Questions w/Responses 16-31 36C25525R0090 Amendment A0003
589A5-25-128 Site Prep for Fluoroscopy, Topeka
16. The drawings note things to be installed by the “Contractor”, installed by “Customer” and
“CSH”. Are we to provide and install only items noted as “Contractor” or items also noted as
“Customer”?
Response: Contractor is to provide/install all items that are not identified to be provided and/or installed by Carestream (CSH). See note updates below.
17. Can we assume all items noted as provide and install by CSH is not our responsibility.
Response: Yes (see note updates that follow), contractor in some cases must hook up
CSH items to systems provided by the contractor. Contractor is to coordinate the system removal/installation/start up with the equipment provider.
18. Sheet E1 Electrical Legend R1, R2 and WD. The existing Risers (R1 and R2) and wall ducts are to be relocated, however it states to “provide dividers per local code”. Can you provide the details of what will be installed in the wall ducts and risers so the number of can be determined or provide the number of dividers to be provided?
Response: Items installed in the wall ducts are identified in the plans.
19. Sheet E1 Electrical Legend TR. Who is responsible for providing the transformer? Can you provide the information on the transformer?
Response: Equipment supplier has provided an update that the TR is to be provided by
CSH and wired into the PDU by the contractor.
20. Sheet E1 Electrical Legend PD1 and PD2. Please confirm PD1 and PD2 requirements.
PD1 is CSH provided and Contractor installed, PD2 is CSH provided and installed?
Response: Correct PD1 is CSH provided and installed by the contractor. PD2 is supplied and installed by CSH.
21. Sheet E1 Electrical Legend EPO. The EPO is listed as an Optional EPO switch. Are we to include this option?
Response: Yes, provide a switch that meets current codes.
22. Sheet E1 Network and Outlet Legend – NW1, NW2 and NW3. Can you provide the location these are to connect too? Please provide a floor plan showing connection point to the network.
Response: CAT6 cable length shall be long enough (and have coil length) to be run through hallway corridors to Room A223A. VA expecs this project to be complete before the EHRM TR room is complete in this area so the current patch panel is in room B216. If the EHRM TR room is complete in this area the patch panel will be in room A223A.
23. Sheet E1 Network and Outlet Legend – NW1, NW2 and NW3. Are there requirements to connect this to an existing patch panel? And are there any testing requirements for these cables?
Questions w/Responses 16-31 36C25525R0090 Amendment A0003
589A5-25-128 Site Prep for Fluoroscopy, Topeka
Response: See not above. For testing do our normal IT connectivity tests and provide report.
24. Referencing S1, structural note 1. Is the VA responsible for verifying the structure can support equipment loads?
Response: Yes
25. Sheet S3 detail Section C. There is a requirement for ¾” plywood fire blocking at transformer and PDU. Can you confirm it is understood this plywood will be exposed? Please confirm finish for this plywood.
Response: remove any reference of the use of wood on exposed surfaces and replace with backing material of stainless-steel sheeting of a thickness that can support the loads installed to it.
26. Sheet S3 detail Details B and C. These details require 1X10 wood blocking to be surface mounted or flush mounted if new construction. Can you confirm whether this should be flush or surface mounted?
Response: This area is not new construction.
27. The scope or work states to provide a welded seam sheet floor with integral base. What is the base height requirement? Is there a requirement for stop bar at the top of the base?
Response: 6”
28. Please advise what welded sheet flooring needs to be installed.
Response: See Specs
29. Drawing S3 Section-C calls to uses ¾ fire treated blocking surface mounted to the wall. This goes against the VA standard of not having exposed wood in patient areas. Please advise.
Response: See response above.
30. Drawing S1 Note 1 call for all equipment loads to be verified. Is it the contractor responsibility to retain the structural engineer to confirm this?
Response: See response to question 24.
31. Is shielding integrity testing required before turnover of the rooms
Response: The VA will do the integrity testing and the contractor is responsible for correcting integrity issues they have created as part of the project.
