About this file

This is a Standard Form 30 Amendment/Modification to a solicitation for Non-Emergency Medical Transportation (NEMT) services issued by the Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO 10), for the Aleda E. Lutz VA Medical Center in Saginaw, Michigan. The amendment, effective July 30, 2026, extends the proposal due date to September 10, 2026, at 1:00 PM EDT and re-issues the RFQ (36C25026Q2655) to clarify pricing structures, performance requirements, and contractual terms based on responses to vendor questions. The solicitation is set aside for Veteran Owned Small Businesses (VOSB) and is issued under FAR 12 Simplified Acquisition Procedures with NAICS code 485991 (Special Needs Transportation) and PSC V226.

The amendment addresses critical clarifications regarding pricing methodology, minimum personnel requirements, invoicing procedures, and service area coverage. Key revisions include: mileage will be calculated only during patient transport with no unaccompanied miles invoiced; medical personnel are not required for routine transports; the 30-minute dispatch-time requirement applies to non-Saginaw VAMC origins while 30-minute response time applies to Saginaw VAMC originating requests; wait-time grace periods and rounding rules apply separately at pickup and drop-off locations; VetRide is the controlling invoicing system; past performance evaluation will focus on PSC V226 or equivalent ambulette transport services; and wage determinations will be updated to cover all applicable counties in Michigan, Ohio, and Indiana service areas. The incumbent contractor information is disclosed, with the 2,500-trip minimum guarantee applicable only to the base period. Service utilization data from September 2025 through June 2026 indicates approximately 2,477 total one-way transports (1,797 non-ambulatory, 680 ambulatory) across 43,477 miles with 181 cancellations.

Question and Responses

1. Please clarify the pricing calculation for a one-way transport exceeding 40 miles. Does the base-rate CLIN compensate the first 40 actual point-to-point miles and the mileage CLIN compensate only miles above 40, or does the mileage CLIN apply to all actual miles once the 40-mile threshold is exceeded? For trips that do not originate or terminate at Saginaw VAMC, which designated VA Medical Center or reference point controls the 40-mile threshold, and what origin and destination points will the Government use in the Bing Maps API calculation?

Response: The VA has decided to adjust the CLINs not include mileage with the base rates and have mileage calculated only while a patient is being transported. Never will an unaccompanied mile be invoiced.

1. Please provide the most recent available 12-month utilization data, by transport mode, for one-way trips, mileage, wait time, cancellations or no-shows, time of day, day of week, origin and destination category, and peak simultaneous demand. If this information is unavailable, should offerors treat the estimated CLIN quantities, the stated typical volume of 10-25 weekday transports, and the requirement to support up to 15 simultaneous requests as the complete Government-furnished demand baseline?

Response: Data was available for September 2025 through June 2026:

Non-ambulatory one-way transports: 1,797 transports Non-ambulatory transport mileage: 30,716 miles Non-ambulatory transport wait time: Total 22 hrs Ambulatory one-way transports: 680 transports Ambulatory transport mileage: 12,761 Ambulatory Transport Wait Time: 143 hrs Total Cancelations: 181 cancelations Most transports are from the Veteran’s residence to community care facilities, CBOCs or main facility for medical appointments.

1. Please clarify whether the 30-minute requirement for unscheduled requests that do not originate at Saginaw VAMC is a dispatch-time requirement only or an arrival-at-pickup requirement. How should this requirement be applied to remote CBOCs, veteran residences, community-care facilities, and out-of-state origins where physical arrival within 30 minutes may be infeasible? Please also explain how the immediate-dispatch requirement for transports out of Saginaw VAMC relates to the separate 60-minute response allowance for add-on requests originating at Saginaw VAMC.

Response: This would be a dispatch-time requirement. For transports that do not originate at the Saginaw VAMC they have 30 minutes to dispatch and for transports that do originate at the Saginaw VAMC the response time is 30-minutes.

1. Please identify the minimum personnel configuration required for each transport mode. Are EMTs, paramedics, Critical Care Transport Paramedics, attendants, or other medical personnel required for routine ambulatory, non-ambulatory, and reclining-positioning trips, or only when specifically identified in an order? Please reconcile the Special Agreement Check requirement and annual fingerprinting language in PWS 4.3.1.5 with the National Agency Check with Written Inquiries and favorable-adjudication language in Section 12. May personnel begin performance while a VA investigation is pending? If an offeror requests additional mobilization time on D.6, will that request be evaluated as technically acceptable, conditionally acceptable, or noncompliant?

Response: Medical personnel are not required for routine transports. Saginaw VAMC currently does not utilize the NEMT vendor for transports that require medical personnel.

1. Please reconcile the general no-show provision, which permits a base rate plus accrued enroute mileage after dispatch, with the provision for transports out of Saginaw VAMC stating that no charge applies when cancellation occurs before the vehicle arrives on campus. Which rule controls for an outbound Saginaw request canceled after dispatch but before arrival? Which CLINs govern a reclining-positioning no-show? When VA staff and equipment must be returned Response: Enroute cancellations will be invoiced as one way trip rate and any accrued enroute mileage. There is no CLIN specific for no-show regardless It is the base trip cost of the Mode that was requested. Finally, this contract does not offer Government Furnished Equipment (GFE). Contractor will be required to provide all equipment and personnel to provide NEMT services. In the rare occasion that section 4.1.2 of the PWS applies equipment/personnel shall be returned and mileage accrued may be invoiced on to the trip, an additional trip cost shall not be invoiced.

1. to the original pickup location, or when a transport diverts to an emergency department, will the additional movement be separately ordered and compensated as an additional one-way trip and authorized mileage?

Response: Each trip will be invoiced as a trip base rate regardless of where the transport vehicle originates from.

1. Please confirm whether the included 30-minute wait-time grace period applies separately at pickup and drop-off or as one combined allowance for the entire one-way trip. Who is authorized to approve billable wait time, must approval be obtained before the charge is incurred or may it be validated after service, and are the 15-minute rounding rules applied separately at each location or to total wait time for the trip?

Response: Wait-time grace period and rounding rules apply separately. The COR will provide a list of authorized personnel or positions at pre-award meeting.

1. Please confirm whether recent non-emergent medical transportation, wheelchair, ambulette, paratransit, or other special-needs transportation experience under PSC V226 will be considered relevant. E.2 states that past performance will be limited to PSC V225 or Ambulance Service, while this solicitation is identified under PSC V226 and principally requires non-emergent transportation. Please also confirm whether references must transmit D.5 questionnaires directly to the Contracting Officer by the quotation deadline, whether offerors may include completed questionnaires within their quotation, and how CPARS records or references will be treated if a questionnaire is not returned Response: This error will be corrected. Past performance will only be considered relevant with a PSC code of V226 or equivalent to Ambulette transport services.

1. Please identify the controlling invoice frequency and submission system. B.1 states bi-monthly invoicing, the PWS references monthly invoices and submission within 30 days of service, and the solicitation references both VetRide or VA-approved software and the Tungsten electronic invoicing system. Please also reconcile the instruction to include full Social Security number, date of birth, diagnosis, and related patient data on the HCFA 1500 with the later instruction that invoices shall not include patient information. What data fields and secure transmission method are required? Finally, what VA cybersecurity, authorization, encryption, retention, and access standards must a commercial GPS portal satisfy before performance?

Response: VetRide will be the governing invoicing system.

1. Please identify the controlling wage determination or determinations for employees performing trips throughout the stated Michigan, Ohio, and Indiana service area. D.2 includes wage determinations for selected Indiana counties and Saginaw County, Michigan, but does not include Ohio or all listed service counties. Will the Government provide the remaining applicable wage determinations, and should the required wage rate be determined by the employee's duty station, the trip origin, the trip destination, or the locality in which the covered work is performed?

Response: The Wage Determination will be revisited and all applicable Wage Determination will be incorporated into attachment D.2.

1. Please identify the exact license or licenses intended by the requirement for an active Emergency, Medical, Fire, and Transportation Service License in the prime contractor's name, including the issuing jurisdiction or jurisdictions. May applicable NEMT, ambulette, wheelchair-transport, or medical-transport licenses collectively satisfy this requirement? Please also confirm the governing state insurance requirements because PWS Section 13 references Ohio, Kentucky, and Indiana while the stated service area is Michigan, Ohio, and Indiana. Does the medical-liability requirement apply to this NEMT scope, and does the asbestos-exclusion requirement in C.7 apply to this transportation contract?

Response: Michigan does not have an EMFTS license specifically. They may have other licenses required to operate ambulette services. It is the vendor’s responsibility to determine what licenses are required and obtain them. EMFTS license verbiage will be taken out of the RFQ.

1. Please confirm whether the 2,500-trip minimum guarantee applies only to the base period and may be satisfied by any mix of ambulatory, non-ambulatory, and reclining-positioning base-rate trips, excluding mileage and wait-time units. Please also reconcile C.6, which states that deliveries are not required after July 31, 2031, with Ordering Period 4, which runs through January 31, 2032. What is the final ordering and performance end date?

Response: Minimum guarantee only applies to Base Period. Any period after base period is optional and will be on the government need if exercised. This I common will all multiple-year contracts. C.6 will be updated to 1/31/2032.

1. Please confirm whether Amendment 0001 also amended CLIN 1006, which is shown as Reclining Positioning in the re-issued RFQ but is omitted from the CLIN list in Amendment 0001. Please also confirm whether the unit for CLIN 4005 is intended to be EA or should be TRP like the corresponding base-rate CLINs in the other periods. For reclining or tilt-positioning transports, must the contractor furnish the WC19-compliant chair and transfer the patient into that chair, or may the patient be transported in Response: All CLINS will be reviewed and any edits/corrections will be completed.

1. Please confirm that the July 23, 2026 re-issued RFQ under Solicitation 36C25026Q2655 is the controlling solicitation source and that the 36C25026Q5532 references on the table-of-contents pages are typographical errors. Please also identify the controlling pass/fail submission list and correct the inconsistent cross-references among E.1, E.2, and D.7, including the insurance reference to C.6 versus C.7, the FAR 52.229-11 reference to E.5 versus E.6, and the D.4 reference to FAR 52.219-14 versus VAAR 852.219-75. Is a separate Technical Approach narrative required, and may a complete quotation exceeding the 9 MB email limit be transmitted in consecutively numbered emails?

Response: 36C25026Q2655 is the RFQ number. All errors will be corrected with a second re-issue of the RFQ. As always, use the most updated SF1449 when submitting a quote.

1. If not brand new, could you please provide the current / previous contract number?

Response: Incumbent information provided in amendment 0001
