About this file

This is a Statement of Work (SOW) for radiology personnel dosimetry badges for the Amarillo VA Health Care System and Lubbock VA Clinic. The contract is firm-fixed price with a 12-month period of performance, requiring yearly delivery of dosimetry badges to five service areas at Amarillo (Radiology, Nuclear Medicine, Surgery, Dental, and Biomed) and one location at Lubbock.

The vendor must provide dosimetry badges that achieve NIST NVLAP accreditation and calculate Deep Dose Equivalent (DDE), Eye Dose Equivalent (LDE), Shallow Dose Equivalent (SDE), Committed Effective Dose Equivalent (CEDE), Committed Dose Equivalent (CDE), Total Organ Dose Equivalent (TODE), and Total Effective Dose Equivalent (TEDE). Currently, the VA requires 85 collar dosimeters and 3 finger dosimeters, with potential for 45 to 90 active participants. Key requirements include: web-based database access allowing personnel management, badge tracking, and report generation; automatic dose computation for missing dosimeters; waterproof color-coded ID labels; multiple finger dosimeter sizes; master controls for all dosimeter types; exposure reports within 14 days; separate fetal dosimetry monitoring ensuring pregnant employee exposure does not exceed 500 mrem; immediate ALARA notifications for high exposures; termination reports within 45 days; monthly/quarterly ALARA summaries with annual Form 5 reviews; individual packing lists with participant details; and 10-day advance delivery before wear periods. All postage, shipping, sorting, packing, duplicate reporting, set-up fees, and ALARA reporting are included in a flat monthly rate. Deliveries for Amarillo go to Johnathan Riley or Christopher Hernandez; Lubbock deliveries go to Gwen Carrizales. The contractor must comply with federal records management, privacy, and information security requirements and cannot access VA systems, patient health information, or remove any memory storage devices from VA custody.

The mission of the Amarillo VA Medical Center Radiology Service is to ensure that the dosimetry processor is approved in this accreditation process for the type of radiations included in the NVLAP program that most closely approximates the type of radiation for which the individual using the dosimeter is monitored. This acquisition will assure correct purchases and enable all Radiology personnel to be accredited in accordance with the NIST.

1.2 Objective

The objective is for the dosimeter processor must be able to calculate, provide and store the following information: Deep Dose Equivalent (DDE) to the whole body, Eye Dose Equivalent (LDE), Shallow Dose Equivalent (SDE), Committed Effective Dose Equivalent (CEDE), Committed Dose Equivalent (CDE), Total Organ Dose Equivalent (TODE), Total Effective Dose Equivalent (TEDE) as well as reports of radiation exposure being made on Nuclear Regulatory Commission (NRC) Form 4 (Cumulative Occupational Dose History) and NRC Form 5 (Occupational Dose Record for a Monitoring Period) or equivalent.

1.3 Scope of Work

The Amarillo VA Medical Center and the Lubbock Outpatient Clinic requires dosimetry badges to be delivered on a yearly basis. These will provide needed information and protection of radiation levels personnel are exposed to.

1.4 Background
