B.4 STATEMENT OF WORK

STATEMENT OF WORK (SOW)

1. Title of Work: ALM Grease Trap Repair

2. General: The grease trap cleaning and scope of work for the remodeling operations to repair the grease trap which is not in compliance with the City of San Antonio ordinance for grease traps maintenance and annual inspections.

3. Scope of Work: The project is a multi-stage, multi day project. It will require the coordination of multiple offices at the Audie L. Murphy location since the grease trap is located under the compactor for the trash. That compactor will not be able to be in its current position during the renovation which is projected to encompass 3 working days.

Stage 1 site prep The coordination of the removal of the compactor will need to be completed first. Once the compactor is removed the area can be cordoned off and made safe and ready for the next portion of work to begin on day 1.

Tasks to be completed on Day 1:

· Initially all the area must be cleaned washed down and cleared of debris and sectioned off for work, which will be running 24 hours a day during the remodel.

· All grease will be removed form the tank along with all the water. This needs to be accomplished when the kitchen is either closed or at its lowest production time.

· The inlet lines to the interceptor will need to be diverted and routed to a temporary holding tank which a vacuum truck will remove as needed throughout the entire project.

· The grease trap will then be thoroughly cleaned of all loose grease and other items in the tank to prepare for complete cleaning to be accomplished at the start of day 2.

Stage 2 Tank cleaning and patching

Tasks to be completed on Day 2:

· The tank will be entered adhering to all confined space requirement and will be always monitored. A copy of the permit will be made available to the VA for review and recordkeeping.

· The tank will be cleaned using degreasers (SDS sheets provided prior to work commencement)

· The tank will be washed using a pressure washer on all surfaces to ensure that all contaminated on the walls, floors, and ceilings is removed. The inlet and outlet piping will be inspected and replaced or repaired as needed.

· All grease and water will be removed from the tank as it is generated being placed into a vacuum truck for disposal

· Concurrently during this operation, the holding rerouting tank will be monitored to ensure it is kept at a safe level and emptied as needed.

· All surfaces of the interceptor will then after cleaning and inspection have patching placed into the areas required to bring the tank back to original specifications.

· After patching is dry and inspected the entire tank will have epoxy applied to all surfaces. (SDS for epoxy will be provided)

· The epoxy will need to cure for 8-10 hours in which time there can be no tank entry or exposure to contaminates.

· This needs to be accomplished when the kitchen is either closed or at its lowest production time.

Stage 3 Inspection

Tasks to be completed on Day 3:

· The epoxy coating will be inspected to ensure adherence to the surfaces. If any areas do not pass inspection the areas will be re epoxied to ensure adherence.

· Once the surfaces pass inspection the area will be cleaned again, and all application items will be removed and preparations for the return to service will begin.

Stage 4 Return to service

Tasks to be completed on Day 3:

· The liquid diversion equipment will be removed, and the tank will be allowed to be filled during which it will be monitored by the technicians on site to ensure no issues arise with flow or other areas.

· The temporary tank will be emptied for the last time and the tank will be removed

· The area will be cleaned up and all debris will be removed.

· The area will be pressure washed clean and be free of any grease or other contaminates.

· The space barricades can then be removed, and the compactor can be put back into place and the entire operation is back to normal service.

· All tanks and trucks will be washed out at disposal site after all the work has been completed

The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. (C) EFT banking information is not required if the Government waived the requirement to pay by EFT.

(2) Invoices will be handled in accordance with the Prompt Payment Act (31 U.S.C. 3903) and Office of Management and Budget (OMB) prompt payment regulations at 5 CFR part 1315.

(h) Patent indemnity. The Contractor shall indemnify the Government and its officers, employees and agents against liability, including costs, for actual or alleged direct or contributory infringement of, or inducement to infringe, any United States or foreign patent, trademark or copyright, arising out of the performance of this contract, provided the Contractor is reasonably notified of such claims and proceedings.

(C) Affected line item or subline item, if applicable; and

(D) Contractor point of contact.

(ii) Provide a copy of the remittance and supporting documentation to the Contracting Officer.

(iv) If a demand for payment was previously issued for the debt, the demand for payment included in the final decision shall identify the same due date as the original demand for payment.

(v) Amounts shall be due at the earliest of the following dates:

(A) The date fixed under this contract.

(B) The date of the first written demand for payment, including any demand for payment resulting from a default termination.

(vi) The interest charge shall be computed for the actual number of calendar days involved beginning on the due date and ending on—
