The inspection was conducted by Terracon Consultants, Inc. in two phases: a limited asbestos-containing material (ACM) survey on March 5, 2025, and a lead-based paint (LBP) screening via X-ray fluorescence (XRF) on April 15, 2025. The survey was performed by EPA-certified AHERA building inspectors Matt Valenti and Kyle Johnson and was limited to materials that would be disturbed during the planned EHRM infrastructure upgrade project. The scope excluded roof sampling due to construction and roof patch warranties per building owner direction.
Key findings indicate that asbestos was not identified in the 18 bulk samples collected and analyzed through polarized light microscopy. However, mirror mastic with 8% chrysotile (Category II non-friable material) was identified in bathrooms from a previous August 2023 inspection report; some material was removed during water leak remediation but approximately 20 square feet remains in place. Additional materials (asphaltic rolled roofing and ceramic tiles) were assumed to contain asbestos per owner restrictions on sampling. Lead-based paint testing of 82 painted surfaces detected no LBP (defined as ≥1.0 mg/cm² per HUD guidelines). Laboratory analysis was performed by Cates Laboratories, an NVLAP-accredited facility using EPA Method 600/R-93/116. The report provides detailed sample location diagrams, laboratory analytical data, chain of custody documentation, and personnel certifications.

Dear Mr. Collar:
As requested by Atriax PLLC, Terracon Consultants, Inc. (Terracon), recently conducted a limited asbestos-containing material (ACM) survey and update to previous inspections at the Veterans Affairs Consolidated Mail Outpatient Pharmacy located at 3675 East Britannia Drive in Tucson, Arizona (the site). The limited survey and update to a previous inspection was conducted by Terracon personnel on March 5, 2025 and was limited to suspect materials that may be impacted during the EHRM Infrastructure upgrades project at the site. Terracon made return visit to the site on April 15, 2025 to conduct a Lead Based Paint (LBP) screening of the site. The project involves renovations of the Veterans Affairs Consolidated Mail Outpatient Pharmacy building. The scope of work was requested by Mr. Stephen Collar, Project Manager, for Atriax, PLLC.
Based on Laboratory analysis, asbestos was not identified in the samples collected from the site. Asbestos containing mirror mastic was identified in the previous inspection report conducted by others, some of this material was removed during a water leak remediation, but some of it is still located on site. Additionally, materials associated with the roof and new ceramic tiles were assumed to be asbestos as we were directed by the building owner to not sample the roofing or ceramic tiles that were new. Painted surfaces were tested by X-ray fluorescence (XRF) to see if lead based paint (LBP) is present. LBP was not detected in the surfaces tested.

1.0 Introduction
Terracon conducted a limited asbestos survey, Lead Based Paint (LBP) screening and an update to previous inspection by others for the Veterans Affairs Consolidated Mail Outpatient Pharmacy (VACMOP) infrastructure project located at 3675 East Britania Drive in Tucson, Arizona. The limited asbestos survey, LBP screening and Update to Previous Inspections included materials that would be disturbed during the EHRM infrastructure upgrade project. Previous report by others can be found in the appendices. The limited asbestos survey and update was conducted on March 5 and April 15, 2025, by Mr. Matt Valenti and Mr. Kyle Johnson, both Environmental Protection Agency (EPA)-certified Asbestos Hazard Emergency Response Act (AHERA) building inspectors for asbestos and EPA certified Lead Inspectors and or Risk Assessors. This work was conducted in general accordance with Professional Service Order dated October 23, 2024.
The scope of the project included materials and painted and/or coated surfaces that may be disturbed during the EHRM infrastructure upgrade project. Homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented. Suspect ACM samples were collected in general accordance with the sampling protocols outlined in Environmental Protection Agency (EPA) regulation 40 CFR 763, AHERA. Samples were delivered to Cates Laboratory (Cates) in Dallas, Texas, a National Voluntary Laboratory Accreditation Program (NVLAP) - accredited laboratory for analysis by Polarized Light Microscopy (PLM).
1.1 Project Objective
We understand this limited asbestos survey and update to a previous inspection was requested due to the planned EHRM infrastructure upgrades project located at 3675 East Britannia Drive in Tucson, Pima County, Arizona. We understand the purpose of this survey was to identify and quantify ACM. EPA regulation 40 CFR 61, Subpart M, National Emission Standards for Hazardous Air Pollutants (NESHAP), prohibits the release of asbestos fibers to the atmosphere during renovation or demolition activities. The asbestos NESHAP requires that potentially regulated asbestos-containing materials be identified, classified, and quantified prior to planned disturbances or demolition activities.
Additionally, Terracon conducted an LBP XRF screening to determine if the painted surfaces contain lead based paint.
1.2 General Conditions and Limitations
The limited asbestos survey, XRF screening, and update was based on information provided to Terracon regarding building conditions. The level of effort and associated tasks performed for this service was limited to the scope of services outlined in Terracon’s Proposal. Terracon

1.3 Reliance
This report is prepared for the exclusive use and reliance of Atriax, PLLC. Use or reliance by any other party is prohibited without the written authorization of Atriax, PLLC and Terracon.
2.0 Building Description
The building is single-story slab on grade structure. The warehouse area consists of concrete flooring and drywall. The offices contained various floor tiles and drywall systems throughout the office space and bathrooms. Acoustical ceiling tiles were throughout the building office space.
3.0 Field Activities
The asbestos survey, LBP XRF screening, and update to previous inspections was conducted by Matt Valenti and Kyle Johnson, both AHERA-accredited asbestos Building Inspectors. Mr.
Johnson and Mr. Valenti are also an EPA certified Lead Based Paint Inspector or Risk Assessor. Copies of their certifications are included in Appendix D. The limited asbestos survey was conducted in general accordance with the sample collection protocols established in EPA regulation 40 CFR 763, AHERA. A summary of sampling activities is provided below.
3.1 Visual Assessment
Asbestos
Our sampling activities began with a visual assessment of the proposed work areas to identify homogeneous areas of suspect ACM. A homogeneous area consists of building materials that appear similar throughout in terms of color, texture, and date of application.
The assessment was for only those materials that may be disturbed during the infrastructure renovation project.
Building materials identified as glass, wood, metal, or rubber were not considered suspect ACM, as per EPA regulation.
Lead Paint Screening by XRF
Terracon visually inspected the painted surfaces to identify color/substrate combinations to be tested by XRF field analysis.

3.2 Physical Assessment
A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material that can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials.
Lead Paint Screening by XRF
A physical assessment was not performed on painted surfaces.
3.3 Sample Collection
Asbestos
Based on results of the visual observation, bulk samples of suspect ACM were collected in general accordance with EPA AHERA sampling protocols. Random samples of suspect materials were collected in each homogeneous area. Terracon personnel collected bulk samples using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker.
Eighteen (18) bulk samples were collected from six (6) homogeneous areas (HAs) of suspect ACM from the building surveyed and five materials were assumed to be asbestos containing do to the owner not wanting the materials sampled. A summary table of suspect ACM samples collected and/or assumed during the limited survey is listed in Appendix A.
Lead Paint Screening by XRF
The U.S. Department of Housing and Urban Development (HUD) lead-based paint (LBP) style limited inspection was conducted by an Environmental Protection Agency (EPA) certified LBP inspectors in a manner similar to the 1997 revision of HUD Guidelines, Chapter 7: Lead- Based Paint Inspection with a portable X-ray fluorescence (XRF), SciAps X-550 Pb HUD/EPA Lead Paint Analyzer – Rh, unit on the painted surfaces of interior and exterior building components present. The X-550 Pb utilizes miniaturized, rugged Rh tube (40kv, 100uA, 4W) and Silicon Drift Detector for measurement of lead-based paint in units of mg/cm2. The X-550 Pb is a handheld XRF accepted by HUD and EPA for testing residential, daycare, schools, and other such properties via a published Performance Characteristics Sheet (PCS). It provides analysis of lead (Pb) loadings for both surface and deeply covered lead in paint.
Calibrations checks for the XRF unit are performed before the start of the day and additionally at 4-hour intervals or whenever the unit is turned off/on during the inspection. A final calibration check is performed at the end of the day.
Terracon measured eighty-two(82) individual painted surfaces throughout the renovation work areas in the limited LBP survey.

3.4 Sample Analysis
Asbestos
Bulk samples were submitted under chain of custody to Cates Laboratories (Cates) in Dallas, Texas for analysis by polarized light microscopy with dispersion staining techniques per USEPA methodology 600/R-93/116. The percentage of asbestos where applicable was determined by microscopic visual estimation. Cates is accredited under the National Voluntary Laboratory Accreditation Program (NVLAP) Accreditation No. 200569-0. A copy of Cates certification is attached to this report.
4.0 Regulatory Overview
Asbestos
The Asbestos NESHAP program in Arizona is enforced by federal, state, and county Asbestos NESHAP Coordinators.
The asbestos NESHAP (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions and asbestos waste disposal practices. The asbestos NESHAP regulation also requires the identification and classification of existing ACM according to friability prior to demolition or renovation activity. Friable ACM is a material containing more than 1% asbestos that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure. All friable ACM is considered regulated asbestos containing material (RACM).
The asbestos NESHAP regulation classifies ACM as either RACM, Category I non-friable ACM or Category II non-friable ACM. RACM includes all friable ACM, along with Category I and Category II non-friable ACM that has become friable, will be or has been subjected to sanding, grinding, cutting, or abrading, or ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder in the course of renovation or demolition activity. Category I non-friable ACM are exclusively asbestos-containing packings, gaskets, resilient floor coverings, resilient floor covering mastics and asphalt roofing products that contain more than 1% asbestos. Category II non-friable ACM are all other non-friable materials other than Category I non-friable ACM that contain more than 1% asbestos. Category II non-friable ACM generally includes but is not limited to cementitious material such as: cement pipes, cement siding, cement panels, glazing, mortar and grouts.
For all demolitions (even when no asbestos is present), the Asbestos NESHAP agency overseeing the project site must be provided with a written NESHAP notification at least 10 working days prior to the commencement of asbestos abatement activities related to the demolition. Some counties have fees for their notification process. Some cities may have separate permit fees as well. RACM must be removed by a licensed asbestos abatement contractor prior to renovation or demolition activities which will disturb the materials.

The United States Occupational Safety and Health Administration (USOSHA) asbestos standard for construction (29 CFR 1926.1101) regulates workplace exposure to asbestos.
The USOSHA standard requires that employee exposure to airborne asbestos must not exceed 0.1 fibers per cubic centimeter of air (0.1 f/cc) as an eight-hour time weighted average (TWA) and not exceed 1.0 fibers per cubic centimeter of air (1.0 f/cc) over a 30-minute time period known as an excursion limit (EL). The TWA and EL are known as USOSHA’s asbestos permissible exposure limits (PELs). The USOSHA standard classifies construction and maintenance activities which could disturb ACM and specifies work practices and precautions which employers must follow when engaging in each class of regulated work.
Lead
The Resource Conservation and Recovery Act (RCRA) gave the EPA authority to regulate the waste status of demolition or renovation debris, including Lead-Containing materials.
Specific notification and testing requirements must be addressed prior to transporting, treating, storing, or disposing of hazardous wastes. Lead-Containing wastes are considered hazardous waste under RCRA if results from samples analyzed after TCLP preparation exceed five milligrams per liter (mg/L).
Detectable lead quantities may constitute a lead dust hazard during renovation and demolition activities. Personnel performing renovation and demolition activities that may disturb painted components and building surfaces with concentrations of lead above the designated analytical detection limit should comply with all current OSHA regulations in order to minimize employee exposure. OSHA defines lead paint as a paint, which contains lead, regardless of the concentration. Currently, any proposed renovation or demolition is subject to the OSHA regulations (29 CFR 1926.62 – Lead Exposure in Construction). The OSHA regulation defines specific training requirements, engineering controls and working practices for construction personnel subject to this standard. Occupational exposure to lead occurring in the course of construction work, including maintenance activities, painting, alteration and repairs is subject to the OSHA “Interim” Lead Exposure in Construction standard.
Construction work covered by 29 CFR 1926.62 includes any repair or renovation activities or other activities that disturb in-place lead-containing materials but does not include routine cleaning and repainting where there is insignificant damage, wear, or corrosion of existing lead-containing coatings or substrates. Employers must assure that no employee will be exposed to lead at concentrations greater than 50 micrograms per cubic meter (g/m³) averaged over an eight-hour period without adequate protection. The OSHA Standard also establishes an action level of 30 g/m³ which, if exceeded, triggers the requirement for medical monitoring.

5.0 Findings and Recommendations
Asbestos
The summary table of the suspect materials sampled at the site, and their corresponding results, are presented in Appendix A. A sample location diagram is presented in Appendix B, Copies of the laboratory analytical reports are included in Appendix C.

The above materials were assumed to be asbestos containing because the building owner did not want to damage them. They also indicated that they would not be disturbed during the EHRM project.
Lead Paint Screening by XRF
The U.S. HUD guidelines define LBP as paint or other surface coatings that contain lead equal to or exceeding 1.0 milligram per square centimeter (mg/cm2) of surface area, 0.5 percent by weight or 5,000 parts per million (ppm). Lead-containing paint with a lead concentration above the laboratory detection limit, but below the HUD LBP threshold limit is regulated by OSHA. LBP was not detected by XRF field analysis.

General Comments
This limited asbestos survey, LBP screening and update to previous asbestos surveys were conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale.
The results, findings, conclusions, and recommendations expressed in this report are based on conditions observed during our limited sampling activities at the site. The information contained in this report is relevant to the date on which this limited sampling was performed and should not be relied upon to represent conditions at a later date. This report has been prepared on behalf of and exclusively for use by the client for specific application to their project as discussed. This report is not a bidding document. Contractors or consultants reviewing this report must draw their own conclusions regarding further investigation or remediation deemed necessary. Terracon does not warrant the work of regulatory agencies, laboratories or other third parties supplying information which may have been used in the preparation of this report. BACKGROUND INFORMATION
EVALUATION DATA SOURCE AND DATE:
This sheet is supplemental information to be used in conjunction with Chapter 7 of the HUD Guidelines for the
Evaluation and Control of Lead-Based Paint Hazards in Housing, 2012 Edition ("HUD Guidelines"). Performance parameters shown on this sheet are calculated using test results on building components in the HUD archive.
Testing was conducted on 146 test samples in February 2022, with two separate instruments of each Anode type, operated in both Timed and Quick modes.
OPERATING PARAMETERS
Performance parameters shown in this sheet are applicable only when properly operating the instrument using the manufacturer's instructions and procedures described in Chapter 7 of the HUD Guidelines.
XRF CALIBRATION CHECK:
The calibration of the XRF instrument should be checked using the paint film nearest 1.0 mg/cm2 in the NIST
Standard Reference Material (SRM) used (e.g., for NIST SRM 2579, use the 1.02 mg/cm2 film; for NIST SRM
2579a, use film 2573 (1.04 mg/cm2).
If the average (rounded to 1 decimal place) of three readings is outside the acceptable calibration check range, follow the manufacturer's instructions to bring the instrument into control before XRF testing proceeds.
EVALUATING THE QUALITY OF XRF TESTING:
Randomly select ten testing combinations for retesting from each house or from two randomly selected units in multifamily housing.
Conduct XRF re-testing at the ten testing combinations selected for retesting.
Determine if the XRF testing in the units or house passed or failed the test by applying the steps below. Compute the Retest Tolerance Limit by the following steps:
Determine XRF results for the original and retest XRF readings. In single-family and multi-family housing, a result is defined as a single reading. Therefore, there will be ten original and ten retest XRF results for each house or for the two selected units.
Calculate the average of the original XRF result and the retest XRF result for each testing combination.
Square the average for each testing combination.
Add the ten squared averages together. Call this quantity C.
Multiply the number C by 0.0072. Call this quantity D.
Add the number 0.032 to D. Call this quantity E.
Take the square root of E. Call this quantity F.
Multiply F by 1.645. The result is the Retest Tolerance Limit.
Compute the average of all ten original XRF readings.
Compute the average of all ten re-test XRF readings.
Find the absolute difference of the two averages.
If the difference is less than the Retest Tolerance Limit, the inspection has passed the retest. If the difference of the overall averages equals or exceeds the Retest Tolerance Limit, this procedure should be repeated with ten new testing combinations. If the difference of the overall averages is equal to or greater than the Retest Tolerance Limit a second time, then the inspection should be considered deficient.
Use of this procedure is estimated to produce a spurious result approximately 1% of the time. That is, results of this procedure will call for further examination when no examination is warranted in approximately 1 out of 100 dwelling units tested.
TESTING TIMES:
The reading time in Archive tests was 10 seconds in Timed mode and from 2-6 seconds in Quick mode, for both the Rh Anode and Au Anode.
CLASSIFICATION OF RESULTS:
XRF results for the Au Anode in Quick mode are classified as positive if they are greater than or equal to 1.0 mg/cm2 and negative if they are less than to 1.0 mg/cm2. XRF results for the Au Anode in Timed mode and for the Rh Anode in Timed or Quick mode are classified as positive if they are greater than or equal to 0.9 mg/cm2 and negative if they are less than to 0.9 mg/cm2

A report titled Methodology for XRF Performance Characteristic Sheets (EPA 747-R-95-008) provides an explanation of the statistical methodology used to develop Performance Characteristic Sheets at the Federal standard (Action Level) of 1.0 mg/cm2 and provides empirical results from using the recommended inconclusive ranges or thresholds for specific XRF instruments. STATEMENT OF LABORATORY ACCREDITATION
The samples were analyzed in general accordance with the procedures outlined in the U.S. EPA Interim Method for the Determination of Asbestos in Bulk Insulation Samples as found in 40 CFR, Part 763, Subpart E, Appendix E (formerly Subpart F, Appendix A), or the current U.S. EPA method (EPA Method 600/R-93/116) for the analysis of asbestos in building materials, by polarized light microscopy. The results of each bulk sample relate only to the material tested and the results shall not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the Federal Government.
