36C25926Q0811 – Chiropractic Tables – Questions & Answers
1. The available packet contains the solicitation, SOW, Buy American certificate, and wage determination, but the instructions also require testing plans, laboratory certifications, and “QASP Attachment F,” none of which appears in that packet. The pricing instructions require base and option-year pricing although the schedule contains only one line for two tables. Volume III identifies Past Performance but gives no submission format or treatment for an offeror with no relevant past performance. Please confirm that Volume I should address the SOW and salient characteristics, that pricing should cover only the one listed line with no option years, and identify the exact Past Performance information required and how no relevant past performance will be evaluated. If testing, laboratory, QASP, or option-year material is required, please provide the missing attachments or line items.
Answer – Volume I – Technical capability changed in Amendment 0002 to remove mention of “QASP Attachment F.” Volume II Price was revised to remove the base and option years reference. Volume III Past Performance was removed and replaced with the LOS requirement. Volume IV was added for the BAA Cert.
2. Please provide the corrected, tailored compliance certificate and identify the operative clause set for this SDVOSB supply acquisition. The instructions require VAAR 852.219-75 for services/construction, while VAAR 819.7011(c) prescribes 852.219-76 for supplies/products; the packet does not include a tailored 852.219-76. The FAR matrix also marks 52.219-6, does not select 52.219-33 or 52.225-1, yet includes the FAR 52.225-2 Buy American Certificate, and attaches Wage Determination 2015-5489 while 52.222-41 is not selected. Please confirm the required 852.219-76 manufacturer/nonmanufacturer certification and whether an SBA nonmanufacturer-rule waiver applies; whether 52.225-1/52.225-2 applies; and whether the wage determination and Service Contract Labor Standards apply to the installation work.
Answer – VAAR 852.219-76 VA Notice of Limitations on Subcontracting – Certificate of Compliance for Supplies and Products has been attached to Amendment 0002 and has been added as Volume III and Factor III as well. The requirements for 52.219-6 Alt I, 52.222-42, 52.222-62, and 52.225-1 Alt I have been removed. Requirements for 52.219-14, 52.219-33, 52.225-1, and 52.225-2 have been added. The wage determination is not a requirement for this solicitation.
3. Please confirm whether the 30-day-after-award requirement runs through final installed and accepted condition and whether the contractor must provide inside delivery to the final room, uncrating, assembly, placement, electrical hookup, functional or biomedical acceptance testing, user demonstration, and removal of packaging and debris.
Please also identify any dock or room location, delivery-hour, appointment, site-access, or installer-credential constraints needed to price and commit to that schedule.
Answer - The 30 days after award requirement runs through the final delivery, installation, debris removal, and acceptance. Inside delivery is required due to the weight of these tables, they will need to be uncrated and placed. Electrical hook up and any installation of parts is minimal and can be done by VA staff.
Delivery should take place between 7:30am-400PM and can be coordinated through VA staff for placement when delivery is at SLC VA in building 550. The table that goes to Ogden would best be delivered on a Wednesday. Salt Lake location is in building 550 on the first floor, room 1A09, the Ogden room number is 1A23. Please coordinate table deliveries with Dr. Mencl for both locations.
