About this file

This document contains an Infection Control Risk Assessment and Matrix of Precautions for a federal construction project to replace the sanitary sewer line at Building 150 of a Department of Veterans Affairs medical center. The project involves replacing approximately 1500 square feet of flooring containing black mastic, replacing plumbing fixtures and fittings in bathrooms and locker rooms, and installing new flooring, sinks, and fixtures. The construction activity is classified as Type C, which will take place in low risk patient areas. A Class II level of precautions is required, including controlling dust, isolating the work area, cleaning workspaces, and verifying HVAC systems are clean upon completion.

1. Reference Key Note #2 Dwg. A101 Please provide basis of design for 72”X24”X60” Shelves

0. Shelving units shall be 72”L x 24”W x 60”H; minimum 4-tier, wire rack, stainless steel, swivel casters, NSF certified, or approved equivalent.

1. Reference Keynote #3 Dwg. A101 Please provide basis of design for 36”X36”X6” Plastic Pallets

1. Pallets shall be HDPE plastic; stackable; USDA compliant; has ability to accept pallet jack forks; size 36”L x 36”W and a minimum of 6” tall.

1. Has a camera been run into existing lines to determine depth? Please provide that information if available.

2. The VA does not possess records of the sanitary sewer pipe depth. This line was determined by our maintenance shop to be between 2-feet and 6-feet below grade, as stated in the project documents.

1. Scope of work has increased significantly from original project solicitation. The period of performance is not sufficient for the amount of work shown in drawings. Please consider an extension of POP

3. Concur. The VA will adjust the current period of performance from 90 days to 120 days for construction. Due to the sensitive nature of this work and the level of effect it will have on multiple departments, this construction NTP cannot be extended beyond 120 days. Administrative actions may be performed prior to the NTP in order to submit on materials for approval and acquire all materials and equipment needed to perform the work, as stated in the contract documents.

1. Please specify whether after hour work will be required.

4. The area under construction will be completely closed to staff. This will allow the construction contractor access without the need to work around VA schedules. Due to this, after hour work will not be required and is not anticipated. However, unforeseen conditions could determine after hour work to be necessary. Coordination with COR prior to any changes to work schedules.

1. Key Note #1 on A101 states that shelving to be moved into this space from Room 206-F. Who is responsible for moving and setting up shelving?

5. The Contractor will be required to procure and set up the shelving called out in the contract documents. Once the shelving is set up, the VA staff can relocate backstock out of the basement in preparation for closure for construction.

1. D101 indicates to maintain poly barrier through the duration of project at double doors at B-002. Please provide egress in and out of Bio Med in order to complete work.

6. Bio-med will use the egress from B-015A into the basement of 154A for access. It is the expectation of the VA that the construction will be phased such that there will not be an open excavation in Corridor C-001 and Mech. Utility B-015A at the same time. If this cannot be achieved, coordination with the COR and VA Infection Preventionist will be required.

1. SOW indicates that contractor to provide temporary mobile cold storage unit for the purpose of providing the kitchen temporary freezer space. Mobile unit to be provided prior to the beginning of construction. Unit to be placed at the direction of the COR.

Please provide additional details on size and type of storage required.

Existing walk in freezer in the basement of Building 150 is approximately 75 sq.ft. and is always approximately 80% full. Storage shall be sufficient enough in size to store similar capacity. This is to ensure the kitchen serving patients has sufficient backstock.

Please indicate the location of storage so that power considerations can be evaluated.

It is the expectation of the VA that the temporary mobile storage unit be located in Parking Lot F (loading dock area). The unit will need to maintain distance for delivery trucks but still be able to be connected to power. Exact location to be coordinated with the COR prior to placement.

Please indicate if a diesel power refer trailer is required. If required please indicate if a ramp and land will need to be provided by the contractor.

Method of power shall be electrical for the cold storage as diesel powered storage would be inappropriate for the location (air handler intakes nearby; noise; etc.) and more costly to maintain over the duration of the project.

1. SOW b. 2 - Furnish all labor, equipment, and materials necessary to perform certified asbestos abatement of the black mastic under all existing flooring during removal. Please provide square footage of black mastic areas to be abated.

8. Approximately 1500 square feet of flooring that is anticipated to contain black mastic.

1. SOW b.5 -CRITICAL: Operability of the existing sanitary sewer line during construction and installation of new sanitary sewer line must be maintained until the new line is ready to be tied into the system. This is to ensure vital hospital functions continue over the duration of the construction. Please clarify if any further lines are connected to main line beyond clean out in front of maintenance supervisors’ office on basement level?

9. There are no lines that are known that currently run plan south past the maintenance supervisor’s office. However, there will be lines connecting, in various locations, along the plan north run of the line. Partial historical drawings of the existing plumbing infrastructure can be provided upon request. These lines will need to be determined to be “in-use” or “abandoned” and either removed from the new system or re-connected to the new system for future use.

1. P101 shows new laterals to be replaced through bathroom A-003, B015A, A-106 and B-001. These laterals go under or through existing walls and footings. Please provide structural drawings for these areas.

10. Please see newly provided document, 150 Structural, included with this RFI response.

1. P101 – Keynote 6 indicates to provide shop sink with countertop area. Please provide elevation drawings of desired countertop configuration.

11. Stainless free-standing sinks are manufactured to be adjustable in height. Exact configuration will be determined by each location’s features and available space.

1. Per Sheet Note 14. Please provide locations on P101 where sanitary sewer is tied into to upper floors.

12. Key Note 3 of Sheet P-101 calls out where the sanitary sewer line is tied into upper floors. There is potential for the line to connect elsewhere. Further site investigation may be required.

1. Please clarify if doweling is required for replacement of concrete slab? Provide detail or description.

13. Yes, doweling is required per the contract documents. Please see key note 5 on sheet P-101.

1. DW101 Indicates that a poly barrier is required throughout the duration of project at the double door of B-002 (Bio med shop) Access for personnel and equipment will be need to complete scope of work. Please clarify.

14. Please see response to question 7. Phasing plan shall be created with these factors taken into account.

1. Sheet note 11. On F01 indicates to install flooring in manner that allows positive drainage to floor drains. Are sub floor elevations being raised to complete this? Please clarify.

15. It is the expectation of the VA that the new flooring be installed properly to ensure sloping exists towards the drain(s) in each space being remodeled. Elevations may need to be altered via proper sub-floor adjustments prior to flooring installation.

1. Provide floor tile type for locker room.

16. Floor tile type called out in the Specifications, SECTION 09 06 00 SCHEDULE FOR FINISHES (Page 198). This tile is to be used for both the bathroom and locker room.

1. Are floor drains to be replaced in women’s locker room?

17. Yes, they are to be replaced. Please see sheet P-101 for clarification.

1. Key note 9 indicates to replace water closets in women’s locker room. There is no notes indicating demolition of walls and or tile. Can fixtures be replaced without wall demotion on bathroom side?

18. No, construction for the water closets in the woman’s locker room WILL require some level of demolition to the existing walls and tile to achieve proper replacement.

1. There is a 4” line that appears to be screwed into a tapped clean-out tee coming out of a vertical pipe chase in the mechanical room near the coolers (RM B-012). What is this draining, and does it need modified so it is a more permanent installation?

19. Further clarification may be needed for this via site investigation. It will need modification for permanent installation.

1. It appears that a portion of the piping in Corridor C-001 has already been replaced with PVC. Does this need to be exposed and replaced again?

20. This run of pipe was added in 1961. Please see drawings for scope of project. Removal and replacement of this section is included.

1. Is there any information on the invert elevations of the lateral branches coming off the existing sanitary main?

21. Please refer to drawings for all known information about the system. Partial historical drawings of the existing plumbing infrastructure can be provided upon request but may not be representative of current existing conditions.
