About this file

This is a Price Cost Schedule template for Request for Proposal No. 36C26026R0117, which is a construction project (663A4-27-001) for Under Dispenser Containment (UDC) and Sump Installation at the VA Puget Sound Health Care System's American Lake Campus in Tacoma, Washington. The schedule requires offerors to provide fixed-price line item costs broken down by 26 Construction Specification Institute (CSI) Divisions, including General Requirements, Existing Conditions, Concrete, Plumbing Facility Fuel Piping, Electrical, Electronic Safety and Security, Earthwork, Exterior Improvements, and Utilities. Additional cost categories include mobilization/demobilization, insurance/bonds, taxes/fees, operations and maintenance manuals, red line drawings, overhead rate, and profit rate. The template specifies a 90-day period of performance and requires offerors to complete all yellow-highlighted areas; blank entries will render the proposal noncompliant. The principal NAICS code is 237120 (Oil and Gas Pipeline and Related Structures Construction), and the single line item covers the complete UDC Repair and Sump Installation work priced on a job basis. Offerors must accurately verify all formulas and transfer totals from the Proposal Price Schedule tab to the Items tab, as both tabs are mandatory for proposal compliance. All pricing is fixed-price, and field office overhead and general conditions are treated as a single cost category applied only once to the contract and any changes.

Instructions

INSTRUCTIONS
1Awardee is Responsible for determining accuracy of formulas to calculate all totals as well as accurately transfering totals from the Proposal Price Schedule tab to the Items tab.
2The term “Division” used with each line item in the Proposal Price Schedule refers to the general construction industry term and association with the Construction Specification Institute or common CSI specification breakdown of work actions. These Divisions roughly match those presented to the Contractors when they review the contract document specification binders for this project as created by the architect of record.
3The proposal price schedule is for informational purposes only and does not constitute separate billable deliverables.
4Field office overhead and general conditions are considered to represent the same category of costs and the terms are used interchangeably. As such, this category of costs will be applied only once to the contract and to any changes, if applicable.
5All pricing is fixed price.
6The Items tab identifies the services to be acquired in this solicitation and is required per FAR 4.1001.
7Both the Proposal Price Schedule tab and the Items tab are required to be fully completed for a proposal to be considered compliant with the proposal submission instructions. Each tab has yellow highlighted areas indicating where information shall be entered by the offeror. All yellow highlighted areas must be completed. If not applicable to the offer, N/A or $0.00 shall be entered. A blank space will be considered as incomplete and the proposal will be considered noncompliant.
