About this file
This document is an amendment to a Request for Quote (RFQ) for Biohazardous Waste Removal and Disposal Services for the John D. Dingell VA Medical Center. The solicitation is set aside for small businesses, with a NAICS code of 562211 and a size standard of $47 million. The contract will cover a base period from January 2026 through December 2026, with four additional one-year option periods through 2030. The scope includes weekly waste removal from the main campus and bimonthly pickups from multiple community-based outpatient clinics (CBOCs) in Michigan, involving the collection, transportation, and disposal of various medical waste streams including sharps, pathological, trace chemotherapy, and regular medical waste.
Key details include a site visit scheduled for November 13, 2025, at 10 AM, with technical questions due by November 17, 2025, and quotes due by November 20, 2025 at 12:00 PM EDT. The contractor must provide a 53' refrigerated trailer for the main campus, maintain specific documentation and training requirements, and comply with DOT and state regulations for waste handling. The main campus generates between 11,090 and 14,482 pounds of waste monthly, with additional smaller volumes from seven clinic locations. Evaluation will be based on administrative requirements, pricing, technical approach, and past performance, with the government seeking the best overall value through a comparative evaluation process.

Morgan Stein Contracting Officer Questions with Responses:
1. Who is the current vendor?
RESPONSE: This is not a technical question, you can check SAM or FPDS
1. Is there any incumbent/recompete or this is a brand-new project? If yes, could you please provide us with their award number or even if the customer bought similar services in the past.
RESPONSE: This is not a technical question, you can check SAM or FPDS
1. Is the solicitation a new requirement set by the government or is this a contract currently being performed that the government is renewing. If this is a renewal, do you have the contract number associated with the old contract We are performing independent research on the Presolicitation (36C25026Q0064).
RESPONSE: This is not a technical question, you can check SAM or FPDS
1. We would like to know if this is a brand new contract OR if there is (was) an incumbent performing these services. If not brand new, could you please provide the current / previous contract number? What is the current total contract value? Can we answer that?
RESPONSE: This is not a technical question, you can check SAM or FPDS
1. Will the Government entertain the 53’ refrigerated trailer being stationary (for storage) versus exchanged weekly with each service?
RESPONSE: Yes, we would but reserve the right to decline
1. Would the Government allow the vendor to collect the medical waste containers/boxes from a route box truck off the trailer on a weekly basis versus exchanging the trailer each week due to the volume of containers each week as a 53’ trailer can hold many more boxes/containers than approximately 50 containers weekly? This method would also allow vendors to provide a much more competitive pricing structure without the necessity to exchange trailers each week that are not full.
RESPONSE: Yes, this seems like a reasonable plan
1. Who owns the trailer that is onsite? Is it owned by the current vendor?
RESPONSE: It is owned by the current vendor
1. Does the Government utilize disposable boxes in addition to reusable containers for the collection of medical waste?
RESPONSE: Yes, please see the RFQ
1. Can the required VA staff training be provided in an online format?
RESPONSE: Recertifications, yes. But initial training would need to be held either live or hybrid so that staff can ask questions.
1. Do the offsite clinic locations require reusable containers or disposable boxes or both for the collection of medical waste?
RESPONSE: Disposable boxes will work
1. Can you please review the two solicitations below. They appear to be associated with the same solicitation number? Please let me know if I am misreading it. Can you please clarify it they should be under separate solicitation numbers? I believe one is for Dayton VA and one for Detroit VA.
RESPONSE: This requirement is for Detroit
1. John D. Dingell VAMC - Main Campus 4646 John R. Street, Detroit MI 48201, Pick-Up Weekly Mondays Between 0700 – 1000. Is this a requirement or can this day change be depending on route schedule?
RESPONSE: We can be flexible there but not before 0800 and not after 1600.
1. We may not be able to guarantee the pickup days will fall on the 1st and the 15th between 0800 – 1000 for all non-acute locations. Please confirm that this is a requirement.
RESPONSE: We can be flexible there but not before 0800 and not after 1600.
1. Is electricity provided by the VA? How much amp is going to be provided?
RESPONSE: No, the trailer has its own generator and does not need building services.
1. Where does the trailer need to be stationed?
RESPONSE: The trailer is parked at the VA Waste Dock
1. Do the clinics require storage/trailers?
RESPONSE: No
1. What are the address for the Dearborn and Southfield?
RESPONSE:
· Southfield: 18000 W. Nine Mile, Southfield, MI 48075
· Dearborn: TBD.
1. Can you confirm all of the different type of waste that will be removed on each location?
RESPONSE: All cbocs will have sharps waste only. The Main campus will have it all.

Clinic Service - 1
All VA Clinics require pickup of biohazardous waste from the suite twice per month during the 1st and 3rd week of each month.
Typically services are routed every other week as opposed to the 1st & 3rd Week. Can this be updated to every other week, which is the industry standard?
No.
6
Main Campus Services - 1.i. & Clinic Service 7
The VA will provide all sharps cabinets and sharps containers.
There are several references to sharps containers, trace chemo and desktop containers, especially on page 7. Please confirm the VA will be providing all sharps and trace chemo containers. Contractors are only provide tubs and boxes for medical waste transportation and processing.
The VA will provide its own sharps containers. The contractor will provide the following:
a. Super pack used to pack sharps on the truck
b. Totes and lids to pack regulated medical waste
c. Boxes for incineration
d. Red Bag liners for the totes
7
Regulated Waste Packaging, Storage, and Shipping - 6 , 7, 11, & 13 (yellow containers and sharps containers)
6. Containers and boxes for antineoplastic, pathological, cytotoxic and trace chemotherapy shall be differentiated from those for regular medical waste to deter confusion among VA employees. The minimum requirements for these containers are that trace chemotherapy containers shall be yellow with an appropriate lockable lid. Antineoplastic, pathological, cytotoxic and trace chemotherapy waste containers will have labels that state incineration only.
Typically incinerate only waste includes both trace chemotherapeutic waste and pathological waste. During the site visit we noticed fiber boxes, that we are assuming are used for the pathological waste and trace chemotherapeutic waste. We did not see any 28 gallon yellow containers.
The VA provides its own inserts for sharps inserts but the contractor provide:
7. The collection containers provided by the Contractor for sharps, antineoplastic, pathological, cytotoxic, and trace chemotherapy waste shall be compliant with OSHA Bloodborne Pathogen Standards (39CFR 1910.130).
We will also note that on page 6/69 item number 3.i the bid specifies “The VA will provide all sharps cabinets and inserts.” Under Main Campus Services"
a. Super pack used to pack sharps on the truck
11. The Contractor shall provide yellow leak-proof plastic containers that meet state and federal requirements for routine accumulation and collection of antineoplastic, pathological, cytotoxic and trace chemotherapy wastes to include but not limited to the discard of gloves, gowns, sharps tubing, and any other disposable materials contaminated with hazardous drugs, blood, and body fluids.
Can you confirm that fiber boxes are acceptable for the waste requiring incineration (Trace-chemotherapeutic waste and pathological waste), since we did see those during the site inspection? & Can you confirm the VA acquires their own trace chemo sharps containers?
b. Totes and lids to pack regulated medical waste
c. Boxes for incineration
13. Desktop containers with secure lids for sharps and other small item collection.
d. Red Bag liners for the totes
7
Regulated Waste Packaging, Storage, and Shipping - 9
All containers shall have lids appropriate to their waste stream and meet VA, federal, and state regulations. The lids will also provide a visual and audible feature to ensure that staff have adequately secured the lid to the container. The lid shall also have tamper-resistant and secure features to meet federal regulations.
Can you please clarify the required audible feature for the container being referred to in this section. We are assuming a clicking sound would meet this requirement. If anything different, pls explain
Yes, the clicking sound would meet this requirement.
7
Regulated Waste Packaging, Storage, and Shipping - 10
For each pick-up of regulated medical waste, the Contractor shall provide a complete copy of the shipping papers for the transportation and disposal of said regulated medical waste. The shipping papers shall at least contain the Generator Copy of the manifest when the medical waste is picked up. The Contractor shall then provide the VA with a Generator Copy of the manifest signed by the disposal facility operator(s) within two (2) weeks after the medical waste is delivered to the disposal facility. The Contractor shall submit this paperwork within the established two (2) week timeline.
Manifests are typically provided with the monthly invoice., We would request this be 45 days to prevent costs for special handling to pull them early. The normal process is to run the invoices and attach the completed manifests each month with the invoice submission.
We can agree to this change across the board.
7
Disposal and Destruction - 3
The VA will not be charged for any contaminants that may appear in the biohazardous waste stream.
This requirement should be removed, the VA is responsible for the waste they are shipping from the cradle to the grave.
We can charge that statement like this: The VA is a mercury free site and is not responsible for contractors’ equipment if contaminated with mercury due to this fact.
The VA should also be conforming with the contractor’s waste acceptance protocol, since non-conforming waste is always the responsibility of the waste generator, not the transporter.
An alternative consideration may be that the VA would be granted the opportunity to retrieve non-conforming waste or make their own arrangements to have it removed at the VA’s expense within 48 hours.
8
Training & Reports - 3.b
b. Breakdown of each waste stream, including amounts transported for the quarter, training conducted for the medical center, with total students served. Waste streams are as follows:
Please provide the most recent 12 months reporting for the following waste streams:
The previous contract did not go into the level of detail that we want in the future. Please bid based on the RFQ.

Price/Cost Schedule
Clins ending 001AA-AB
We recommend adding a separate haul or transportation fee for the main hospital, as this structure will provide the lowest overall cost to the VA. Currently, pricing includes disposal and the trailer, but without a defined haul fee, disposal rates will inevitably be inflated to cover transportation.
Refer to advised CLINS
It was noted during the pre-bid meeting that the current service frequency is every other week—more often than weekly. This reduced frequency would generate significant cost savings for the VA if a separate haul fee were established. Without this transparency, the incumbent would also have an unfair advantage, as other bidders are unaware of the exact haul frequency and would be forced to assume a higher number of pulls.
By listing the haul as a fixed, per-service line item, the VA captures the savings directly when the trailer is hauled less frequently, rather than the hauler retaining those savings. This approach ensures competitive, fair, and fully transparent pricing for all bidders.
26-Sep
Price/Cost Schedule
Clins ending 001AC-AH
For line items 0001AC through 0001AH, we recommend converting the pricing structure from a per-pound rate to a firm-fixed monthly fee. Each CBOC is scheduled for two pickups per month (26 annually), yet most generate very low volumes—often 0 to 50 lbs. per month, with several pickups resulting in no waste at all. Under the current per-pound model, contractors incur the fixed cost of performing the service without compensation when no waste is present.
Refer to advised CLINS
Because the majority of contractor costs—labor, vehicle, fuel, scheduling, compliance, and insurance—are fixed per trip, not based on weight, a monthly flat fee provides a more accurate and competitive pricing structure. This model also gives the VA predictable budgeting, eliminates wide fluctuations tied to low poundage, and avoids inflated per-pound rates designed to offset undercompensated trips. A firm-fixed monthly rate is the industry-accepted approach for low-volume CBOCs with scheduled service.
If a monthly pricing model cannot be adopted, we respectfully request a 12-month historical report for each location, including:
Note we typically see a per container or per month rate for CBOCs, not per pound due to the low volumes.
26-Sep
Price/Cost Schedule
Clins ending 001AC-AH
If a monthly pricing model cannot be adopted, we respectfully request a 12-month historical report for each location, including:

Number of pulls completed
This data is necessary for all bidders to prepare accurate, fair, and competitive pricing based on actual service history.
