LITTLE ROCK NATIONAL CEMETERY
July 2, 2019 A. The Contractor: Shall furnish all labor, material, equipment, tools, and supervision necessary to clean and inspect the entire storm drain system at Little Rock National Cemetery in accordance with the contract specifications, drawings, applicable standards and requirements as described herein. Storm drain system cleaning shall include post-cleaning inspection to assure satisfactory results
B. General Requirements and Conditions:
1. The Contractor shall be solely responsible for safety during the performance of all work. Contractor shall take satisfactory precautions to protect the storm drain system segments and appurtenances from damage that might be inflicted upon them by the use of cleaning equipment. Any damage inflicted upon a storm drain system segment or other government, public or private property as a result of the Contractor’s cleaning operations, regardless of the cleaning method used and regardless of any other circumstance which may contribute to the damage, shall be repaired by Contractor at their sole expense.
2. During storm drain system cleaning operations, satisfactory precautions shall be taken in the use of cleaning equipment. When hydraulically propelled cleaning tools which depend upon water pressure to provide their cleaning force or tools which retard the flow in the storm drain system line are used, precautions shall be taken to ensure that the water pressure created does not damage or cause flooding of government, public or private property being served by the storm drain system. When possible, the flow of water in the storm drain system shall be utilized to provide the necessary pressure for hydraulic cleaning devices.
3. Work includes removing and replacing catch basin and drain inlet lids.
4. CONFINED SPACE ENTRY requirements for catch basin and drain inlet access and cleaning:
a. Confined space entry must comply with OSHA 29 CFR 1926, OSHA 29 CFR 1910, OSHA 29 CFR 1910.146, and OSHA Directive CPL 2.100.
b. Confined Space Entry Plan: A site-specific Confined Space Entry Plan (including permitting process) shall be developed and submitted to the Contracting Officer and COR. Develop a confined or enclosed space entry plan in accordance with applicable OSHA standards 29 CFR 1910, 29 CFR 1915, and 29 CFR 1926, OSHA Directive CPL 2.100, and any other federal, state and local regulatory requirements identified in this contract. Identify the qualified person's name and qualifications, training, and experience. Delineate the qualified person's authority to direct work stoppage in the event of hazardous conditions. Include procedure for rescue by contractor personnel and the coordination with emergency responders.
c. The Competent Person must be on-site at all times when the work that presents the hazards associated with their professional expertise is being performed. Provide the credentials of the Competent Persons(s) to the COR and Contracting Officer.
d. Competent Person Qualifications: Provide Competent Persons meeting the requirements and with thorough knowledge of OSHA’s Confined Space Standard, 29 CFR 1910.146. Designated in writing to be responsible for the immediate supervision, implementation and monitoring of the confined space program, who through training, knowledge and experience in confined space entry is capable of identifying, evaluating and addressing existing and potential confined space hazards and, who has the authority to take prompt corrective measures with regard to such hazards.
e. Entry Procedures: Prohibit entry into a confined space by personnel for any purpose until the qualified person has conducted appropriate tests to ensure the confined or enclosed space is safe for the work intended and that all potential hazards are controlled or eliminated and documented. Hazards pertaining to the space must be reviewed with each employee.
f. Forced Air Ventilation: Forced air ventilation is required for all confined space entry operations and the minimum air exchange requirements must be maintained to ensure exposure to any hazardous atmosphere is kept below its action level.
g. Rescue Procedures and Coordination with Local Emergency Responders: Develop and implement an on-site rescue and recovery plan and procedures. The rescue plan must not rely on local emergency responders for rescue from a confined space.
5. Contractor shall provide the necessary safety and inspection equipment needed to allow for the visual inspection and confirmation of all cleaning and algae, sediment, and debris removal by the COR.
6. Work may require confined space permits for catch basin and drain inlet access.
7. Coordinate with the COR to locate and review all storm drain inlets, outlets, catch basins, and piping routes prior to start of work using the contract drawings. All utilities and irrigation system components shall be identified and marked prior to the start of work.
8. All precautions shall be taken by the Contractor to protect the storm drain system from damage that might result from the use of unsuitable equipment or improper use of approved cleaning equipment. Any drains damaged during the cleaning operations as a result of the Contractors operations shall be promptly repaired to an acceptable condition (as determined by the Contracting Officer) by and at the expense of the Contractor. The Contractor shall act immediately to remedy problems created by the cleaning procedure, which represent a hazard to the general public, such as the collapse of the ground surface above a storm drain. If equipment retrieval necessitates excavation, the Contractor shall be responsible for accomplishing the work at his own expense. Following removal of the equipment, the Contractor shall restore the line and the site back to its original condition.
9. Storm Drain System Dewatering: The Contractor is responsible for dewatering any wet materials. Materials shall not be allowed to leak through the sides of any vehicle or equipment. During the video inspection process, every effort shall be exerted to obtain a full view of the pipe interior. For instances where the camera lens becomes submerged or where a large portion of the pipe shall contain water, and these conditions persist for significant portions of the storm drainage being inspected, the Contractor shall attempt to dewater the pipe.
10. It shall be the responsibility of the Contractor to remove internal deposits from the catch basins or storm drain system structures, using accepted storm drain system cleaning techniques. The cleaning operation shall remove all debris, solids, soil, sand, and pieces of broken pipe, bricks, grit, and other debris that would collect within the Storm Drain System. Manholes, catch basins, and inlets shall all be classified as storm drain system structures.
11. All material shall be removed at the downstream manhole of the section being cleaned. Passing material from one manhole section to another shall not be permitted. In the event that roots, dirt, sand, rocks, encrustation, and other solid or semisolid material or debris resulting from the cleaning operations are observed and/or detected by the COR as passing to downstream storm drain system segments(s), Contractor shall be responsible for cleaning such downstream storm drain system segment(s) at no additional cost to the Government.
12. Precautions shall be taken to ensure that the cleaning operation will not cause any damage or flooding to public and /or private property being served by the storm drain system line section involved. The Contractor shall bear full costs associated with any flooding or damage to basements or structures.
13. The Contractor shall recognize that there are some conditions such as broken pipe and/or major blockages that can prevent cleaning from being accomplished or where damage could result if cleaning were attempted or continued. Perform a reverse set-up cleaning when a blockage in the storm drain system prevents completion of cleaning from the downstream manhole by moving equipment to the upstream manhole and attempting to complete the cleaning of the remainder of the line.
14. Attempt to remove a specific blockage in the storm drain system before advising the COR and Contracting Officer that the blockage cannot be removed. The COR shall immediately be notified by the Contractor of any and all conditions, which in the opinion of Contractor warrant termination of cleaning activities. If Contractor’s cleaning equipment becomes lodged in a storm drain system, it shall be removed by Contractor at his/her expense. This shall include excavation and repair of the storm drain system, underground utilities, backfilling, and surface restoration.
15. The Contractor shall prepare and submit in advance a written traffic control program in accordance with the recommendations in Part VI of the "Manual of Uniform Traffic Control Devices," (MUTCD) current edition, for review and approval by the COR.
16. Contractor shall also submit a project phasing plan with their schedule. The Contractor’s plan shall incorporate the following restrictions:
a. During public visiting (i.e. business) hours (Sunday – Saturday, 7:00 am to 4:30 pm) two-way traffic shall be maintained at all times on the main entrance boulevard. The Contractor shall provide and place temporary signage and barriers in accordance with the MUTCD to direct and notify traffic of any new two-way conditions.
b. Work around or near the Committal Shelter shall be scheduled around committal services or during the weekend and/or after business hours.
c. Weekend and after-hours closure may be considered for other areas by approval of the COR and engineer.
d. The construction access route to the cemetery shall be via the west side entrance off of East 26th Street. The inbound and outbound lanes of main entrance be maintained open at all times for visitors and funeral corteges.
e. The Contractor is required to discontinue his work sufficiently in advance of Easter Sunday, Mother's Day, Father's Day, Memorial Day, Veteran's Day and/or Federal holidays, to permit him to clean up all areas of operation adjacent to existing burial plots before these dates.
17. When other than normal business hours are approved in any case, all traffic control devices shall be removed to allow normal traffic flow (one lane) the following morning (i.e. at 7:00 am).
18. All debris, trash, sediment, roots, rocks, sand, solids or semi-solids resulting from the cleaning operations shall become the property of the Contractor, shall be removed from the site at the end of each workday, and shall be disposed of in compliance with applicable local, state and federals laws and regulations by the Contractor. Material shall be disposed of at a licensed landfill and all fees shall be paid for by the contractor for disposal. Copies of records of all disposals shall be submitted to the Contracting Officer indicating disposal site, date, amount and a brief description of material disposed. Any sampling and testing required or necessary to secure a disposal site for the removed material will be the Contractor’s responsibility and expense. Trucks hauling solids or semisolids from the site shall be watertight so that no leakage or spillage will occur.
19. The Contractor shall keep his haul route and work area(s) neat, clean, and reasonably free of odor, and shall bear all responsibility for the cleanup of any spill.
20. Availability and use of utility services: The Government shall make all reasonably required amounts of water available to the Contractor from existing outlets and supplies. The Contractor, at Contractor's expense, shall install and maintain all necessary temporary connections and distribution lines. Before final acceptance of the work by the Government, the Contractor shall remove all the temporary connections, distribution lines, and associated paraphernalia. No other utility services will be provided by the Government. If the Cemetery water supply is insufficient (flow, pressure, capacity) to meet the Contractor’s needs, the Contractor shall be responsible to provide or obtain their own water, such as coordinating with the City of Little Rock to utilize local fire hydrants. The cost of using municipal fire hydrants are the responsibility of the Contractor and shall be included in the contract proposal. If the water is obtained from a potable supply, appropriate backflow prevention devices shall be provided to protect the potable system from cross connections and contamination. Contractor shall be solely responsible for preventing cross contamination of any public or private water systems used for this purpose
21. Remove, cut, alter, replace, patch and repair existing work as necessary to perform the work. Existing work to be altered or extended and that is found to be defective in any way, shall be reported to the COR before it is disturbed. Materials and workmanship used in restoring work shall conform in type and quality to that of original existing construction, except as otherwise shown or specified.
22. The Contractor shall preserve and protect all structures, equipment, and vegetation (such as trees, shrubs, and grass) on or adjacent to the work site not to be removed and do not unreasonably interfere with the work required under this contract. If any limbs or branches of trees are broken during contract performance, or by the careless operation of equipment, or by workmen, the Contractor shall trim those limbs or branches with a clean cut and paint the cut with a tree-pruning compound as directed by the Contracting Officer.
23. Prior to starting work, locate all utility and irrigation system components and install barriers and/or protection devices to prevent damage and to restrict public access while working in and around the storm drain system areas. The Contractor shall protect from damage all existing improvements and utilities at or near the work site and on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any damage to those facilities, including those that are the property of a third party, resulting from failure to comply with the requirements of this contract or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor.
24. All utilities required for the continuous operation of all existing facilities shall be maintained in service at all times except when disruptions are needed. The Contractor shall inform the COR, Cemetery Director, and the Contracting Officer (CO) in writing for such disruptions and obtain approval from the CO in writing at least 2 weeks prior to such need. When option exists, implement options to minimize interruption of services to the facilities.

The work shall not be accepted until all unsatisfactory conditions have been corrected to the satisfaction of the Contracting Officer and the COR.
C. The Work: The Work shall include, but shall not be limited to the following (not necessarily in the order indicated):
1. CLEANING:
a. The intent of the storm drain system cleaning is to remove all sludge, dirt, sand, rocks, roots, and other solids or semisolid material from the piping, catch basins, and inlets to restore the storm drain system to its maximum operating condition possible and to perform a visual inspection/evaluation of the entire system. The cleaning is to ensure defects are not obscured and to allow the water level to drop so that defects are visible. The piping, catch basins, inlets, and all system interior surfaces shall be clean enough to allow viewing of the entire area during inspection. Since the success of the other phases of work depends a great deal on the cleanliness of the lines and catch basins, the importance of this phase of the operation is emphasized.
b. The Contractor shall be solely responsible for his means and methods of the storm drain system cleaning. Selection of the cleaning equipment and the method for cleaning shall be based on the conditions of the storm drain system and its structures at the time work commences and shall comply with this statement of work. Contractor is responsible to determine and execute the most effective method for debris, sediment, and root removal, pressure cleaning/washing, vacuuming, cleaning, and management of the resultant spoils to achieve the final results. The finished product shall be thoroughly clean storm system catch basins, drain inlets, and all associated drain piping to a condition where 100% visual inspection can be completed.
c. If cleaning of an entire storm drain system section cannot be successfully performed from one manhole, the equipment shall be set up on the other manhole and cleaning attempted again. If, again, successful cleaning cannot be performed or the equipment fails to traverse the entire storm drain system section, it will be assumed that a major blockage may exist, the cleaning effort shall be suspended, and the Contracting Officer and the COR shall be notified.
d. Conditions such as a broken pipe or major blockage may prevent cleaning from being accomplished or cause further damage if further attempts to clean the line are made. If such problems beyond large accumulations of sediment or debris are encountered, notify the Contracting Officer and the COR immediately for further instruction.
e. All storm drain system cleaning efforts shall require documentation of all quantities and types of materials removed during cleaning. Materials shall be disposed of from the site at least once at the end of each workday. The Contractor shall be responsible for the proper disposal of the materials removed from the storm drain system.
2. CATCH BASINS AND INLETS:
a. Cleaning of catch basins shall include entire catch basin interior, including catch basin benches and walls. It also includes removal of all foreign object or debris from the catch basins which shall be removed either manually or mechanically.
b. Cleaning shall include the use of cleaning equipment to remove sludge, dirt, etc. from the interior walls and bottoms of the catch basin. Incorporate into line cleaning operation by scouring walls after pipe segment cleaning operation is complete. This also includes removal of all foreign object or debris from the catch basins which shall be removed either manually or mechanically.
c. After completion, each catch basin and inlet shall be visually inspected by the COR and approved by the COR as completed satisfactorily. Re-clean as necessary until approved by the COR. a. Storm drain system line cleaning shall be performed by electronically or manually operated, directly or remotely controlled, mechanically or hydraulically propelled cleaning equipment. Heavy line cleaning shall be performed by mechanical equipment when water jetting is unsuccessful.
b. Satisfactory precautions shall be taken to protect the storm drain system lines from damage that might be inflicted by improper use of the cleaning equipment. Whenever hydraulically propelled cleaning tools that depend upon water pressure to provide their cleaning force, or any tools that retard the flow of water in the storm drain system line, are used, precautions shall be taken to ensure that the water does not cause damage or flooding to public or private property.
c. Storm drain system line sections shall be cleaned using mechanically powered, hydraulically propelled, or high velocity storm drain system cleaning equipment. The equipment selected for cleaning shall be capable of removing dirt, grease, rocks, sand, sticks, branches, leaves and other deleterious materials and obstructions from the storm drain system lines and catch basins.
d. If cleaning of an entire section cannot be successfully performed from one catch basin, the equipment shall be re-setup at the catch basin on the opposite end of the pipe segment being cleaned and cleaning shall be re-attempted.
e. The retrieval of equipment lodged in pipes or a catch basin is the Contractor’s responsibility and will be performed at the Contractor’s expense. Contractor will be solely responsible and held liable for any claims or damages arising from any action or lack of action arising from reckless, negligent, or careless behavior.
f. After completion, each section of piping between catch basins, inlets, or outlets shall be visually inspected by the COR and approved by the COR as completed satisfactorily. Picture/video files shall be organized in folders by date which the catch basin or infiltration box was cleaned. All photos shall be at least 4 megapixel (MP) in quality. Video files shall be a minimum of 720p resolution.
f. The inspection will be done one (1) reach (distance between two consecutive manholes) at a time. The reach being inspected will be suitably isolated from the remainder of the storm drainage system by restricting all upstream flows to allow maximum exposure of the pipe being inspected. In some instances, more than one (1) storm drainage reach may have to be inspected per setup (buried manholes). In these instances, the Contractor shall have adequate cable to deploy the video camera.
g. The inspection will be done one manhole section at a time and the flow in the section being inspected will be suitably controlled.
h. The height of the camera shall be adjusted so that the camera lens is always centered in the pipe being televised. The lighting system shall be adequate for quality pictures. A reflector in front of the camera may be required to enhance lighting in black pipe.
i. The importance of accurate distance measurements is emphasized. Measurement for location of defects shall be by means of a footage counter with the value displayed on the video. The footage counter shall be set such that zero is the center of the beginning manhole. Marking on the cable, or the like, which would require interpolation for depth of manhole, will not be allowed. Accuracy of the footage counter shall be checked by use of a walking meter, roll-a-tape, or other suitable device, and the counter shall be calibrated to an accuracy of +/- ½ foot or to the satisfaction of the COR.
j. Manholes and structures shall be visually inspected from above ground. Obtain photographs of the exterior condition of the structure and surrounding area. The interior of the structure shall be inspected by rotating the camera clockwise to produce a 360-degree panorama view of the structure. Begin at the rim of the structure facing the largest diameter outgoing pipe. Provide photos and defects of the structure. Repeat process lowering the camera 2-3 feet until the invert is reached.
9. FINAL ACCEPTANCE:
a. Acceptance of storm drain system cleaning work is contingent upon the successful completion of the catch basin and inlet cleaning inspection and review of the piping inspection videos be to the satisfaction of the COR. If the catch basin or piping inspection shows debris, solids, sand, roots, and deposits remaining in the structures or piping, the cleaning will be considered unsatisfactory, and the Contractor shall repeat the cleaning and re-inspection of the identified areas/items, at no additional cost to the Government until the cleaning is acceptable to the COR.
b. The Contractor shall ensure that all structures and system piping submitted for payment will be free from debris upon completion of…
