About this file

This is Amendment 004 to a Request for Quote (RFQ) for a Patient Monitoring and Telemetry System at the Salt Lake City VA Health Care System (George E. Wahlen VA Medical Center). The amendment extends the quote submission deadline from July 31, 2026, to August 5, 2026, at 12:00 PM Mountain Standard Time. Quotes must be submitted via email only to Stephanie Cahill at stephanie.cahill@va.gov; hand-carried and faxed submissions are not acceptable. The Government reserves the right to make award decisions based solely on initial quotes received and specifies that complete RFQ packages must be signed, dated, and submitted in their entirety to be eligible for consideration. Offerors submitting multiple emails must label them sequentially in the subject line (e.g., "Email 1 of __") to ensure the Government can track complete quote packages, with all components required to arrive before the deadline to avoid deemed ineligibility.

The underlying solicitation (36C25926Q0356) is a combined synopsis/solicitation for commercial items prepared under FAR Part 12. The procurement involves patient monitoring equipment and related services, with the place of performance in Salt Lake City, Utah. The contracting office is located in Greenwood Village, Colorado (80111), with primary points of contact being Stephanie Cahill (Contract Specialist) and John Cheng (Contracting Officer). The solicitation included a mandatory site visit on July 7, 2026, and a live Q&A session on July 14, 2026. Prior amendments clarified technical requirements, including correcting low-acuity monitor quantities to 36 units and specifying that all quotes must include new monitoring equipment. The procurement is not set aside for any specific business classification, and no Recovery Act funds apply.
