B.2 PRICE/COST SCHEDULE

Contractor shall provide both normal business hours and after-hours coverage of intraoperative neurophysiologic monitoring (IONM) services including providing certified technicians, equipment, instrumentation and disposable accessories that are required for the accurate neurophysiologic monitoring of the beneficiaries of the Salt Lake City Veterans Affairs Medical Center (SLCVAMC). Neurophysiologic monitoring is the use of electrophysiological methods such as electroencephalography (EEG), electromyography (EMG), and evoked potentials to monitor the functional integrity of certain neural structures (e.g., nerves, spinal cord, and parts of the brain) during surgery. The purpose of intraoperative neurophysiologic monitoring is to reduce the risk to the patient of iatrogenic damage to the nervous system, and/or to provide functional guidance to the surgeon and anesthesiologist. Contractor personnel shall be available for emergent cases as needed.

Industry Questions & Answers

Question 1: Does the medical center require concurrent coverage?

Answer 1: Neurosurgery: No

Ortho Spine: Since there are 2 spine surgery teams, there is always the possibility of having concurrent cases that would require neuromonitoring. I am not aware of this happening and if it did, this would be a rare, especially given that ortho spine and neurosurgery have different OR days. Regardless, the capability for concurrent case coverage is needed.

Question 2: What historical case mix, average case duration and after-hours usage can the VA provide?

Answer 2: Neurosurgery: Primarily spine cases (all cervical cases and lumbar fusions), occasional cranial cases; Average 3 hours

Ortho Spine: Our use of neuromonitoring is primarily for cervical spine surgeries both anterior and posterior which range from 2-3 hours in length. We will use neuromonitoring for our lateral approach lumbar cases, but this is rare, maybe 1-2 times a year and these cases are usually 3-4 hours long.

Question 3: How many after-hour cases have there been over the last 36 months?

Answer 3: Neurosurgery: 10

Ortho Spine: After-hour cases are rare with our service if defining after hours by those occurring on weekends and/or cases that start between the hours of 5 pm and 7 am. If so, our after-hours cases are fewer than 10 a year.

Question 4: How many surgeons are using IONM services?

Answer 4: Neurosurgery: 2 Neurosurgeons (not concurrently)

Ortho Spine: We have 4 attendings and 3 fellows, but they rotate through. We only have scheduled OR time on Wednesdays with time for 1-3 cases, however 3 cases in one day is rare.

Question 5: How many canceled cases have there been over the last 36 months?

Answer 5: Neurosurgery: 18
