Each binder shall include (at a minimum) a maintenance log, OEM user manual, OEM maintenance manual, and any other relevant documentation. The Contractor shall also maintain a digital copy of each binder for redundancy and submit updates to the COR upon request. The Contractor shall propose a suitable location for each binder in close proximity to the source equipment, subject to coordination and approval by the COR. Upon approval, the Contractor shall furnish and install any necessary mounting materials to secure the binder in the designated location. Binders shall be updated immediately following each service visit and kept current throughout the contract period.
3.2.1.9. The Contractor shall conduct a quarterly PM out‑brief during one of the scheduled PM service visits. This out‑brief shall be incorporated into the recurring PM schedule required under Section 3.2.1.5 and coordinated in advance with the COR to ensure participation from VA in‑house maintenance personnel. The purpose of the out‑brief is to review PM activities completed during the quarter, discuss equipment condition and findings, and support coordination with VA staff.
3.2.2. Corrective Maintenance Services for Medical Gas and Vacuum Systems
3.2.2.1. The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary to repair or replace the medical gas and vacuum systems, as required.
3.2.2.2. The Contractor shall respond to service and troubleshooting requests in a timely manner to ensure continuity of operations and patient safety. For non-emergency issues, the Contractor shall acknowledge receipt of the service request within one (1) business day and confirm an estimated response time or site visit schedule within three (3) business days. For urgent or critical (emergency) issues that may impact patient care or facility operations, the Contractor shall respond within two (2) hours of notification and coordinate immediate corrective action with the COR. The Contractor shall maintain availability during normal business hours and provide emergency contact information for after-hours support.
3.2.2.3. The Contractor shall ensure that extended downtime of medical gas or vacuum equipment is avoided to maintain uninterrupted clinical operations. In cases where corrective maintenance activities require prolonged system outages, the Contractor shall coordinate with the COR to provide temporary equipment or other measures necessary to sustain operations for the duration of the downtime. All temporary systems or provisions shall comply with NFPA 99 and shall not be removed until permanent service is restored and approved by the COR.
3.2.2.4. All corrective maintenance services shall be initiated through written service requests issued by the COR or through written recommendations submitted by the Contractor for COR review. No work shall commence until the CO provides written authorization.
3.2.2.5. For the purposes of this contract, a work order is defined as a corrective maintenance task that can be fully completed at a total cost of $2,500 or less, including all labor, materials, equipment, and incidental expenses. The Contractor shall be capable of performing all work orders without additional cost to the Government beyond the agreed contract pricing. All work orders shall require authorization from the COR prior to commencement of work.
Work orders are intended for minor repairs that do not require formal proposals and extensive approval process.
3.2.2.6. Any corrective maintenance task that is estimated to exceed the $2,500 work order threshold shall be classified as a service order for the purposes of this contract. Service orders require a formal proposal from the Contractor and written approval from the CO prior to commencement of work. The Contractor shall provide a detailed scope of work for all requested and recommended service orders. Each proposal shall include an itemized cost breakdown (labor, materials, equipment, travel, and any other associated expenses), estimated timelines for completion, and any anticipated system downtime or interruptions. Proposals shall include all costs associated with assessing, inspecting, or evaluating the system(s), including but not limited to diagnostic labor, testing, site visits, material evaluations, and any other related expenses.
3.2.2.7. Service order proposals will be evaluated and approved by the CO. The Contractor shall only proceed with a service order after receiving an official Notice to Proceed, which must be issued in writing prior to commencement of work. Verbal authorization is permitted only for emergency situations where immediate action is required to protect patient safety or facility operations. Any verbal authorization must be documented in writing by the CO within one (1) business day.
3.2.2.8. Payment shall only be made for service orders that were previously quoted, approved in writing by the CO, and completed in full by the Contractor. Completion includes all required testing, inspections, and submission of applicable documentation that meets NFPA 99 and facility standards. Partial payments for incomplete work are not authorized.
3.2.2.9. All service orders performed under each option year shall not exceed a cumulative budget of $50,000.00. This budget represents the maximum amount available for approved service orders during that year. If the budget is fully utilized and the COR determines that additional service order funds are required, the COR shall request a contract modification from the CO to increase the budget for that respective option year.
3.2.2.10. The Contractor shall ensure that medical gas technicians who replace or breach the integrity of the medical gas and/or vacuum systems are ASSE 6010 certified, Professional Qualification Standard for Medical Gas Installers.
3.2.2.11. The Contractor shall ensure that corrective actions restore affected systems to a condition that meets or exceeds the requirements of NFPA 99 and the facility’s active accreditation agency. Compliance shall be verified prior to returning the system to service.
3.2.2.12. The Contractor shall coordinate verification by a third-party ASSE 6030-certified Medical Gas Systems Verifier, following any breach of the medical gas or vacuum systems, in accordance with NFPA 99 requirements.
Verification shall be completed and documented prior to returning the system to service.
3.2.2.13. The Contractor shall provide a minimum one (1) year warranty on all parts and labor associated with repair or replacement services. The warranty shall cover defects in materials and workmanship and shall begin upon completion and acceptance of the work by the Government. Any warranty-related service, including re-inspection, re-verification, or re-testing of affected systems, shall be performed at no additional cost to the Government.

C.2 SUPPLEMENTAL INSURANCE REQUIREMENTS
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: $500,000.00 per occurrences.
(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.
(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.
C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 calendar days.
(End of Clause)
C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR
2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 15 calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 calendar days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.
