PERFORMANCE WORK STATEMENT
For Veterans Integrated Service Network (VISN) 21, Sierra Pacific Network The U.S. Department of Veterans Health Administration (VHA) 391 Taylor Blvd., Suite 200 Pleasant Hill, CA 94523
PRESCRIPTION EYEGLASSES AND OPTICIAN SERVICE
Date: 12/3/2020
1. SCOPE
1.1 The Department of Veterans Affairs (VA) has an ongoing program to provide prescription eyeglasses to veterans. VA anticipates award of an Indefinite–Delivery Indefinite- Quantity (IDIQ) type contract on a fixed price basis for the VA Central California Health Care System (VACCHCS), VA Northern California Health Care System (VANCHCS), VA San Francisco Health Care System (VASFHCS), and the Sierra Nevada VA Health Care System (VASNHCS). The awarded contract will contain one base period of 1 year beginning on (TBD) with four (4) one-year options to extend. The Department of Veterans Affairs reserves the right to exercise the option years, subject to the availability of funds. For purposes of evaluation, if the first one-year optional extension period (52.217-8) is exercised, pricing shall remain unchanged.
1.2 The contract is intended for use by four different VA Health Care System Medical Centers and Outpatient Clinics located within VISN 21. New clinics associated with major sites may be added during the life of this agreement. Currently these four Health Care Systems provide approximately 45,000 pairs of eyeglasses to eligible veterans annually (fabrication volume by Health Care System is in paragraph 2.4).
1.3 All items provided by the contractor shall be new. The contractor shall provide completely assembled eyeglass frames placed in standard alignment and include mounting of lenses. All items provided by the contractor shall include fitting and adjustment services and a minimum of one-year warranty in accordance with (IAW) paragraph 4.3.
1.4 No retail sales are allowed on government property to veterans, veterans’ families, friends, and employees.
2. DELIVERY
2.1 The contractor shall deliver all items no later than SEVEN (7) BUSINESS DAYS after receipt of both the prescription consult (VA Form 513) and purchase order issued by Government authorized personnel against the contract. Delivery of eyeglasses shall be to the designated address of the veteran or contracted ordering facility or, as indicated on the prescription (CPRS Prosthetics ‘Eyeglass’ consult/VA Form SF 513)/purchase order issued by the Government. All eyeglasses shall be delivered as directed on the purchase order and delivery shall be made using traceable means. There may be occasion for the Government to request receipt of an expedited delivery. In such cases, the ordering activity will telephonically contact the contractor and notify the contractor of such a requirement.
2.2 The expectation is that the contractor will monitor their stock and propose a substitute before that style of frame becomes unavailable. The contractor shall notify the VA Prosthetics Contracting Officer Representative (COR) via email or fax in the event of backorders or other delays. If there is a backorder on any frame style, the contractor must contact the VA Prosthetics COR no later than 24 hours after the contractor is notified by the frame manufacturer/supplier. The contractor shall offer the VA/Veteran a substitute of equal or higher quality. The contractor’s notification shall include the cause for delay, remedy date and date of expected shipment to the designated address as indicated on the order. Failure by the contractor to provide a written notification within twenty-four (24) hours as specified above constitutes a performance failure and the Prosthetics COR will notify the CO, who in turn will undertake any action authorized pursuant to the contract or otherwise in accordance with applicable law and regulations. The COR will document in their contract file when frames are substituted. When a frame substitution is made, the frame will be the same price and of equal or higher quality than the frame it is substituting and approved by the COR/CO before the frame order is placed.
2.3 The contractor shall be responsible for informing the Prosthetics COR of any Government deficiencies in writing within 24 hours in such cases as missing information, illegible prescriptions and ambiguous authorization for eyeglasses, illegible handwriting, and inability to reach the Government representative, payment delays or other issues. The contractor’s failure to notify the Prosthetics COR will not be considered an excusable delay and subject circumstances should not be alleged to have prevented the contractor from delivering the items within the time required under this contract.
