About this file

This is Amendment 0003 to solicitation 36C26225R0004 for Community Based Outpatient Clinic (CBOC) services in El Centro, California, issued by the Department of Veterans Affairs Network Contracting Office 22. The amendment makes three key changes: (1) replaces Section B.2 Schedule of Services and Price to reflect a single per member per month (PMPM) payment structure for primary care services across the base year and nine option years, with estimated quantities ranging from 1,829 to 1,910 members; (2) removes the separate Mental Health LINE Item Number (LIN) billing requirements from Section B.3 Performance Work Statement; and (3) extends the proposal due date to January 14th at 11:00am.

The amendment also addresses two contractor questions, clarifying that the VA is consolidating primary care and PC-MHI services into a single PMPM rate due to historically low PC-MHI utilization (1.74%) at the El Centro location, though usage is expected to increase with on-site staff. Additionally, contractors wanting to propose multiple CBOC locations must submit separate complete proposals for each location rather than combining locations in one proposal.

c. Section B.3 PERFORMANCE WORK STATEMENT, Paragraph 19. BILLING AND INVOICING PROCEDURES, Mental Health LIN, is deleted in its entirety.

The billable roster for the MH LIN (if there is a MH LIN) will include all patients provided at least one primary care-mental health integration (PCMHI) and/or general/specialty MH service by the contractor’s professional MH staff within the previous 12 months under a 500 series stop coded clinic with a separate CPT coded encounter.

The ONLY time the contractor may bill against the MH LIN is when PCMHI, and/or general/specialty MH services are provided by the contractor’s professional MH staff under the 500 series (MH) stop coded clinics with separate CPT coded encounters. MH screening and services provided as part of primary care in a 300 series stop coded clinic are NOT billed against the MH LIN.

“Professional MH Staff” eligible to provide services that will result in billing against the MH LIN are psychologist, psychiatrist, licensed social worker practicing at the independent level, MH nurse practitioner (NP), mental health physician assistant (PA), licensed professional mental health counselor (LPMHC) and marriage and family therapist.

2. Additional Questions and Answers.

Question 1: VA’s historic data available for PC-MHI services for El Centro reflects that only 1.74% of veterans at the site are currently engaged in PC-MHI, significantly less than the 10% VA is recommending contractors use for pricing. Would VA amend the solicitation and contract to use a single primary care price inclusive of PCMHI ?

Answer: Yes. See amendment identified above removing language from the solicitation PWS and the attached updated section B.2 SCHEDULE OF SERVICES and PRICE and attached updated section D.19 PRICE WORKSHEET that now reflects only the PMPM line item. However, the VA does anticipate that the number of patients at the El Centro CBOC requiring PCMHI will increase because of the available on-site PCMHI staff (currently this is provided via tele-mental health).

Question 2: Can you please clarify if vendors are allowed to submit more than one location? If so, do the page restrictions apply to each location submitted? Lastly, will you require a complete and separate proposal for a second location, or can vendors submit one proposal with multiple locations?

Answer: If an offeror wants to offer more than one location for the proposed CBOC, then they would submit two separate and complete proposals, both meeting all the Terms and Conditions and requirements of this solicitation.
