QUESTION 1
Can you clarify the age of the units and the last time they were serviced?
ANSWER:
This amendment revises “Reliance Vision SC” to AMSCO 5052 on the Performance Work Statement and Price/Cost Schedule, section B.2 of the Solicitation. SEE REVISIONS BELOW.
The serial numbers for the equipment are as follows:
Equipment Unit 1 Unit 2
Sterilizer 400 S/N 032792217 S/N 032832209
Reliance Vision SC Replaced by AMSCO 5052
AMSCO 70 Sinks S/N 3629719008 S/N 3628219006
UNITY ultrasonic washers S/N 5242282302 S/N 5242282304
AMSCO(5052) S/N 3617026012
The age of the equipment and most recent servicing date (if applicable) follows:
Equipment Age Last Serviced
Sterilizer 400 Both are 3 years (Purchased 1/2023) August 2026
AMSCO 70 Sinks Both are 6 years (Purchased 2/2020) July 2026
UNITY ultrasonic washers
Both are 3 months (Purchased 6/2026) N/A
AMSCO(5052) 3 months (Purchased 6/2026) N/A
QUESTION 2
There appears to be a discrepancy between the Equipment List and the Pricing Sheet:
2. Scope
The contractor shall provide all labor, parts, tools, equipment, transportation, and supervision necessary to perform scheduled preventive maintenance (PM), unscheduled repairs, diagnostics, and functional testing on AMSCO 70 series sinks, Quantity: 2, and associated equipment which will include:
UNITY ultrasonic washers (Innowave Sonic Irrigator 15 Gal) Quantity: 2
AMSCO disinfecting units/washer (5052) Quantity: 1
Reliance Vision multi-chamber washer disinfector (SC 200-208V 3ph 60Hz Steam HTD ) Quantity: 1
Sterilizers (400 series) Quantity:2
ANSWER: The quantities listed in the scope are correct. The Reliance Vision is being removed and will be reflected in the revised PWS attached to this amendment.
QUESTION 3
We would like to request an exact Equipment List including the serial number of each piece of equipment and the start date for each piece of equipment. This may include changes to the Price Sheet.
ANSWER: See response to QUESTION 1 for the serial numbers. The needed start dates are listed in B.4, 2. Scope, under COVERAGE SCHEDULE.
QUESTION 4
Unscheduled repairs and parts: PWS paragraph 7 states that the contract includes all unscheduled repair calls, parts and associated costs. Please confirm that offerors must include an unlimited quantity and value of unscheduled repairs and OEM parts in the monthly firm-fixed prices, with no ceiling, deductible, exclusions or separately reimbursable parts.
ANSWER: Please see revision to the PWS, section 7.
QUESTION 5
Historical repair information: Please provide the number of unscheduled service calls, labor hours and parts expenditures for each covered unit during the previous three years, if available.
ANSWER: One repair occurred in the last three years. No other unscheduled repairs have occurred on this equipment. Please see revised PWS, section 7.
QUESTION 6
Equipment information: Please provide the model number, serial number, installation date, current operational status, warranty status and available maintenance history for each covered unit.
ANSWER: See response to QUESTION 1 for the serial numbers and installation (purchase date). See response to QUESTION 5 for maintenance history. The start dates listed in B.4, 2. Scope, under COVERAGE SCHEDULE, represent the end of warranty:
Equipment Under Warranty until:
AMSCO 70 Series Sinks 10/4/26
Ultrasonic Washer 6/30/27
Washer/Disinfector
(AMSCO 5052)
6/30/27
Sterilizer 400 8/31/30
QUESTION 7
Existing deficiencies: Please identify any presently known equipment deficiencies, open repair requirements or components requiring replacement.
ANSWER: There are no presently known equipment deficiencies, open repair requirements or components requiring replacement.
QUESTION 8
Major component failures: Are failures caused by misuse, utility problems, water quality, accident, facility plumbing/electrical conditions, obsolete components or catastrophic equipment failure included in the contractor’s firm-fixed price?
ANSWER: Major component failures not falling under scheduled preventative maintenance would be an unscheduled repair. See revised section 7 of the PWS.
QUESTION 9
Parts availability: If an OEM part is unavailable or subject to a manufacturer lead time exceeding 72 hours, will the 72-hour repair-completion standard be extended when the contractor documents the delay?
ANSWER: Please see revised PWS, section 7.
QUESTION 10
Response-time standard: Does the 72-hour requirement mean that the technician must respond or arrive within 72 hours, or that every repair—including repairs requiring ordered OEM parts—must be fully completed within 72 hours?
ANSWER: See revised PWS, section 7.
QUESTION 11
Preventive-maintenance materials: Please confirm whether all OEM PM kits, seals, filters, chemicals, lubricants, calibration materials and other consumables must be included in the monthly firm-fixed prices.
ANSWER: Yes, firm-fixed price must include all costs to perform the services.
QUESTION 12
Past performance: Will the Government consider relevant past performance of a proposed first-tier similarly situated SDVOSB subcontractor or the relevant experience of proposed OEM-trained technicians?
ANSWER: Past performance will only be evaluated for the entity submitting a quote.
QUESTION 13
Wage determination: Wage Determination 2015-5253 identifies Bexar and surrounding San Antonio-area counties, while performance will occur in Big Spring, Howard County, Texas. Please confirm whether the attached determination is correct or provide the applicable determination for Howard County.
ANSWER: The attached Wage Determination is incorrect. This amendment will address the revision. The applicable Wage Determination is 2015-5269, rev 33 dated 8/16/26.
QUESTION 14
Monthly invoicing: The solicitation requires monthly invoicing, but certain base-period CLINs use a 96-day unit and option-year CLINs use monthly units. Please confirm how offerors should calculate and invoice CLINs 0002 and 0003 during their partial base-year coverage.
ANSWER: Invoices are expected to be submitted monthly for services rendered within a given month.
The quoted unit prices are to be submitted per the price/cost schedule. Yes, some are months and some units are days due to the staggering start dates.
QUESTION 15
Equipment quantities: Please confirm that the covered inventory consists of two AMSCO 70-series sinks, two Unity/Innowave ultrasonic washers, one AMSCO 5052 washer/disinfector, two AMSCO 400-series sterilizers and one Reliance Vision multi-chamber washer/disinfector.
ANSWER: See QUESTION 1.
QUESTION 16
Site inspection: Will the Government provide an opportunity for prospective offerors or their proposed technicians to inspect the equipment before quotations are due?
ANSWER: No.
QUESTION 17
Past Performance References: We have this same information in an Excel spreadsheet. Would it be OK to use that instead of this Word document?
ANSWER: Yes. Per E.1 52.252-1, under Volume 3-Past Performance, Vendor may utilize attachment D.2 to submit references.
REVISION 1
The Performance Work Statement is hereby replaced in whole by the attached PWS titled UPDATED PWS. Changes are shown via highlights.
