About this file
This is a Quality Assurance Surveillance Plan (QASP) attachment for semen analysis sendout laboratory services under VISN 22's procurement for semen and vasectomy test kits. The QASP establishes performance standards and inspection requirements for the contractor's delivery of semen analysis services, including quality metrics for specimen collection, processing, analysis accuracy, and reporting timelines. The document outlines specific performance levels that must be met, defect thresholds, and monitoring procedures to ensure the contractor maintains compliance with established quality benchmarks throughout contract performance.
The QASP details surveillance activities focused on verifying that semen samples are properly collected, stored, and analyzed according to clinical standards, with particular emphasis on result accuracy and turnaround time for delivering analysis reports to Veterans Affairs facilities. Performance monitoring includes inspection of documentation, verification of testing protocols, and assessment of contractor responsiveness to customer inquiries. The plan establishes consequences for failing to meet quality standards, including potential service adjustments or remediation requirements to ensure Veterans Health Administration facilities receive reliable laboratory services for patient diagnostic needs.

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.
This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.
