The purpose of this amendment is to provide Q&A responses, please see attached. This amendment also extends the due date for responses to Monday January 5, 2026 at 1pm EST.

1) Can the deadline for questions be extended due to the Holiday Week?

As the site visit is scheduled for December 16 &17, 2025, and to accommodate upcoming holidays the Q&A period as well as the due date for responses shall be extended. Questions will be accepted until 3pm EST December 17, 2025 & quotes are now due by January 5, 2026 @1pm EST. Both are to be submitted via email to tiera.sims@va.gov.

2) Would a site visit be possible to both locations prior to bid date?

Site visits shall take place on Tuesday December 16, 2025 @ 11am at Marion Campus 1700 E 38th St, Marion, IN 46953 Building 76 Room 200 POC Mike Turner- 765-674-3321 x73819 and Wednesday December 17, 2025 @ 11am Fort Wayne Campus 2121 Lake Ave, Fort Wayne, IN 46805 Building 2, POC Mike Foster – 260-888-9903 or Lukas Winkler – 260-466-8997. You do not need to contact POC’s regarding attendance, please contact the CO at tiera.sims@va.gov.

3) When is the last time these inspections and testing were done?

Inspections were last done in January 2025 at Marion campus and September 2025 at Fort Wayne campus.

4) We would like to know why is the pricing schedule on a monthly basis even though the services are to be done annually/triennially. Is that a mistake?

Pricing shall reflect the frequency of services as specified in the statement of work (SOW), schedule shall be adjusted to a 1-year period.
