2.H.3 of the Statement of Work states the following:

“The contractor shall repair any damage caused by the execution of this contract at no cost to the Government. Any damage shall be reported to the COR immediately for appropriate action. Any damage caused by the contractor will be repaired and/or replaced to the satisfaction of the VA.”

Shall be changed to:

Damage caused by Contractor negligence during execution of this contract shall be reported to the COR immediately for appropriate action. Damage determined to be due to Contractor negligence only and not age or equipment failure shall be paid for by the Contractor.

Deficiencies that are deemed critical or potentially catastrophic shall be reported immediately to the COR and/or Chief of Facilities Management.

Deficiencies that do not require immediate action will be listed on the Contractors report. All deficiencies observed, will be identified; to include the equipment ID and the location of each deficiency and a clear photo. The Contractor, will propose, the recommended corrective action that is needed, for each deficiency on the report.

Section 9.0.B of the Statement of Work states the following:

“Contractor shall be licensed to operate in the State of Wisconsin and have personnel that are certified by the National Electrical Testing Association (NETA) as NETA Certified Technicians (or an equivalent certification) and have completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training. There shall be at least one OSHA certified person present at the job sites at all times.”

Shall be changed to read:

Each Contractor testing team shall include at least one licensed electrician that is certified by the National Electrical Testing Association (NETA) as NETA Certified Technicians (or an equivalent certification) and has completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training. There shall be at least one OSHA certified person present at the job sites at all times.

Instructions to Offerors Technical Evaluation Factors shall be changed to the following:

Te(2) Technical Capability: (Will be used for evaluation under the Technical Capability factor in 52.212-2)

a. Offerors shall submit copies of licenses of Offerors permanent staff electrical technicians who will perform service under this contract and proof that these technicians have required 3 years of experience per Section 9.A.

b. Offerors shall submit copy of license authorizing Offeror to provide this service in the State of Wisconsin.

c. Offeror shall submit copies of certification that prove technicians performing work under this contract are certified by the International Electrical Testing Association (NETA) as NETA Certified Technicians (or an equivalent certification) and have completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training per Section 9.B.

d. Offeror shall submit explanation of how requirement to have at least one OSHA certified present at the job site at all times will be met per Section 9.B.

e. Offeror shall submit proof/certification that demonstrates technicians performing work under this contract have safety training – either on-the-job or class-room type - in electrical safety outlined in the OSHA Standard 29 Code of Federal Regulations (CFR) 1910 Subpart S – Electrical, and the NFPA 70E – Standard for Electrical Safety in the Workplace per Section 9.C.

f. Offeror shall submit proof/certification that demonstrates each of the Offeror’s contractor’s on-site crew leaders holds a current certification, Level III or higher, in electrical testing per Section 9.C. This certification shall be in accordance with ANSI/NETA ETT-2000, Standard for Certification of Electrical Testing Personnel.

g. Offeror shall proof/certification that demonstrates field service personnel of the manufacturer of the equipment being evaluated are certified by the manufacturer to perform these electrical tests and inspections.

h. Offerors shall submit a sample of the final report format that the Offeror proposes to use.
