The purpose of this Amendment is as follows:
1. To add Questions and Answers (see Continuation page).
2. To extend the Quote due date from 05 August 2022, to 10 August 2022 by 11:00 a.m. CDT.
3. Questions and Answers session is now closed as of 05 August 2022.
Questions and Answers
1. Question:
Please provide clarification. Amendment 4, page 9, 4(d)(a)(i) states "1. 0730 -
1500 = 3 staff" and "2. 1500 - 0730 = 2 staff". Is the requirement for a total of 3 staff for both Outpatient Escort and Screening or 3 staff members for Outpatient
Escort and 3 staff for Screening? Additionally, Is the requirement for a total of 2 staff for both Outpatient Escort and Screening or 2 staff members for Outpatient
Escort and 2 staff for Screening?
Answer:
IT would be a total of 3 staff 0730-1500 and a total 2 staff 1500-0730. (e.g. 3 staff from 0730-1500 would provide both screening and outpatient escort). The
KCVA is not so much “screening” but really just ensuring compliance with masking and sanitizing hands which is why we feel it appropriate to combine the two services so as long as one person remains at the door to ensure masking and sanitizing the other(s) can provide outpatient escort.
2. Question:
Please provide clarification. Amendment 4, page 9, 4(d)(b), states "Inpatient and ED Escort". Are these departments considered one in the same or are the individuals? Additionally, are the staff numbers required combined, and will cover both departments, or are these staff numbers for each department.
Inpatient and ED are both under the same service line here at the KCVA so the staff required would be the total number providing escort for both the inpatient units and the ED. (e.g., Monday-Friday excluding holidays 0900-1900 4 staff would provide escort for both inpatient units and ED)
3. Question:
In the new price table in Amendment 0004, the new manhours in green have strike-through markings just like the red hours do. We assume this is a typo and we should ignore the strikethrough, but other bidders might be confused.
Yes the Green strike-thru was by mistake and has been corrected in the following pages.
4. Question:
Amendment 0004 contains only the change pages for the SF 1449 document. To be compliant, should bidders fill in both the original SF 1449 in its entirety minus the old pricing table plus all of the fill-ins in Amendment 0004, or should bidders only fill in the pricing table in Amendment 0004 and then ignore the rest of the original SF 1449 pages?
Use the Section B.2 PRICE/COST SCHEDULE in this Amendment 0005 as the fill-in for your quote amounts.
The Statement of Work “SOW” has changes made in Amendment 0004 but there are no fill-ins in any of the SOW pieces. You will be sending in all of the Amendments, signed/dated, which shall contain all pages for each Amendment so the most updated SOW will be present with your quote.
You will continue to use the original SF1449 and fill out all Offeror required “Fill-ins”.
There is no need to fill in Section B.2 on the original SF1449 document as you will use Section B.2 from this Amendment 0005.
5. Question:
The contract calls for a supervisor to be on site during all hours of operation. The total number of work hours in a week is 24 x 7 = 168 hours. There is no overtime on the contract, so workers can only work 8 hours per day. When you divide the 168 hours by 8, you get a total of 21 supervisors needed. This means the total number of supervisors called for in the RFQ Amendment 0004--five (5) of them, as it still states in the table in Amendment 0004--is not enough to meet the requirement that a supervisor always be on site during operations. Should bidders just ignore this fact for now and simply quote the number of supervisor manhours shown in the new price table?
Answer:
This would be 168 hours divided by 40 hours = 4.2 FTEs for supervisors needed, not 21 as supervisors would be working 40 hours/wk, not 8 hours/wk.
