About this file
This is Amendment Number 0003 to Solicitation 36C26226Q0387, issued by the Department of Veterans Affairs (VA), Veterans Health Administration, Veterans Integrated Service Network 22, Mesa Annex. The amendment extends the offer receipt deadline to March 18, 2026, at 1:00 PM MST and updates the Statement of Work (SOW) while responding to contractor questions regarding the medical gas testing and certification services.
The solicitation requires annual 100% testing and certification of medical gas systems at Phoenix VA Health Care System, Carl T. Hayden Medical Center, covering oxygen, medical air, vacuum, waste anesthesia, and nitrous oxide outlets across four buildings: the main facility (Building 1), Community Living Center (Building 16), Ambulatory Care Center (Building 8), and Dental Clinic (Building 29). Testing must include outlet flow rate verification, purity checks, static pressure measurement, leak detection, and annual master alarm panel testing at three designated locations. The contractor must employ NFPA-qualified certifiers holding current ASSE/IAPMO/ANSI 6040 or 6030 credentials. The contract includes a one-year base period (April 1, 2026–March 31, 2027) with four one-year options. Pricing items include: Item 0001/1001/2001/3001/4001 for annual medical gas system testing and certification; Item 0002/1002/2002/3002/4002 for annual master alarm panel testing; and Item 0003/1003/2003/3003/4003 for miscellaneous/emergency services. The NAICS code is 541380 (Testing Laboratories and Services), and the PSC code is H165. All offers must be submitted via email to Felicia Simpson at Felicia.Simpson@va.gov with complete documentation under Section E and acknowledgment of all amendments. The amendment includes detailed Q&A responses specifying outlet/valve counts by building (Main Building: 1,698 outlets, 216 valves, 101 area alarms, 60 master alarms; Building 8: 105 outlets, 44 valves, 41 area alarms, 69 master alarms; Dental Clinic: 53 outlets, 13 valves, 11 area alarms, 10 master alarms; Onyx Clinic: 72 outlets, 9 valves, 9 area alarms) and calibration equipment serial numbers for carbon dioxide and dew point meters.

2. How many Nonstationary booms / articulating assemblies does the facility have that need to be tested?
ANSWER: 0 (there are 10 booms in OR surgical area , none contain medical gases that we are aware of, all are just lights) The Outlets are located in the ceilings in OR
3. According to the list of tasks, "the Contractor shall check flow rate at each outlet. Oxygen, Medical Air and Nitrous Oxide must deliver a minimum of 100 liters per minute (1PM). Surgical outlets must deliver a minimum of 142 lpm. Medical Vacuum outlets must provide a minimum of 85 lpm." Does the Medical Gas Report need to document the actual flow rate at each Medical Gas Outlet or is pass/fail adequate?
ANSWER: Pass/Fail is adequate
4. Does each building need to have its own separate medical gas inspection report, or would the VA prefer to have them all on a single report?
ANSWER: Historically they are on the same report tabbed and split into the different sections
5. Does the VA want a pre-determined amount of money quoted to make minor repairs such as damaged/leaking outlets during inspection?
ANSWER: Yes, and the solicitation will be updated to reflect miscellaneous/emergency’s CLIN.
6. The amendment provides a list of outlets and valves for the Main Building, Bldg 8 Ambulatory Care Center, and Dental Clinic (Building 29). However, the statement of work also includes the Community Living Center (Building 16). Are there any outlets/inlets or related equipment in the Community Center? If so, please provide an inventory.
ANSWER: The outlets for bldg. 16 are calculated within the main building numbers and have historically been a part of the Main building report section.
7. The NFPA 99 dictates the quality of medical air. I would recommend you test to this relevant code and not the one mentioned in the SOW.

ANSWER: NFPA 99 2024 5.1.3.6.1* Quality of Medical Air.
Medical air shall be required to have the following characteristics:
(1) It shall be supplied from cylinders, bulk containers, or medical air compressor sources, or it shall be reconstituted from oxygen USP and oil-free, dry nitrogen NF.
(2) It shall meet the requirements of medical air USP.
(3) It shall have no detectable liquid hydrocarbons.
(4) It shall have less than 25 ppm gaseous hydrocarbons.
(5) It shall have equal to or less than 1 mg/m3 (6.85 × 10−7 lb/yd3) of permanent particulates sized 1 micron or larger in the air at normal atmospheric pressure.
The Contractor shall test each outlet (except Medical Vacuum) for suspended particulate by passing a gas stream through a particulate filter at a maximum flow rate for 5 seconds. Any presence of discoloration or visible particulate matter shall be documented.
8. This statement above is not a requirement of any Med Gas Inspection. There is no requirement for particulate sampling on an annual inspection. It is also not a requirement of a new construction certification for every outlet. It would make this annual cost prohibitive to perform to complete this level of testing. As an example of when it's required, on a new medical gas construction certification, the requirement for particulate sampling is:
5.1.12.4.7.2 Twenty-five percent of the zones shall be tested at the outlet most remote from the source.
ANSWER: Agree with reviewer. Remove from SOW The Prime Contractor shall have completed NFPA 99C Medical Gas Certifier training and provide current certificates of qualification.
9. This language is outdated (NFPA 99C). Current certifications that allow a contractor to do an annual inspection are (from the NFPA 99 2024): 6040 Medical gas maintenance personnel or 6030 Medical gas verifiers.
ANSWER: 5.1.14.4.2.5 Qualifications
(C) Persons maintaining these systems shall be qualified to perform these operations.
(D) Appropriate qualifications shall be demonstrated by any of the following:
a. A documented training program acceptable to the health care facility by which such persons are employed or contracted to work with specific equipment as installed in that facility.
b. Credentialing to the requirements of ASSE/IAPMO/ANSI 6040, Professional Qualifications Standard for Medical Gas Systems Maintenance Personnel, and technically competent on the specific equipment as installed in that facility.
c. Credentialing to the requirements of ASSE/IAPMO/ANSI 6030, Professional Qualifications Standard for Medical Gas Systems Verifiers, and technically competent on the specific equipment as installed in that facility.
The Contractor shall annually inspect and test for leaks, all nonstationary booms and articulating assemblies, other than head walls utilizing flexible connectors per manufactures recommendations.
10. We need a count of these assemblies for pricing.
ANSWER: This answer is the same as #2 The Contractor shall annually calibrate carbon dioxide and dew point meters and provide documentation of the calibration.
11. This isn't an issue, but it is typically part of a maintenance or preventative maintenance contract, not the annual. We will need models and serials # on the devices that require calibration.

The contractor will complete inspection annually on the same date every year (+/- 30 days from previous inspection). The last inspection took place on 6/27/2025
12. Is that the start or end date? Also that is a Saturday which is typically outside the VA work times for vendors.
ANSWER: The vendor has from 5/27/26 to 7/27/26 to complete the inspection for year 1.
13. We need a current count or inventory of medical gas systems, zone valve, area alarms, Booms or articulating assemblies (mentioned above) and a count of all patient outlets to determine annual pricing. A previous inspection report would also have this information.
ANSWER: Same as #1.
AMENDMENT THREE IS TO UPDATE THE SOW AND TO RESPOND TO QUESTIONS AND TO EXTEND THE CLOSING DATE OF THE SOLICITATION.
