664-19-320
B.1 INTRODUCTION:
This requirement is for construction services to remove and replace the existing flooring system and repair and seal the walls of six (6) Operating Rooms (ORs) located at the 5th floor west wing of Building #1 on the existing VA San Diego Medical Center campus as described in this document and specifications prepared by VA Engineering Services. This project is referred to as OR Flooring and Wall Repairs in the San Diego California VA Health System (SDVAHCS). This requirement is classified under construction as it calls for the alteration of real property.
B.2 BACKGROUND / DESCRIPTION OF WORK:
The Veterans Affairs Healthcare System located in San Diego, California (664) requires construction services to resolve the deficiencies in Operating Rooms’ flooring and walls.
The contract will include the following services (see contract phasing requirements in paragraph B.5 F “Project Schedule”):
1. Part I (Work Plans) – Develop a work plan with phasing schedule. Work will include coordinating with the contracting officer representative (COR) to ensure the phasing plan minimizes the impact to daily operations. Due to the sensitivity of the area of work, Infection Control Risk Assessment Guidelines (ICRA - Attachment B) must be incorporated in the work plan to ensure that completion of work results in minimal impact to the Operating Rooms’ operations. Detailed construction barrier setup and identification of all infection control elements are required to be shown on the plans for all phases of construction.
2. Product specifications and samples to be provided with work plan for approval prior to installation.
3. The OR department will only be able to shut down one operating room at a time. The project will require a total of 6 phases to complete (one phase per OR/room). There will be some downtime between phases based on OR booking schedule.
4. Part II (Construction) – Execution of the work as described in paragraph B.5.
B.3 CONSTRUCTION DOCUMENT:
Contractor shall furnish all labor, equipment, certification, supervision, and materials necessary to perform all operations in connections with this order. Contractor shall perform work in strict accordance with the General Specifications, Statement of Work, schedule, and all other supporting documents for this contract to include, but not limited to the following documents:
