2. OVERVIEW
The United States Department of Veterans Affairs Consolidated Mail Outpatient Pharmacy (VA CMOP) processes over 500,000 prescriptions per day at seven regional sites.
Prescriptions are entered into the patient’s Electronic Health Record (EHR) at VA hospitals and clinics, then transmitted to the assigned CMOP for processing and mailing to the veteran.
Currently, the Mid-South CMOP (M-CMOP) processes an average of 53,000 scripts per day at two separate locations. The first location, Sam Jared, is an older facility at the Alvin C. York VA Medical Center in Murfreesboro, TN. The second, newer facility, known as Elam Farms, is approximately ten miles south. The goal of the VA is to service the full Mid- South prescription volume from the Elam Farms facility.
In 2013 the VA contracted with a commercial automation vendor to provide a comprehensive Automated Pharmacy Fulfillment System (APFS) for Elam Farms. After years of effort, the system has not been able to perform to specifications (requiring Sam Jared to remain open to process prescriptions) and the government has cancelled the APFS vendor’s contract effective on June 1, 2021.
A plan has been developed that will convert the existing Elam Farms APFS into an effective automation system that will use some components from the current APFS but will rely primarily upon major components transferred from a soon to be decommissioned CMOP in Charleston, SC.
This RFQ is intended to solicit quotes from vendors that will support the VA in dis-assemble, demolition, relocating and staging for disposal of existing APFS equipment and the installation and subsequent tear out of temporary workstations that are required for the facility to maintain operations during the transition period.
Quotes will be evaluated to select the best, most reliable support, and the most cost-effective means of providing a best value solution for the Murfreesboro CMOP.

3.0 Scope of Demolition
The following section describes the equipment and details for breaking down and removing the existing APFS equipment within the Elam Farms space.
All disassembly work included in this section will be inclusive of electrical and pneumatic lines feeding the equipment. Such lines and cabling will be removed back to their respective control panel or piping manifold. All floors will be left in a condition conducive to subsequent installation of equipment by others. This includes:
Clean, bare, swept floors with no materials or debris
No obstructions
In the drawings below, red signifies equipment to be removed. Pictures of the target portion of the system have been included in order to provide a better visual of the work to be accomplished.
The phases are intended to maintain organization of the equipment and components. They will occur sequentially without the need to demobilize.
3.1 Disassembly Description and Identification
3.1.1 Existing A-Frame Removal
Phase 1 of the transition will commence with the demolition of the existing APFS UoU system (A-Frame dispenser labeling robotic system and tote sortation system) and an associated elevated, floor mounted belt conveyor system.
The A-Frame section includes disassembly of the following equipment:
o UoU (UUA) dispensers o Robotic labelers/scan equipment o Tote sortation system o Elevated floor supported belt conveyor
3.1.2 Existing Multi-Pack Line Removal
During this phase the Vendor will be responsible for disassembly of Multi-pack lines 1,2,3 and 4.
The Multi-pack system consists of the following major components:
Automatic Baggers
Printing Equipment
Tote Supply and Return Conveyor
Sortation Equipment
3.1.3 Existing TCA Module removal
All four lines of Tablet Counting Automation will be removed along with associated takeaway equipment. A portion of this equipment will be reused in the final configuration and will therefore be removed and reinstalled by the equipment integrator.
The TCA Equipment consist of the following components:
Bottle singulators/sorters
Bottle labelers
Modular filling robots
Weighing equipment /Bottle Cappers
Bottle Buffers.
3.2 Removal of Decommissioned Equipment:
The Government will attempt to dispose of decommissioned equipment from the Elam Farms facility via appropriate means for possible resale or relocation to other CMOP sites. To that end decommissioned equipment will need to be fully inventoried (including photos), palletized and stretch wrapped, etc. to protect it for this subsequent disposal effort. The Contractor’s scope should include time and materials to support this effort for all major pieces of decommissioned automation equipment.
Several Files have been included in order to better itemize the break-down of the equipment and the items that need to be removed within the equipment sections, itemized, logged, and packaged for removal or reuse in the future. Disassembly of the system must be done with care and by skilled individuals to ensure that the integrity of the system components is maintained.
This includes the necessary provisions for disassembly of electrical, electronic, control and robotic components. In all cases these items should be packaged in accordance with the manufacturers requirements to ensure they can be transported without damage. For electronic related components, these should be removed in a manner that prevents static discharge or damage to the component and they should be packaged in a manner that is consistent with protecting the electronic integrity of the device (boards, controller cards, PLC, etc.).
3.2.1 Equipment Breakdown Identification
Provided in this statement of work is an AutoCAD version and a Pdf version of the system layout. These drawings have the conveyor itemized into two categories (Salvaged, Discard).
The drawing has had all conveyors (primarily downstream of the filling cells) identified with a number reference. Red numbers mark discards; black identify recommended salvage. Flexlink conveyor has not been included in this itemization. Flexlink conveyor within the filling modules is not to be removed and should remain within the modules.
An excel spreadsheet is provided that contains two worksheets with information on the conveyor and a number that corresponds to the identification location on the drawing. The contractor should ensure that during disassembly and packaging of the conveyor that these components are itemized in accordance with the listing provided. All Flexlink conveyor external to a module should be disassembled, itemized, and packaged independent of the conveyor identified in the two worksheets provided. This type conveyor is limited and has not been pre-itemized for this contract.
Also included in the excel spreadsheet is an additional worksheet that lists the robotic components that need to be removed from the system and packaged separately for reuse or redistribution. This listing provides the vendor the identification information of these components, the condition, the disposition as well as general notes, footprint, and estimated weight of the items for packaging and shipping purposes. Included in the spreadsheet is another copy of the entire system layout as well as additional worksheets that contain detailed specifications of many of the components identified within the equipment listing.
3.2.2 Equipment/Electronic Component Removal and Packaging
Since electronic products are considered fragile, the electronics product packaging must consider protection from extreme temperatures, mechanical damage, electrostatic discharge, and high frequency noise emission. Care should be taken by trained technicians to disassemble these components, inventory, and package them for future use. The contractor should provide packaging information for approval by VA PROJECT MANAGER in order to ensure it is appropriate for the needed application. In most instances the level of packaging will conform to an industry standard at a Level 3, Level 4 or Level 5 as needed. Below is a list of electronic components that have been identified for removal and packaging throughout the automated system:
Conveylinx Control Cards Sharp Max 20 Bagger (includes consumable feeder and storage portion) Zebra Printer Panther Industries Phantom Printer WAGO 750-8202 PLC Processor WAGO 750-1506 IO Module Banner Safety Controller SC26-2e Banner Safety Relay UM-FA-11A Pepperl Fuchs Sensor Interface VAA-4E4A-KE-ZE/R Imaging Source DFK 23UX236 Metaphase light MB-DAL205-W-24 Cap skew backlight WLAW105X180DQ Cap skew camera Cognex 821-0084-6R B Cognex cameras in UOU (Part number hidden need to disassemble) Matrix 300N 412-010 Lights in UOU ODDM-302-WHI-W Ricoh Printers CASI Totes
3.2.3 Packaged and Palletized Equipment and Materials
All items identified for packaging and palletization in accordance with manufacturers requirements in order to ensure the integrity of the equipment and materials remains in good working condition for future use or shipping. For all items determined to be handled in this manner the contractor will be required to be inventoried and documented in a manner that facilitates the process for final disposition of the item. The contractor will be required to take pictures in accordance with the GSA guidelines including the naming convention for these pictures, in order to expedite the final disposition of these items. These guidelines have been provided as attachments in section 3.2.4 of the statement of conditions.
Use of cameras and pictures at the CMOP facility is prohibited except for this intended purpose.
All pictures taken shall be reviewed and approved by the local CMOP Project Manager to ensure no private patient or employee information or images are present in the photographs.

The contract will set aside an initial estimated number of hours for this type work to support the subsequent APFS reconfiguration. Contractor should provide a unit cost (Hourly Rate) within the quote for this type work. For any work performed under this option, the contractor will provide a pre-approved estimate of the hours needed for the requested work and the hours actually expended by the contractor will be verified by the local project manager once the work commences.
3.3.2 Additional Automation assembly, Disassembly Work (Option 2) - This line item will be a no cost line item to the contract and held for the purpose of providing additional assembly, disassembly, packaging and relocation of automation and non-automation type items that are unforeseen at the time of the contract. These type jobs will be handled through an independent quote request for the scope of work identified within the option. All requirements of the base contract will remain in effect for any work awarded under this contract option.
4. Project Management Requirements/Site Conditions
It will not be acceptable for the pharmacy to be shut down to accomplish the disassembly and packaging. Much of the work can be accomplished during normal operating hours. Any activities that would require disruption to the current operation will need to be planned and scheduled during non-operating hours or weekends. This should be coordinated and approved by the local VA PROJECT MANAGER in advance of this work taking place.
4.1. Project Schedule
A project schedule shall be submitted to the VA CMOP PM or their designated representative as part of the contract award. The schedule shall be revised and re-issued bi-weekly.
