STATEMENT OF WORK
644-17-003 RENOVATE PCT/PMRS BACKFILL BACK-FILL
Statement of Work
RENOVATE PCT/PMRS CLINIC BACKFILL
644-17-003 5 October 2018 Carl T. Hayden Medical Center
650 E. Indian School Road
Phoenix, AZ 85012
POC: Gary Zettl, VA Engineer
NOTE: No security accreditation package is required for this service. Contractor will not have access to any VA Information Technology (IT) systems or VA sensitive information. Contractor also will not require the need of contractor-owned IT devices.
1. PURPOSE: This Statement of Work covers the furnishing of labor, materials, and supervision for a remodel construction of the new Post-Traumatic Stress Disorder Clinic Team (PCT). This location is a backfill of a suite vacated by the VA’s PMRS clinic some years ago. It is a 7,000-square foot vacated suite in the center of hospital’s 1st floor. This scope of work is a construction to go in behind a stalled-out VA self-perform construction effort in which the demo/abatement was completed, and some stud framing put backs have been applied. Our VA’s internal construction work crew were lost due to manning reductions three years ago. The VA did already have specialty companies install the medical gasses, some fire sprinklers, and most HVAC mechanicals to include VAV’s. There are government purchased construction materials to contribute to this project that our work crew had not yet used in a hope to reduce the project’s overall cost (see materials list in the attachments to the SOW and Electrical materials detailed plan set page EP-605). The performance period for this contract is 275 Calendar Days. The General Contractor will have a site superintendent present at all times there is active construction work in progress on this project on the VA Phoenix Campus.
2. BACKGROUND: The Remodel PCT project includes creating a new Emergency Power Electrical Closet, and tripling the size of the Normal Power Electrical Closet. The electrical work needed is thought to be the most complex portion of this project due to there being 3 adjacent clinics powered by circuits out of this PCT’s electrical closet in the center of the work area. The PCT electrical closet will have all the existing power panels removed and new breaker panels installed. The active circuits will need to be migrated over after clinical hours in a way that allows the VA’s active users in adjacent spaces to continue their patient appointments.
A. The bulk of construction materials delivery to the interior of the hospital are to be before 08:00 AM each week day morning. This work site is at the core center of the in-patient hospital building. Adjacent to the work site, on the East wall line, is a corridor with the highest patient traffic path in the hospital.
B. The VA provided construction materials are located at the automobile parking garage and building 14. These spaces are to be freed up for VA use as the project progresses. The builder’s own provided materials must be staged from a separate outdoor location.
C. CAUTION: The floor above this work space houses the Blood and Pathology Laboratories. There is a network of lateral glass sanitation pipes and a large glass to cast iron vertical drain in the plumbing chase. These glass drains contain highly toxic substances posing a high health risk to any tradesman who may inadvertently breaks one of these glass lines. See the drawings key notes on this high-risk existing condition.
D. Space for one conex will be provided on the VA Phoenix grounds. Specific location is to be coordinated with the project COTR.
3. SCOPE OF WORK:
A. A site visit and survey are recommended for all bidders prior to submitting a bid for the specified work in project #644-17-003.
B. Complete work as detailed in the attached Specifications and Drawings for project 644-17-003. Project Specifications include requirements for all products, materials, equipment, installation methods, testing and verification of systems shown on the construction Drawings. Contractor is recommended to review all contract documents (Specifications, Drawings, Narrative, etc.) carefully. Upon issuance of the NTP for Construction, it will be the expectation of VA that the Contractor knows the contract requirements in detail. If there are any discrepancies between the Specifications and Drawings, the Contractor shall bring that to the attention of the Contracting Officer immediately. Hierarchy of VA provided documentation is (1) Specifications, (2) Drawings, (3) Project Narrative.
C. Notice to Proceed for Submittals ONLY shall be issued to the Contractor no more than 90 days from the date of award.
D. Below is a summary of the work that will be required in this project: (For detailed instructions and guidance, contractor is to reference the project Specifications and Drawings)
1) Life Safety and General Notes
2) Architectural
3) Plumbing
4) Fire Protection
5) Mechanical (HVAC and Control System Connections & startup)
6) Electrical
7) Telecommunications
8) Special Systems E. Prior to final acceptance, Contractor shall thoroughly clean building, equipment, furniture and surrounding project site of grease, oil, plaster, dust, stains or other debris.
F. WARNING: Building 1 utilizes post tension flooring. Before core drilling the floor - perform imaging of the existing post tension cable locations. The builder must be insuring that all post tension cables within the concrete floors remain intact.
G. Phasing of Work: The scheduled timing of the utilities outages requires a 14 days’ notice to the VA’s Contracting Officer’s Representative who will be the liaison to the General Contractor throughout the course of construction.
H. The construction elements will follow strict Infection Controls and Interim Life Safety Measures.
a. The most stringent Infection Controls will be required in the interior of the existing occupied buildings. The demolition evolutions will conduct Class IV dust control measures then later in the build back phase the dust control will be lowered to Class III procedures. HEPA machines capable of creating a verifiable negative air pressure is required 24/7 during the rebuild of the existing interior spaces. This is required to ensure that adjacent manned offices and public corridors are risk free of the actions of the construction work.
b. The fire egress stairwell in the adjacent buildings will remain open with signed egress routes through the construction site as necessary to assure occupant evacuations in the event of an emergency.
I. A&E Provided Submittal Exchange: The A&E submittal exchange will be placed into effect after the construction award has been presented by Contracting. The pre-bid requests for information will be presented to the VA Contracting Officer in accordance with the VA’s solicitation instructions.
J. Construction Safety Plan: The General Contractor shall develop a plan for both the area of work and the interaction of the work within the facility per OSHA, AIA, and VA guidelines. The Infection Prevention Measures, as set forth in the VA Infection Control Risk Assessment, shall be included in the Safety Plan. The General Contractor shall submit a copy of their Daily Safety Inspection form. The Safety Plan shall be a submittal and approved by the VA prior to the start of construction. Some general safety precautions are listed below:
1) The General Contractor’s Safety program will follow VA Specification 01-35-26
2) Work shall be conducted in a manner to afford maximum protection to patients, employees, public guests, building facilities, and to prevent unreasonable delay or interference with normal hospital activities.
3) Provide fire extinguishers that are readily available always.
4) It will be the obligation of the contractor to maintain clear passageway in facility hallways. Material and tools shall be kept within the confines of entrance partitions and trash will be removed daily.
5) Barriers shall extend to full height, from floor to ceiling. Reference project specifications or coordinate with COTR on appropriate barrier materials.
6) Provide necessary ventilation to trap and extradite unwanted fumes, dust, and odors when conducting work. Methods of ventilation shall be detailed in Construction Safety Plan for review and approval.
7) Once the ceiling has been removed the fire sprinkler system will be compromised by an undetermined amount. Smoke alarms must be maintained, or new ones established which tie into the facility’s automated fire/smoke alarm system. A smoke sensor system in each construction zone will be acceptable as the required fire watch while the fire sprinklers are degraded. The Safety Plan will include fire watch procedures.
8) Once the ceiling is removed in each space, a temporary smoke barrier must be established above the room’s interstitial space to prevent the spread of smoke to adjacent rooms in the event of a fire.
9) Construction barriers from occupied spaces will be constructed of solid materials with a locking hinged door if in place more than 7 days. All door locks into the construction zones will use VA supplied construction cores (5 pin best lock cores). For shorter durations, barriers can be made of flame resistant plastic with zip opening doorways. Coordinate with COTR on appropriate barrier materials and describe these procedures in the Safety Plan.
K. General Submittal Procedures: Unless noted otherwise, the Contractor is required to submit all shop drawings and material submittals for the contract within 10 working days after issue of the Notice of Award or as directed by the Contracting Officer. Data shall be collected into a single submittal for each element.
L. Schedule of Values: General Contractor’s Schedule of Values will be broken down by phase of construction and CSI division of work and materials. The cost of the General Conditions per day must also be presented on builders the Schedule of Values. The Schedule of Values is to be presented as a submittal for the VA Contracting Officer’s approval at the Notice to Proceed meeting.
M. Building Schedule of Activities: General Contractor will present their building schedule in GANTT format which highlights the critical path elements. If the schedule internal elements, critical path, or end date are forecasted to change a new GANTT schedule will be presented to the Contracting Officer and the COR.
N. Building Start Up: The General Contractor is responsible for the startup of the remodeled suite to include HVAC system controls tie ins, testing, air flow balancing to meet VA HVAC guides required air flows, and presentation of controls systems monitoring and control prior to the building’s acceptance.
4. GENERAL CONTRACTOR COST OF GENERAL CONDITIONS:
Unit Price Break Downs: General Contractor will include their schedule of values their daily cost for general conditions. This value will be valid for the entire project and will also be used if the performance period later needs to be extended. General Conditions cost break down will be based on a 5 day per week work presence.
5. MEASURES OF PERFORMANCE:
A. The General Contractor (GC’s) is responsible for the Quality Control of their work. The General Contractor shall develop a plan for which all construction crews under this project shall follow. The General Contractor shall specifically identify appropriate standard forms for Quality Control inspections that will become part of the construction documents. This includes the punch list format that the Contractor will be using internally for their work inspections.
1) Quality Control Plan shall be a submittal and approved by the VA prior to the start of construction.
2) The VA will be in the roll of Quality Assurance. The VA final punch list inspection will not be accomplished until after the General Contractor submits their internal, signed off, completed punch lists.
