STATEMENT OF WORK
Repair Steam Vents - 583-25-517
1. Introduction City steam is vented through a shallow concrete trench into risers from the main steam vault. Three vent pipes connected to relief valves ranging from 35 psi to 110 psi are located in the trench.
The three steam vent pipes have failed, leading to a critical situation where steam is backing up into the mechanical space, creating a hazardous work environment.
Immediate action is required to prevent further risks. Steam is also venting openly through the concrete shallow trench, posing a potential threat to VA employees entering/exiting the Richard L. Roudebush Veterans Affairs Medical Center (VAMC) through the south basement entrance.
2. Objective The main objective of this project is to remove the three (3) failed steam vent pipes, merge the remaining sections of the three steam vent pipes into one main collector in the mechanical space, and connect the collector to an existing riser.
3. Period of Performance and Work Hours The contractor shall complete all the requirements described in this scope of work within 30 calendar days after the notice to proceed.
Maintaining the medical center's operational capability is paramount, and the contractor shall coordinate all activity schedules with the COR prior to execution and two weeks in advance. According to the nature of the work, this project can be conducted during regular business hours and/or after-hours/weekends. Regular business hours are 7 AM to 4 PM, Monday through Friday. Work requiring outages shall be completed after hours or on weekends as coordinated with the COR.
4. Scope of Work The contractor shall provide all necessary services, supervision, labor, transportation, supplies, equipment, tools, licenses, chemicals, and materials to execute the following:
a. All work shall be completed during the non-heating season to avoid or minimize medical center operational disruptions.
b. Work shall be phased based on the available shutdown time windows (five hours or less) and VA-approved shutdown dates.
c. c. Shutdowns proposed by the contractor shall be properly coordinated with the affected services in advance and approved by the VA.
d. Plan, implement, monitor, and adjust all the required site-specific safety measures to accomplish the work needed in compliance with OSHA.
e. The main steam vault is under high-temperature conditions, and the contractor must comply with the OSHA heat stress regulation.
f. The main steam vault is a permit-required confined space (PRCS), and the contractor must comply with the OSHA confined space regulation and VA PRCS program.
g. Plan, implement, monitor, and adjust all the required safety measures to accomplish the work in a PRCS under high-temperature conditions (i.e., organizational measures, collective and personal protective equipment, rescue team, air and temperature monitoring, trained personnel, etc., as needed).

i. Excavate and clean debris from the concrete shallow trench and remove it off-site.
j. Cut the three (3) failed vent pipes in the mechanical space and at the riser and remove them off-site. Vent pipe sizes are 10-inch, 8-inch, and 6-inch in diameter.
k. Remove the 8-inch and 6-inch risers off site.
l. Complete Non-Destructive Testing (i.e., X-ray inspection) to determine if the metal thickness of the existing 10-inch riser is enough to weld on.
m. Merge the remaining sections of the three (3) steam vent pipes into a 10-inch collector inside the steam vault.
n. Provide and install enough and appropriate pipe supports/stands for the new steam vent pipe system.
o. The new steam vent collector shall go through one of the existing wall penetrations and follow the path of the existing steam vent pipes up to the 10-inch riser.
p. Connect the steam vent collector to the existing 10-inch riser.
q. Pipe material shall be ASTM A106, Grade B, seamless carbon steel, schedule
40, butt weld. All pipe connections shall be welded.
r. Close the two (2) unused concrete wall penetrations on both sides of the wall with stainless-steel plates and stainless-steel anchors for corrosion resistance. A high-temperature resistance gasket shall be installed between the concrete wall and the stainless-steel plates.
s. Dimensions and details are based on the best available information. However, the contractor shall not strictly rely on the provided information. The contractor shall verify existing site conditions, locations, dimensions, quantities, and accuracy.
t. The contractor shall provide, maintain, and adjust safety equipment and measures to protect construction workers and the public. This shall include additional equipment and measures that may be required by the COR or the VA Safety Service during the contract.

Federal Acquisition Registrations require that federal contractors register in SAM at www.sam.gov and enter all mandatory information into the system.
Warning: Award cannot be made to unregistered contractors. Registration must be active at time of bid opening and award. Bid(s) received by any unregistered offeror(s) shall be rejected.
THE EXCLUDED PARTIES LIST SYSTEM (EPLS): To ensure that the individuals or businesses providing services under the contract have not engaged in fraud or abuse regarding Sections 1128 and 1128A of the Social Security Act regarding federal health care programs, the contractor is required to check the Excluded Parties List System (EPLS) located at www.sam.gov for each person or business providing services under this contract. During the performance of this contract the Contractor is prohibited from using any individual or business listed on the List of Excluded Individuals/Entities.
VETS-4212 FEDERAL CONTRACTING REPORTING SYSTEM: Contractors are required to submit a required annual Form VETS-4212, Federal Contractor Veterans' Employment Report in all cases where the contractor or subcontractor has received an award of $100,000 or more in any fiscal year. Report submission link:
https://www.dol.gov/agencies/vets/programs/vets4212. The rules and regulations can be found here: https://www.dol.gov/agencies/vets/programs/vets4212. Contracting Officers are prohibited from awarding a contract to a contractor that has not submitted a required VETS-4212 Report with respect to the preceding fiscal year if the contractor was subject to the reporting requirement of 38 U.S. Code, Section 4212, codified at 41 CFR Section 61-300. The level of background investigation is commensurate with the level of access needed to perform the statement of work. This requirement is also applicable to all subcontractor personnel requiring the same level of access.
Levels and associated types of Background Investigations:
If the project requires that workers have only limited physical access to the VA facility for a period of one year or less, a local flash badge with fingerprint check is issued.
If the project requires that workers require un-supervised, logical and/or physical access for 180 consecutive or aggregate days in a 365-day period, over a 3-year period, a background investigation is required (Special Agency Check (SAC)) which must be completed prior to being allowed access.
If the project requires that workers have un-supervised, full-time, logical, and/or physical access for more than 6 months or more than 180 aggregate days in a one-year period, a background investigation is required (Special Agency Check and National Agency Check with Written Inquiries (NACI)) and access is not allowed until a favorable SAC completion.
The contractor bears all costs for SAC and NACI checks. The VA may, at its discretion, make deductions against the contract to recoup the investigation costs. The current cost for a SAC check is $25.00 per employee, and the current cost for a NACI check is $177.00 per employee (subject to change at any time).
MEDICAL CENTER ACCESS: Anyone participating in the site visit, or for any other reason that involves visiting the medical center related to this solicitation, must have a valid ID on your person and your vehicle must be properly registered and insured.

E-VERIFY SYSTEM: Companies awarded a contract with the federal government shall enroll in E-Verify within 30 days of the contract award date. They shall also begin using the E-Verify system to confirm that all of their new hires and employees directly working on federal contracts are legally authorized to work in the United States. E-Verify is an Internet-based system that allows an employer to use information reported on an employee's Form I-9 to determine the eligibility of that employee to work in the United States. There is no charge to employers for using E-Verify. (FAR 52.222-54).

REVIEW REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE
ASSESSMENT SYSTEM (CPARS)
(a) As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all construction contracts that exceed $700,000 and shares those evaluations with other Federal Government contract specialists and procurement officials.
The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement VA uses an online database, CPARS, which is maintained by the Naval Seal Logistics Center in Portsmouth, Virginia. CPARS has connectivity with the Past Performance Information Retrieval System (PPIRS) database, which is available to all Federal agencies. PPIRS is the system used to collect and retrieve performance assessment reports used in source selection determinations and completed CPARS report cards transferred to PPIRS.
CPARS also includes access to the federal awardee performance and integrity information system (FAPIIS). FAPIIS is a web-enabled application accessed via CPARS for contractor responsibility determination information.
(b) Each contractor whose contract award is estimated to exceed $150,000 is required to register with CPARS database at the following web address:
www.cpars.csd.disa.mil. Help in registering can be obtained by contacting Customer Support Desk at DSN: 684-1690 or COMM: 207-438-1690.
Registration should occur no later than thirty days after contract award and must be kept current should there be any change to the contractor's registered representative.
(c) For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the contractor's performance annually.
Interim reports will be filed each year until the last year of the contract, when the final report will be completed. The report shall be assigned in CPARS to the contractor's designated representative for comment. The contractor representative will have thirty days to submit any comments and re-assign the report to the VA contracting officer.
Failure to have a current registration in the CPARS database, or to re-assign the report to the VA contracting officer within those thirty days, will result in the Government's evaluation being placed on file in the database with a statement that the contractor failed to respond.”
