B.2 STATEMENT OF WORK

The St. Cloud Veterans Affairs Health Care System (SCVAHCS) has a requirement for a Wandering Management System at 4801 Veterans Drive, St. Cloud, MN 56303.

The system should include but is not limited to the following characteristics:

1. PRODUCTS:

Controllers for delayed egress magnetic locks on 8 doors Magnetic locks which meet NFPA 101 delayed egress requirements 8 doors Controllers for 9 elevators 15 Wrist transmitters Doors should be able to lock and produce audio alarm based on wrist transmitter proximity Elopement prevention with door locking upon patient approach Patient “escort” ability Wrist transmitter must be detected by the controller sensors Wrist transmitter battery life should exceed 90 days Wrist transmitters should be able to be added after initial installation without expanding network hardware Licenses for wander management system must be included Patient ID must be available at door and nursing station controllers Software should be able to apply exceptions to patient wrist transmitter for door controllers Software should be able to provide electronic reports on wrist transmitter alert trends Software should have ability to set alert parameters Server required to provide software Individual controllers must be networked to the server at a remote location on the facility Provide power supplies, electrical cables, communication cables for a complete working system Provide 7 desktop workstations running on Windows 10 (or newer OS version), along with keyboard, mice and monitor Wrist transmitters should be tamper-proof so that they are not easily removable for dementia/vulnerable patients (require heavy duty scissors to remove) Must have Fire Alarm “dry contacts” for releasing doors during Fire Alarm Activation by floor of activation NFPA 72 and NFPA 101 Must integrate with Rauland R5000 Nurse Call System on each unit works independently of each other Include alarm types: wander alarm, low battery on wrist transmitters, door ajar, patient loiter Full reporting capabilities via Wandering System Software Provide as-built drawings of the completed Wandering System

2. INSTALLATION

Contractor will furnish all labor, transportation, parts, test equipment, tools, and expertise necessary to provide and install Wandering Management System at the St. Cloud Veterans Affairs Health Care System (SCVAHCS).

Work shall be performed during normal business hours Monday through Friday 8:00 am to 4:30 pm excluding Federal Holidays, unless otherwise authorized by the VA POC

Contractor shall provide and wear appropriate Personal Protective Equipment (PPE) when necessary as required by VA guidelines

St. Cloud VA Contractor badge shall be worn by all Contractor employees while on premises

Contractor is responsible for the shipping costs and logistics for all tools and equipment necessary for the performance of this contract

All contractor tools will be kept in personal control while on VA Property

Contractor’s equipment, supplies and tools will be brought to and from the work site daily; no overnight storage on-site

Contractor will use existing door controller wall box locations

Contractor is responsible to remove components of the old system which includes cabling infrastructure

Contractor is responsible to provide any patchwork on walls

All penetrations needed through fire walls, ceiling, floors and cable trays require installation or reinstallation of approved firestop by a trained firestop installer

Contractor will remove all packing material and debris for proper disposal off-site following environmental guidelines

On-site installation and set-up once the items are delivered to include networking using the VA’s Information Technology (IT) infrastructure (vendor will need to provide new cables from controllers/detectors to IT data closet).

Contractor must coordinate with IT/Daniel Dillman for any networking/IP changes

As part of installation, high efficiency particulate air (HEPA)-grade dust carts with HEPA filtration shall be provided by the vendor to prevent dust from entering patient care areas when working above the ceiling, or any dust producing wall work

Cables shall be marked at each end indicating the termination point of the other end.

Network cabling, terminations, and any patch panels used shall be category 6 cable (CAT6) certified.

All cables shall be terminated by the Telecommunications Industry Association 568B (TIA568B) standard

Upon completion of contract, contractor shall demonstrate, test and verify a proper and fully functional Wandering Management System with VA POC

Infection Control Risk Assessment (ICRA) REQUIREMENTS FOR INSTALLATION

Contractor shall implement the ICRA requirements as identified on the Matrix and below. Contractor shall assess their installation activity per the matrix. They shall implement the prevention measures as required. During installation the vendor shall ensure that no dust contaminates the adjacent operation of the VA. Dust from vendor installation activities can contain microorganism that cause negative health impacts to patients and staff. All ICRA measures shall be implemented prior to starting dust creating activity. When opening ceilings and/or walls for product install, the vendor shall physically contain the dust created via a sealed containment system. This can be a mobile cart style or a built-in place system. The vendor shall include a HEPA vacuum system to treat the air within the sealed containment. Prior to exiting the containment system or dissembling the containment system the vendor shall clean themselves, tools, material, etc. and the work site.

3. TRAINING

Vendor will provide on-site Wander Management System Training for eighty (80) clinical staff. Provide 8 sessions, 1 hour per each session, on 4 different days during the hours of 6-7AM and 3-4PM.

Training to include operations, service, preventive maintenance, and software updates on the equipment. Certifications for maintenance, preventive maintenance and updates are to be issued to three (3) VA Maintenance Staff.

After training, VA staff will have the authorized ability to install all software updates and perform functionality changes to be provided by the equipment manufacturer

4. SUPPORT AGREEMENT

Vendor will provide a 12-month support agreement for the unit to include repair and software and configuration support after Go-Live

The contractor will respond by phone at no additional charge, 24/7 with on-site repair within four (4) hours for system failure

5. REPORTING REQUIREMNETS

Work is to be coordinated with VA POC: Curtis Gregory at (320)252-1670, ext. 7245 Upon arrival at the St. Cloud VA, the contractor shall be required to check in with Bio Electric in Bldg. 48 room 214. Contractor badges are issued in B-3, second floor, Facilities Maintenance and Operations Office. Wear of issued contractor badge is mandatory.

When service is completed, manufacturer qualified vendor technician shall document services rendered on a final commissioning report (FCR) and submit to the Bio Electric Technician who set-up the service. The FCR can be submitted when logging out with Bio Electric, via an internet web site or e-mail. FCR should be submitted no later than 5 business days after service is complete. Preferred method of delivery is via email to vhastcfmbio-electric@va.gov

6. DEFECTS

Contractor shall immediately, but not later than 24 consecutive hours after discovery, notify the contracting officer (CO) and St. Cloud VA Point of Contact, (in writing), of the existence of the development of any defects in, or repairs required to the scheduled equipment which the contractor considers he/she is not responsible for under the terms of the contract.

Contactor shall furnish the CO with a written estimate of the cost to make necessary repairs

7. PARKING, SMOKING and WEAPONS:

It is the responsibility of the contractor to park in the appropriate designated parking areas. VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions.

Smoking is prohibited on all VA property

Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search.

8. CONTRACTOR QUALIFICATIONS

Contractor must have full time staff that is “fully qualified”

“Fully qualified” is based upon training and on experience in the field. For training, the FSE (s) has successfully completed a formalized training program for the equipment identified in this solicitation as required by the Original Equipment Manufacturer (OEM) and equal to what the manufacturer provides their own field service personnel.

Contractor must provide, upon request, evidence of appropriate training of any FSE(s) providing services under terms of the contract. Subcontracting of any ensuing award of this solicitation will not be allowed without written permission of the Contracting Officer.

Contractor must have access to all OEM proprietary information due to the critical nature of these services. Examples of this information would be software updates and factory service bulletins that describe updates/modifications needed to make the equipment under this contract safe for use.

C.2 52.219-14 LIMITATIONS ON SUBCONTRACTING (MAR 2020) (JUN 2020) (DEVIATION)

(a) This clause does not apply to the unrestricted portion of a partial set-aside.

(b) Definition. “Similarly situated entity,” as used in this clause, means a first-tier subcontractor, including an independent contractor, that—

(1) Has the same small business program status as that which qualified the prime contractor for the award (e.g., for a small business set-aside contract, any small business concern, without regard to its socioeconomic status); and

(2) Is considered small for the size standard under the North American Industry Classification System (NAICS) code the prime contractor assigned to the subcontract.

(c) Applicability. This clause applies only to—

(1) Contracts that have been set aside for any of the small business concerns identified in 19.000(a)(3);

(2) Part or parts of a multiple-award contract that have been set aside for any of the small business concerns identified in 19.000(a)(3);

(3) Contracts that have been awarded on a sole-source basis in accordance with subparts 19.8, 19.13, 19.14, and 19.15;

(4) Orders expected to exceed the simplified acquisition threshold and that are—

(i) Set aside for small business concerns under multiple-award contracts, as described in 8.405-5 and 16.505(b)(2)(i)(F); or

(ii) Issued directly to small business concerns under multiple-award contracts as described in 19.504(c)(1)(ii);

(5) Orders, regardless of dollar value, that are—

(i) Set aside in accordance with subparts 19.8, 19.13, 19.14, or 19.15 under multiple-award contracts, as described in 8.405-5 and 16.505(b)(2)(i)(F); or

(ii) Issued directly to concerns that qualify for the programs described in subparts 19.8, 19.13, 19.14, or 19.15 under multiple-award contracts, as described in 19.504(c)(1)(ii); and

(d) Independent contractors. An independent contractor shall be considered a subcontractor.

(e) Limitations on subcontracting. By submission of an offer and execution of a contract, the Contractor agrees that, in performance of a contract assigned a North American Industry Classification System (NAICS) code for—

(1) Services (except construction), it will not pay more than 50 percent of the amount paid by the Government for contract performance to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count towards the prime contractor’s 50 percent subcontract amount that cannot be exceeded. When a contract includes both services and supplies, the 50 percent limitation shall apply only to the service portion of the contract;

(2) Supplies (other than procurement from a non-manufacturer of such supplies), it will not pay more than 50 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count towards the prime contractor’s 50 percent subcontract amount that cannot be exceeded. When a contract includes both supplies and services, the 50 percent limitation shall apply only to the supply portion of the contract;

(3) General construction, it will not pay more than 85 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count towards the prime contractor’s 85 percent subcontract amount that cannot be exceeded; or

(4) Construction by special trade contractors, it will not pay more than 75 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count towards the prime contractor’s 75 percent subcontract amount that cannot be exceeded.

(f) The Contractor shall comply with the limitations on subcontracting as follows:

(1) For contracts, in accordance with paragraphs (c)(1), (2), (3), and (6) of this clause— [Contracting Officer check as appropriate.] _ By the end of the base term of the contract and then by the end of each subsequent option period; or _ By the end of the performance period for each order issued under the contract.

(2) For orders, in accordance with paragraphs (c)(4) and (5) of this clause, by the end of the performance period for the order.

(g) A joint venture agrees that, in the performance of the contract, the applicable percentage specified in paragraph (e) of this clause will be performed by the aggregate of the joint venture participants.

(End of Clause) C.3 52.222-19 CHILD LABOR—COOPERATION WITH AUTHORITIES AND REMEDIES (JUL 2020) (DEVIATION)

(a) Applicability. This clause does not apply to the extent that the Contractor is supplying end products mined, produced, or manufactured in—

(1) Israel, and the anticipated value of the acquisition is $50,000 or more;

(2) Mexico, and the anticipated value of the acquisition is $83,099 or more; or

(a) No used, refurbished, or remanufactured supplies or equipment/parts shall be provided. This procurement is for new Original Equipment Manufacturer (OEM) items only. No gray market items shall be provided. Gray market items are OEM goods intentionally or unintentionally sold outside an authorized sales territory or sold by non-authorized dealers in an authorized sales territory.

(b) No counterfeit supplies or equipment/parts shall be provided. Counterfeit items include unlawful or unauthorized reproductions, substitutions, or alterations that have been mismarked, misidentified, or otherwise misrepresented to be an authentic, unmodified item from the original manufacturer, or a source with the express written authority of the original manufacturer or current design activity, including an authorized aftermarket manufacturer. Unlawful or unauthorized substitutions include used items represented as new, or the false identification of grade, serial number, lot number, date code, or performance characteristics.

(a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned Small Business Set-Aside.

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. (d) Comptroller General Examination of Record. VA INFORMATION AND INFORMATION SYSTEM SECURITY/PRIVACY language FOR Inclusion into CONTRACTS, as appropriate

1. GENERAL

Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.

ACCESS to VA INFORMATION AND VA INFORMATION SYSTEMS

1. A contractor/subcontrator shall request logical (technical) or physical access to VA information and VA information systems for their employees, subcontractors, and affiliates only to the extent necessary to perform the services specified in the contract, agreement, or task order.

1. All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures.

Contract personnel who require access to national security programs must have a valid security clearance. National Industrial Security Program (NISP) was established by Executive Order 12829 to ensure that cleared U.S. defense industry contract personnel safeguard the classified information in their possession while performing work on contracts, programs, bids, or research and development efforts. The Department of Veterans Affairs does not have a Memorandum of Agreement with Defense Security Service (DSS). Verification of a Security Clearance must be processed through the Special Security Officer located in the Planning and National Security Service within the Office of Operations, Security, and Preparedness.

Custom software development and outsourced operations must be located in the U.S. to the maximum extent practical. If such services are proposed to be performed abroad and are not disallowed by other VA policy or mandates, the contractor/subcontractor must state where all non-U.S. services are provided and detail a security plan, deemed to be acceptable by VA, specifically to address mitigation of the resulting problems of communication, control, data protection, and so forth. Location within the U.S. may be an evaluation factor.
