About this file

This is an Asbestos Survey Report prepared by McLellan Integrated Professional Services, LLC for the Department of Veterans Affairs Network Contracting Office 17, focusing on Building 29 (CLC Gym) at the Bonham VAMC in Bonham, Texas. The survey was conducted on May 30, 2025, by Sean McLellan, a licensed and EPA-accredited asbestos inspector, with the objective of identifying the presence and location of readily accessible asbestos-containing materials (ACM) scheduled for renovation.

Nine samples were collected from three homogeneous material types: 6" vinyl cove base with mastic, wallpaper mastic with sheetrock and joint compound, and roll-out vinyl floor covering with mastic. The samples were analyzed by Cates Laboratories using Polarized Light Microscopy, and the results showed no asbestos detected in any of the materials. The report emphasizes that while the survey covers materials scheduled for renovation, there is a potential for additional unidentified materials in hidden or concealed areas. The documentation includes detailed analysis sheets, chain of custody forms, and licenses for the inspector and laboratory.

ASBESTOS SURVEY REPORT

1201 E. 9th Street Bonham, Texas 75418

1.0 INTRODUCTION

McLellan Integrated Professional Services, LLC (MIPS) was retained by Samantha Hites on behalf of the Department of Veterans Affairs Network Contracting Office 17 (Client) to conduct an asbestos survey of selected materials within the Bonham VAMC – Building 29 – CLC Gym at 1201 E. 9th Street, Bonham, Texas 75418. Only materials scheduled for renovation were included in the Building 29, CLC Gym survey. The survey was performed on Friday, May 30, 2025, by Sean McLellan, a State of Texas licensed and EPA-accredited asbestos inspector (Lic. No. 603824; Certificate No. 242399444).

2.0 OBJECTIVE

The objective of the asbestos survey was to identify the presence and location of readily accessible ACM used in the construction of the areas previously referenced. MIPS sampling did not include materials such as concrete flooring and/or hidden inaccessible components. Only readily accessible materials were included in the survey. Hidden materials or materials beyond reasonable access to the inspectors during the site visit were not evaluated as part of the survey. MIPS did not perform destructive sampling as part of this survey.

3.0 FIELD SERVICES

At the direction of the Client, the asbestos survey was performed within readily accessible interior areas previously described.

3.1 VISUAL SURVEY

The survey activities began with MIPS personnel's preliminary visual observation of readily accessible portions of the survey area to identify homogeneous areas of suspect ACM. A homogeneous area consists of building materials that appear similar in terms of color, texture, and date of application. An interior assessment was conducted throughout the area(s) surveyed. Visual observations identified homogeneous materials.

4.0 SAMPLING METHODOLOGY

Based on the visual observation results, bulk samples of suspect ACM were collected in general accordance with the Asbestos Hazard Emergency Response Act (AHERA) sampling protocols.

Each homogeneous area of suspect ACM was physically touched to assess its friability and condition.

The EPA defines a friable material as one that can be crumbled, pulverized, or reduced to powder by hand pressure when dry.

The suspect ACM samples collected were obtained by physically removing a small portion of the suspect material using a sharp instrument. The disturbance of adjacent material was kept to a minimum during the sampling program. Appropriately attired inspectors collected bulk samples using wet methods as applicable to reduce the potential for fiber release. Each sample was placed into a separate labeled container and then sealed. The sampling instrument was cleaned after each sample was collected.

4500 S. Lancaster Road – Survey Report MIPS Project No: 01M1010074 Page 2

Each sample was placed into a sealable container and labeled with a sample number with an indelible marker.

The samples were forwarded to Cates Laboratories, Inc., in Dallas, Texas, an accredited NVLAP and Texas Department of State Health Services (TDSHS) licensed laboratory, for analysis by Polarized Light Microscopy (PLM) with dispersion staining techniques utilizing visual area estimation in general accordance with the EPA method presented in 40CFR, Ch. 1, Pt. 763, Subpt. F, App. A.

5.0 RESULTS

Nine (9) samples of suspect asbestos-containing material (ACM) were collected. The attached homogeneous materials table presents homogeneous areas of suspect ACM identified and sampled during the survey.

This survey applies only to the materials sampled in the above-referenced locations. Until testing is performed, any materials located in the building that have not been evaluated as part of this survey should be assumed to contain asbestos.

6.0 CONCLUSIONS AND RECOMMENDATIONS

Based on the analytical results from the samples collected, no asbestos was identified in the materials scheduled to be affected by the proposed renovation associated with the moisture intrusion.

7.0 STANDARD OF CARE AND LIMITATION

MIPS performed its services consistent with the level of care and expertise exercised by asbestos professionals performing the same or similar services at the same time and in the same geographic area. No express or implied warranties apply to these services or this report. Due to the limited extent of the assessment and sampling activities at the Site, MIPS cannot and does not imply, warranty, or guarantee that materials not sampled contain no asbestos. This Asbestos Survey was intended to identify reasonably accessible materials most likely to contain asbestos in quantities subject to regulation. Please note that due to the non-destructive nature of the survey, the potential exists for additional materials to be present in hidden or concealed areas (i.e., above ceilings, in voids, chases, behind wall coverings, etc.).

7.1 REPORT USE AND LIMITATION OF LIABILITY

All conclusions and recommendations in this report represent the professional opinions of the MIPS personnel involved with the project. The results, findings, conclusions, and recommendations expressed in this report are based on access provided, conditions observed, and samples taken during the MIPS survey of the building. The information in this report is relevant as of the date the fieldwork was performed and should not be relied upon to represent site conditions later. This study and report were prepared on behalf of and for the Client's exclusive use solely for their use and reliance in determining the presence of asbestos in identified areas of the site. The results of this report are not intended to be construed as a legal interpretation of existing federal, state, or local environmental, health, and safety laws or regulations. MIPS assumes no responsibility or liability for errors in information or data provided to MIPS by the Client or any third party or developments resulting from activities or situations outside this project's scope.

Contractors or consultants reviewing this report must draw their own conclusions regarding further investigation or remediation deemed necessary. MIPS does not warrant the work of regulatory agencies, laboratories, or other third parties supplying information that may have been used to prepare this report. No warranty, express or implied, is made.

These samples were analyzed by layers. The overall percent asbestos for the sample is reported when relevant. The EPA considers a material to be asbestos containing only if it contains greater than one percent asbestos by Calibrated Visual Area Estimation (CVAE). EPA regulations also indicate that Regulated Asbestos Containing

Materials (RACM) – materials that are friable or may become friable – be further analyzed by point counting when the results indicate less than ten percent asbestos by CVAE. CatesLab utilizes CVAE on a routine basis and does not include point counting unless specifically requested by the client. The results may not be reproduced except in full.
