CONTRACTOR QUESTIONS
VA RESPONSES TO QUESTIONS
1.
4/22/2025
4/24/2025
Upon reviewing the RFI responses, Azimuth respectfully requests additional clarification regarding the response to Question #8.

Specifically, has the CLIN 4 scope of work been revised to eliminate the installation of the foundation wall and footing along the perimeter of the porch and adjacent portions of the building (originally estimated at approximately 80 linear feet)? If this portion has been removed from the scope, we recommend the VA reconsider its elimination. The proposed aggregate installation will exert lateral shear pressure on the existing masonry wall. Without a properly designed retaining system, this pressure poses a significant risk of structural failure, potentially leading to blowout of the masonry wall.

d.CLIN 4 – Fill existing crawl space.
i.Contractor shall furnish and install compacted CA7 aggregate in existing walls and footings so new Portland Cement Concrete (PCC) porch can be constructed as a slab on grade. Roughly 120 tons of CA7 gravel.

8. CLIN 4 -Install support wall.

i. The existing 10-inch footings and wall behind the masonry along the perimeter will not be demolished and remain in place. A support wall will be added to the existing footing at the south end of building, approximately 9-foot X 29-foot, 6-inch thick to support the newly poured concrete slab.

STATEMENT OF WORK

578-M25114 Building 12 Exterior Stairs

1. Location of Service The Contractor shall provide all labor, material, tools, travel, management, and equipment necessary for the repair of items described in this document at the Edward Hines Jr. VA Hospital (HINES VAH), 5000 South 5th Avenue, Hines, Illinois 60141.

2. Period of Performance The period of performance shall not exceed one hundred (100) days after contract award. This period reflects the entire contract period to include wait time of materials, administrative time, and contract close out. Actual onsite construction time is not expected to exceed fifty (50) days from initial mobilization. A detailed schedule must be provided to the COR within 7 days after receiving the contract notice to proceed and prior to any onsite work.

3. Background/History/Current Conditions Hines VA is an active medical campus. The contractor must not interfere with patient care or VA staff unless given permission in writing from the COR/CO. Currently, building 12 is a patient care building. The exterior stairs on the south side of the building are failing along with the concrete slab supporting the columns supporting the exterior canopy. There is rebar sticking out and pieces of concrete falling off creating trip hazards on this fire exit route. The failing concrete slab is exposing the columns holding the roof of the exterior patio to the elements. With the weather changing and snow season setting in, it is possible the structural columns may fail causing the roof to collapse with the added weight.

4. Scope of Work Reconstruction of the existing porch and stairs located at the south entrance of Building 12. Complete the repairs and restore the function and safety of the exterior stairs on the main entrance of the east side of building 12, explicitly detailed in line 6 of this document.
