The purpose of Amendment 0 003 is to extend the quote due date and update Quotation Submission Checklist requirement.

6. Quotation Submission Checklist:

6.1 Specific Instructions: Submit quotations via email to Contract Specialist Andrew O’Neal at Andrew.O’Neal@va.gov with Subject: 36C26022Q0030. All quotes shall be legible and prepared in the format referenced below in Parts I thru V below.

______Part I EXECUTED REQUEST FOR QUOTATIONS: Quoter shall complete blocks 17a (in block 17a include company name, address, phone number, corresponding Dun and Bradstreet number, and Tax ID Number), 30a, 30b, and 30c, on SF 1449, which is Page 1 of this Request for Quotations. Quoter must acknowledge any Amendments that may be issued.

_______Part II TECHNICAL: There are Four (4) technical subfactors. Quoter must meet or exceed the requirements listed in the Statement of Work (SOW).

1)Technical Subfactor One: Vehicles/Drivers/Specimen Requirements: Quoters must submit a capability statement demonstrating their ability to provide drivers and vehicles, as well as maintain the specimen temperature requirements during transport to meet the courier requirements per the Statement of Work.

2)Technical Subfactor Two: Delivery Schedule Quoters must show their ability to provide the delivery schedule requirements as it relates to providing delivery of laboratory, radiology and pharmaceutical specimens to patient homes and facilities to include emergent transport in accordance with the Statement of Work.

3)Technical Subfactor Three: Technical Approach Quoters must submit their technical approach that illustrates their ability to pick-up and deliver specimens within designated pickup schedules, as well as the ability to maintain security requirements for all specimens during transport in accordance with the Statement of Work.

4)Technical Subfactor Four: Biological Hazard Training and Equipment Quoters must submit biological hazard training documentation illustrating that their personnel are trained to address biological hazard spills and have Personal Protective Equipment as well as spill kits in their vehicles as required by the Department of Transportation (DOT) for carriers that transport patients, clinical or diagnostic specimens (referred to as Biological Substances, Category B since 2007 DOT)”.

(Maximum two pages for this subfactor, Times New Roman, Font Size: 11, Page Size: 8.5 x 11) ______Part III PAST PERFORMANCE: Quoters shall submit no more than a total of Two Past Performance Questionnaires (PPQs) from contracts/purchase orders/task orders performed within the last three years. It is the Quoters responsibility to submit PPQs to the Evaluator from their previous contracts and to ensure these PPQs are emailed to Contract Specialist Andrew O’Neal at Andrew.O’Neal@va.gov by the quote due date listed in Block 8 of the solicitation. To be considered, the PPQs must be completed by the EVALUATOR from their previous contracts purchase orders/task orders and emailed directly by the Evaluator to the Contract Specialist for receipt no later than the time and date quotes are due. All Questionnaires received after the quote due date and time will not be considered. PPQs submitted directly by Quoter will not be considered. The Government may contact the Evaluators for any clarifications. Quoters shall submit information on recent and relevant projects of similar scope, magnitude of effort, and complexity. Performance ratings for contracts/purchase orders/task orders that are 75% complete or completed within the past three (3) years from the date offers are due, shall be considered recent.

A reproducible PPQ form can be found as an attachment in section D of this solicitation.

______Part IV PRICE: Quoter must submit their price quote. All prices shall be represented in two decimal positions only. Example: $0.27, not $0.27458.

Note: Quantities listed are estimated and does not commit the Government to that quantity. The Contractor shall invoice only for services/products requested by the Government and provided by the Contractor.

______Part V: Quoters must complete and submit with their quote VAAR Clause 852.219-77 VA Notice of Limitations on Subcontracting-Certificate of Compliance for Services and Construction (Sep 2021).
