E.2 NOTIFICATIONS TO OFFERORS
ADDENDUM TO FAR 52.212-1 INSTRUCTIONS TO OFFERORS – COMMERCIAL ITEMS (OCT 2015)
The following provisions are incorporated into 52.212-1 as an addendum:
1. GENERAL INSTRUCTIONS FOR PROPOSAL SUBMISSIONS
a. Period of Acceptance of Offers. The Offeror agrees to hold prices in its offer firm for a period of one-hundred-twenty (120) calendar days from the date specified for the receipt of offers.
b. Submitting your proposal: Proposals are due on Monday, August 20, 2018 at 9:00 AM PDT Submit your company’s proposal by responding to this RFP via email only to Monica Griffin, Contracting Officer at monica.griffin@va.gov. All proposals/attachments in response to this RFP shall be in Adobe or Microsoft Office Word Format. It is the offeror’s responsibility to confirm that all required documents are included and received by the solicitation due date.
c. Offerors shall be registered and verified as a Service Disabled Veteran Owned Small Business in www.vetbiz.gov at the time of proposal submission.
d. Offerors may submit a proposal to provide courier services for one, more than one, or all of the following: laboratory services BASE YEAR: CALab (Line Items 1-34), SDSPS (Line items 1-4), LBSPS (Line items 1-2), OPTION YEAR I: CALab (Line Items1-34), OPTION YEAR II-SDSPS (Line Items 1-4). LBSP (Line Items1-2), OPTION YEAR III CALab (Line Items 1-34), SDSPS (Line items 1-4), -LBSPS (Line Items 1-2), CALab (Line Items 1-34), SDSPS (Line Items 1-4), LBSPS (Line Items 1-2), AZLab (Line items 35-44) and OPTION YEAR IV CALab (Line Items 1-34), SDSPS (Line Item 1-4), LBSPS (Line items 1-2), and/or AZLab (Line Items 35-44).
2. REQUIREMENTS FOR SUBMITTAL: The following subsections are the requirements to be submitted by the offeror.
2.1.1 Offeror must provide a cover sheet to include the following:
2.1.2 Solicitation No. 36C26218R0723
2.1.3 Company’s name, address, point-of-contact, phone number, email address
2.1.4 DUNS number
2.1.5 Offeror must complete and provide each of the following:
2.1.6 Page 1 - SF1449, block 30(a), 30(b) and 30(c).
2.1.7 Page 3 - Section B.1(1)(a) Contract Administration Data and subsection (5) Acknowledge of Amendments. Amendments – SF 30, if any, block 15A, 15B and 15C.
2.1.8 Offerors shall complete and submit the Schedule of Services in section B, and the Price Worksheet at section D.
2.1.9 Complete and sign the Business Associate Agreement.
2.1.10 Compliance with the Limitations on Subcontracting. To aid the contracting officer in making a responsibility determination, offerors shall provide the following:
2.1.10.1 Narrative that identifies any intended subcontractors
2.1.10.2 The services that each subcontractor will provide
2.1.10.3 Socio-economic status of the subcontractor (e.g. large, small, VOSB, SDVOSB) Responsibility Matter. Offerors proposing services for BASE YEAR: CALab (Line Items 1-34), SDSPS (Line items 1-4), LBSPS (Line items 1-2), OPTION YEAR I: CALab (Line Items1-34), OPTION YEAR II-SDSPS (Line Items 1-4). LBSP (Line Items1-2), OPTION YEAR III CALab (Line Items 1-34), SDSPS (Line items 1-4), -LBSPS (Line Items 1-2), CALab (Line Items 1-34), SDSPS (Line Items 1-4), LBSPS (Line Items 1-2), AZLab (Line items 35-44) and OPTION YEAR IV CALab (Line Items 1-34), SDSPS (Line Item 1-4), LBSPS (Line items 1-2), and/or AZLab (Line Items 35-44) shall provide a narrative demonstrating that they have the necessary resources to provide these services in order to aid the contracting officer in making a responsibility determination. Specifically, the offeror shall provide a narrative describing their ability to provide the necessary staff and vehicles to support all six (6) healthcare systems at the locations identified in the SOW.
3. Price Evaluation. Offerors shall submit their pricing by completed the schedule of services and price in section B and by completing the worksheets in section D. Pricing will be evaluated using the price worksheets included in section D.

E.3 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)
(a) Basis for Award. The Government shall award a contract(s) resulting from this solicitation to the responsible offeror (s) whose offer conforming to the solicitation offers the lowest price for each category of services and that price is found to be reasonable. After making selection of the lowest priced offeror, for each category of services, the Contracting Officer will determine if the selected offeror is responsible in accordance with the General Standards of Responsibility found in FAR 9.104-1.
(b) Price Evaluation. Prices shall be evaluated separately for laboratory courier services BASE YEAR: CALab (Line Items 1-34), SDSPS (Line items 1-4), LBSPS (Line items 1-2), OPTION YEAR I: CALab (Line Items1-34), OPTION YEAR II-SDSPS (Line Items 1-4). LBSP (Line Items1-2), OPTION YEAR III CALab (Line Items 1-34), SDSPS (Line items 1-4), -LBSPS (Line Items 1-2), CALab (Line Items 1-34), SDSPS (Line Items 1-4), LBSPS (Line Items 1-2), AZLab (Line items 35-44) and OPTION YEAR IV CALab (Line Items 1-34), SDSPS (Line Item 1-4), LBSPS (Line items 1-2), and/or AZLab (Line Items 35-44). The lowest priced offeror for each category of services (Laboratory, SPS Long Beach and SPS San Diego) shall be awarded an IDIQ contract if they meet the general standards of responsibility, offer the lowest price, and that price is found to be reasonable. Pricing will be evaluated using the price worksheets included in section D.
(c) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(d) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
