About this file
This is a Past Performance Questionnaire form used to evaluate contractor performance on federal contracts. The form collects comprehensive information about a completed or ongoing contract engagement, including contractor name, role, contract number, procuring agency details, period of performance, and total dollar value with modifications. It captures contract type (Firm Fixed Price, Time & Materials/Labor Hour, Cost Reimbursement, or Other) and requests a detailed description of the contract service and scope.
The evaluation component requires respondents to rate contractor performance across seven factors: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance. Each factor must receive a rating using the provided guidelines: Unsatisfactory, Marginal, Satisfactory, Very Good, or Exceptional. Mandatory comments must accompany each rating, with provisions for additional attached sheets if needed. The form concludes with a binary question asking whether the evaluator would select the contractor again (Definitely, Probably, or Definitely Not) with required explanation. The rating guidelines provide specific performance criteria for each evaluation factor and rating level to ensure consistent and justified assessments across all contractor evaluations.
