeror must provide a current commercial catalog or price list that demonstrates the items are available commercially.

(10) FSS Price List: If item(s) offered are on a Federal Supply Schedule (FSS), submit a copy of your FSS price list.

(11) Vetbiz: Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) Offer: Offerors asserting they are SDVOSB or VOSB owned business concerns must be registered in the Vendor Information Page (VIP) at the time the offer is submitted. The VA web site for VIP registration is www.vip.vetbiz.gov. Registration in VIP will be required for SDVOSB or VOSB wanting to be considered.

(d) Section II – Volume II – Technical/Minimum Requirements:

(1) All offerors shall provide a Technical Proposal to be considered for award. The Technical Proposal must provide sufficient information to demonstrate the capability of satisfactorily meeting the Government’s requirements detailed in this solicitation (See Attachment D “Minimum Technical Requirements”). At a minimum, the Minimum Technical Requirements Proposal must include the following:

(i) For all products that are not listed in Attachment F “Approved Product List”, the offeror shall submit an Attachment A “Detailed Information for Contract Line Items” and provide standard product documentation and product descriptive literature such as published specifications, product brochures, advertising materials or other materials that fully describe the offered products and their respective features, capabilities, functions, compatibility features, sizes, lengths weights, colors, and fabric/materials. Failure to do so may result in a rejection by the Government of all or part of the offer.

(ii) Descriptive Literatures: For the purpose of determining technical acceptability, descriptive literature must identify the appropriate model/brand, and type to clearly demonstrate that the items offered meet the product descriptions (both minimum technical requirements and minimum functions) listed in the Minimum Technical Requirements, “Attachment D”, Schedule of Items. Notes within a manufacturer/dealer catalog do not meet this requirement. Information that is not marked properly could cause offered CLINs to be considered technically unacceptable. If the descriptive literature does not adequately cover the information the offeror should provide a written proposal documenting their offered products capabilities. The technical proposal, product documentation and descriptive literature will be used when evaluating the items.

(iii) Size Description: If the size descriptions listed for any of the offered items utilize different language to describe a size other than what is used by Industry or the offeror, the offeror can submit an equivalent sized item. If an equivalent size is offered, the offeror shall provide a written explanation in the solicitation’s “Attachment A” describing the equivalent size for the corresponding CLIN, as listed in Attachment E “Price Schedule.

(iv) Warranty Terms: The offeror must provide the terms of any warranty covering the offered products for review by the Government. Please Note: The warranty requirements listed in the minimum technical requirement (See attachment D) are considered minimum, unless otherwise stated. If the manufacturer’s/vendor’s standard warranty exceeds the specified warranty, the manufacturer’s/vendor’s standard warranty will apply.

(v) Approved Product List: Offerors shall identify which items are being offered that are listed in the Approved Product List (Attachment F) by highlighting all devices and associated accessories/replacement parts offered.

(vi) Kits: Offered kits (See Section E.1(i)) shall include a complete explanation of the kit’s contents in attachment A.

(vii) Digital Photograph: A digital photograph of each item offered must be provided on a CD-ROM. If possible, color photographs are preferable. Photographs should be multiple angles of the items to highlight details on various sizes. The resolution must show detail at 4”X4” + 1” (640X480 is ideal).

13. The following language in Section E.9 is deleted:

E.9 52.212-2 EVALUATION – COMMERCIAL ITEMS (OCT 2014)

(a) The Government intends to award multiple contracts resulting from this solicitation. Award will be made on the basis of Lowest Price, Technically Acceptable Product(s).

The technical evaluation will be conducted during Phase I. Offerors may propose and may receive an award on one or more CLIN(s) for various makes and models of PAP device(s) and/or Facial Interface(s) make and model offered. Please note: Models of any particular make of device or Facial Interface must be distinguishable from another model i.e., product number, model name. The evaluation will be conducted in two phases. The initial screening and technical evaluation will be conducted during Phase I. The price evaluation will be conducted in Phase II.

As defined in this solicitation, a “Make” refers to the manufacturer or company of the product. A “Model” refers to the specific name or number given to a product by its manufacturers. For example, Resmed is a Make and a ResMed AirSense ™ 10 Elite CPAP Machine with Heated Humidifier is a Model of Resmed.

PAP DEVICES: The Government anticipates multiple awards resulting from this solicitation for the following types of PAP devices.

There may be a maximum of nine (9) models awarded for each MAKE offered per CLIN, with a maximum of thirty-six (36) models of various MAKES per CLIN.

CLIN 01: Auto-Titrating Continuous Positive Airway Pressure (APAP) Machine;

CLIN 02: Adaptive Servo-Ventilation (ASV) Machine;

CLIN 03: Bi-Level Positive Airway Pressure (BPAP) Machine;

CLIN 03A Bi-Level Positive Airway Pressure (BPAP) Machine;

CLIN 03B Bi-Level Positive Airway Pressure (BPAP) Machine;

There may be a maximum of twelve (12) models awarded for each MAKE offered per CLIN, with a maximum of forty-eight (48) models of various MAKES per CLIN.

CLIN 07: Oral Interfaces;

CLIN 08: Nasal Pillow Interfaces;

CLIN 09: Total Facial Interfaces.

MASK LINERS: The Government anticipates multiple awards resulting from this solicitation for Mask Liners. There may be up to four (4) awards per CLIN 10 for Mask Liners.

There will be no award considered for associated accessories and replacement parts unless an award is made for a corresponding PAP device.

The Government reserves the right not to make an award on any or all CLINs. The Government reserves the right to determine how many awards it will make per CLIN, which may be less than the maximum number of awards identified in the solicitation.

Up to the lowest twelve (12) MODELS for each MAKE offered per CLIN, will be subjected to an evaluation to determine if the offered item is Technically Acceptable.

MASK LINERS: In an initial screening of offers, the lowest total evaluated price for offered Mask Liners will be determined. The purpose of the initial screening is to determine which offered Mask Liners products will be submitted for technical evaluation. Evaluation of price for a fair and reasonable determination will only be conducted in Phase II of the evaluation process. There will be a maximum of four (4) awards for Mask Liners. Each award will consist of various Makes of Mask Liners (consisting of the 17 sizes of Mask Liners for Nasal, Full Face, Nasal Pillow, and Total Face Mask) of various sizes described in Attachment E “Price Schedule”. The lowest total evaluated price for offered Mask Liners will be determined by multiplying the offered unit prices by the estimated quantities for the base and each option period and summing the total price of mask liners offered. To be considered for award for mask liners all 17 sizes, as listed in Attachment E “Price Schedule”, must be offered.

Up to the lowest four (4) offered products for Mask Liners (consisting of the 17 sizes of Mask Liners for Nasal, Full Face, Nasal Pillow, and Total Face Mask) will be submitted for technical evaluation.

The lowest priced offered for a CLIN under this solicitation will be subjected to an evaluation that will be conducted to determine whether the offered item are Technically Acceptable.

Associated Accessories/Replacement Parts: The prices for offered associated accessories/replacement parts will be excluded from the initial screening price evaluation.

Each offered make and model of PAP device and/or Facial Interface and/or Mask Liner Products determined to be the lowest price will be evaluated on a technically acceptable/technically unacceptable basis. Additionally, accessories/replacement parts that are compatible with the lowest priced PAP devices that are being considered for award will also be evaluated on a technically acceptable/technically unacceptable basis. To be considered for award, a product must be determined to be technically acceptable during Phase I. The vendor will proceed to Phase II only if found to be technically acceptable in Phase I. Phase II will consist of Price evaluation.

If the lowest priced product is found to be technically acceptable, the price will be evaluated in Phase II. In Phase II, prices for the PAP devices, Facial Interfaces, Mask Liners, and all accessories/replacement parts will be evaluated for products being considered for award to determine if they are fair and reasonable.

If, however, the lowest priced offered model and make is evaluated as not Technically Acceptable during Phase I, the next lowest price Make and Model offered will be evaluated. If the lowest priced offered make and model is evaluated as Technically Acceptable, the next lowest priced make and model offered will be evaluated. This process will continue until a technically acceptable offer from a vendor is identified for each offered make and model of PAP device and/or Facial Interface.

There will be no award considered for associated accessories and replacement parts unless an award is made for a corresponding CLIN. For example, an award for associated accessories and replacement parts will only be considered for CLIN 0001 if a PAP device is awarded for CLIN 0001.

The lowest total evaluated price for offered Mask Liners will be determined by multiplying the offered unit prices by the estimated quantities for the base and each option period and summing the total price of mask liners offered. To be considered for award for mask liners all 17 sizes or equivalent sizes , as listed in Attachment E “Price Schedule”, must be offered.

Up to the lowest four (4) offered products for Mask Liners (consisting of the 17 sizes or equivalent sizes of Mask Liners for Nasal, Full Face, Nasal Pillow, and Total Face Mask) will be submitted for technical evaluation.

The lowest priced offered for a CLIN under this solicitation will be subjected to an evaluation that will be conducted to determine whether the offered item are Technically Acceptable.

Associated Accessories/Replacement Parts: The prices for offered associated accessories/replacement parts will be excluded from the initial screening price evaluation.

Each offered make and model of PAP device and/or Facial Interface and/or Mask Liner Products determined to be the lowest price will be evaluated on a technically acceptable/technically unacceptable basis. Additionally, accessories/replacement parts that are compatible with the lowest priced PAP devices that are being considered for award will also be evaluated on a technically acceptable/technically unacceptable basis. To be considered for award, a product must be determined to be technically acceptable during Phase I. The product will proceed to Phase II only if found to be technically acceptable in Phase I. Phase II will consist of Price evaluation.

If the lowest priced product is found to be technically acceptable, the price will be evaluated in Phase II. In Phase II, prices for the PAP devices, Facial Interfaces, Mask Liners, and all accessories/replacement parts will be evaluated for products being considered for award to determine the lowest evaluated price.

If, however, the lowest priced offered model and make is evaluated as not Technically Acceptable during Phase I, the next lowest price Make and Model offered will be evaluated. If the lowest priced offered make and model is evaluated as Technically Acceptable, the next lowest priced make and model offered will be evaluated. This process will continue until a technically acceptable offer from a vendor is identified for each offered make and model of PAP device and/or Facial Interface.

There will be no award considered for accessories and/or replacement parts unless an award is made for a corresponding PAP Device CLIN or Facial Interfaces. For example, an award for associated accessories and replacement parts (i.e., Chin Straps, Filters, Tubing, Humidifier Water Chamber, SD Cards, Power Chords, Power Supply) will only be considered for CLIN 0001 if a PAP device is awarded for CLIN 0001. And as an example, for CLIN 0005, an award for replacement parts (i.e., Cushions) will only be considered for CLIN 0005 if a Facial Interface is awarded.

Please note: A specific make and model accessory or replacement part will only be awarded to one offeror. The successful offeror will be the one offering the item(s) at the lowest price.

14. The following language in Section E.9 is deleted:

CASCADING TIERED EVALUATIONS IS LIMITED TO SDVOSB, VOSB, or SMALL BUSINESS

(1) The VA will evaluate the offers, if any, submitted by SDVOSB concerns that meet the requirements of the solicitation.

If the following desired number of MAKES and MODELS per CLIN of PAP Devices and Facial Interfaces and desired awards for Mask Liners are not reached, the VA may proceed to set-aside for VOSB concerns for the remainder of Make and Models of PAP Devices and Facial Interfaces and awards for Mask Liners that were not awarded at the SDVOSB level.

PAP Devices: A maximum of nine (9) models awarded for each MAKE offered per CLIN, with a maximum of thirty-six (36) models of various MAKES per CLIN.

Facial Interfaces: A maximum of twelve (12) models awarded for each MAKE offered per CLIN, with a maximum of forty-eight (48) models of various MAKES per CLIN.

Mask Liners: A maximum of four (4) awards for Mask Liners.

Similarly, if no offers were submitted by SDVOSB concerns and/or none of the offers would result in award at a fair and reasonable price for a particular Make and Model or Mask Liner that offers best value to the United States; the VA will withdraw the SDVOSB set-aside after making the awardable makes and models and proceed to the set-aside for VOSB concerns for the make and models that were not awarded at the SDVOSB level.

(2) The VA will evaluate the offers, if any, submitted by VOSB concerns that meet the requirements of the solicitation.

If the following desired number of MAKES and MODELS per CLIN of PAP Devices and Facial Interfaces and desired awards for Mask Liners are not reached, the VA may proceed to set-aside for VOSB concerns for the remainder of Make and Models of PAP Devices and Facial Interfaces and awards for Mask Liners that were not awarded at the VOSB level.
